Release form and composition
Dosage form - gel for external use: almost transparent, colorless or with a slight yellowish tinge, with a characteristic odor (30 and 50 g each in aluminum tubes, in a cardboard box 1 tube and instructions for using Heparin 1000).
Composition of 1 g of gel:
- active substance: sodium heparin - 1000 international units (ME);
- auxiliary components: lavender oil, orange blossom oil, carbomer, methyl parahydroxybenzoate, trometamol, ethanol 95%, water.

Pharmacodynamics
The active substance of the drug - sodium heparin - is a direct-acting anticoagulant from the group of medium-molecular heparins.
The drug prevents blood clots. By inhibiting the activity of hyaluronidase, it causes the regeneration of connective tissue. Activates the fibrinolytic properties of blood.
Gel for external use Heparin 1000 has a decongestant, anti-inflammatory and local antithrombotic effect, and also activates tissue metabolism and improves blood microcirculation. These properties of the drug accelerate the resorption of hematomas, reduce tissue swelling, and prevent the formation of blood clots.
Pharmacokinetics
Heparin 1000 is applied externally, therefore, if it enters the systemic circulation, then a small amount of the drug. In this regard, the systemic effect of heparin is not manifested, no changes in blood coagulation parameters are noted. The substance does not penetrate the placental barrier.
Indications for use
- subcutaneous hematomas, including after phlebectomy and vein surgery;
- bruises and blunt trauma of soft tissues;
- localized edema and soft tissue infiltrates;
- diseases of superficial veins: varicose veins, chronic venous insufficiency and complications caused by it, such as superficial periphlebitis and superficial thrombophlebitis.
Contraindications
- thrombocytopenia;
- increased tendency to bleeding;
- age up to 18 years;
- violation of the integrity of the skin at the site of application of the gel (ulcerative necrotic lesions or open wounds);
- hypersensitivity to Heparin 1000.
Heparin 1000, instructions for use: method and dosage
Gel Heparin 1000 is used externally: a small amount (strip 3‒10 cm) is applied in a thin layer to the skin and gently rubbed. Frequency rate of applications - from 1 to 3 times a day.
Duration of treatment:
- localized consequences of bruises and injuries (edema, infiltration, hematoma of soft tissues): until symptoms disappear;
- initial symptoms of venous insufficiency (venous edema, pain and heaviness in the legs): from 1 to 3 weeks, depending on the clinical situation;
- varicose veins, as well as superficial thrombophlebitis or superficial periphlebitis as a complication of chronic venous insufficiency: from 4 to 6 weeks, depending on the course of the disease.
The decision to prolong treatment or to repeat courses is made by the attending physician.
Side effects
Heparin 1000 is well tolerated. In very rare cases, with individual intolerance to the drug, hypersensitivity reactions develop: redness of the skin, itching. After discontinuation of therapy, these symptoms usually go away on their own.
Overdose
Due to the low systemic absorption of the drug with external use, overdose is almost impossible.
If the recommended dose is exceeded, the action of heparin can be neutralized with a solution of protamine sulfate.

Gel Heparin 1000 should not be applied to open wounds and mucous membranes, as well as applied to areas with purulent lesions.
The drug is not recommended for the treatment of deep vein thrombosis.

Pediatric use
Heparin 1000 is not used for the treatment of patients under the age of 18 due to lack of sufficient experience.
