Release form and composition
Dosage forms of release:
- Rectal suppositories: of a uniform consistency, yellowish-white color, have a conical shape with a pointed end, a diameter of up to 1 cm (5 or 10 pieces of one concentration in blisters, 1 package in a cardboard box; set of 5 pieces of different concentrations (2 pcs. 100 mcg, 1 pc. 150 mcg, 2 pcs. 200 mcg) in blister strip packs, 1 package in a cardboard box; set of 12 pcs. Of different concentration (3 pcs. 50, 100, 150 and 200 μg) in blisters, 1 package in a cardboard box);
- Solution for intramuscular injection: colorless, transparent (1 ml in ampoules, 10 ampoules of the same concentration in a cardboard box, complete with an ampoule knife).
The cardboard box also contains instructions for the use of Pyrogenal.
1 suppository contains:
- Active ingredient: bacterial (from Salmonella typhi cells) lipopolysaccharide - 50, 100, 150 or 200 μg;
- Additional components: lanolin, cocoa butter, purified water.
The composition of 1 ml of injection solution includes:
- Active ingredient: bacterial (from Salmonella typhi cells) lipopolysaccharide - 10, 25, 50 or 100 mcg;
- Additional components: buffered phosphate-saline solution (pH 6.7-7.3).
Pharmacological properties
Pharmacodynamics
Pyrogenal is a highly active nonspecific immunomodulator of a wide spectrum of action, causing a whole complex of complex physiological and immunological changes in the body. The main one is the activation of several systems: reticuloendothelial, fibrinolytic, and hypothalamic-pituitary.
It has an adjuvant, desensitizing and anti-inflammatory effect, affects the thermoregulatory centers of the hypothalamus, and also increases the general / specific resistance of the body.
Thanks to the use of Pyrogenal, macrophages are activated and phagocytosis is enhanced; stimulates the production of interleukin-1 (IL-1), which causes the proliferation of a number of body cells (endothelial cells, fibroblasts, hematopoietic cells, etc.) and interleukin-2 (IL-2), which is necessary to maintain the growth of lymphocytes (mainly T cells); tumor necrosis factor (TNF), induction of endogenous interferon (IF), production of reactive oxygen species are stimulated. With an increase in the functional activity of phagocytes, an increase in the antimicrobial resistance of the organism occurs, which leads to an acceleration of the formation of antibodies.
Due to the activation of macrophage-monocytic cells and the cytokines secreted by them, an increase in the functional activity of the cellular and humoral immune response is noted.
Under the influence of the drug in the connective tissue, the development of fibroblasts and the formation of collagen fibers is suppressed. As a result, the cellular composition of loose connective tissue is rejuvenated, and in the nervous tissue, the formation of glial scarring is inhibited.
The use of Pyrogenal promotes the resorption of previously formed dense scars and adhesions at the sites of damage, prevents the formation of new ones, which stimulates epithelization and tissue regeneration, and prevents vascular obliteration.
Therapeutic effects of Pyrogenal:
- stimulation of hyaluronidase activity;
- improving the fibrinolytic properties of blood;
- increased activity of lysosomal enzymes;
- stimulation of the kinin system;
- ensuring an increase in tissue permeability;
- acceleration of the penetration of the drugs used into the lesion;
- promoting the process of restoring blood circulation and resorption of foci of inflammation;
- activation of the function of the pituitary gland - adrenal cortex;
- increased concentration of hormones in the blood.
Pharmacokinetics
After administration of the drug, the substance is fixed on the surface of circulating blood leukocytes (up to 85–90% of the dose). Approximately 10% remains in circulating plasma and approximately 5% is adsorbed on erythrocyte membranes.
With the introduction of Pyrogenal in the latent period (from 45 to 90 minutes), leukopenia develops, which is subsequently replaced by leukocytosis, which lasts for 24–48 hours.
The drug is excreted from the body in the urine.
Indications for use
All dosage forms
- Chronic liver disease;
- Burn disease;
- Strictures of the ureters and urethra;
- Adhesive disease of the abdominal cavity;
- Chronic prostatitis and urethritis;
- Venereal diseases;
- Secondary infertility;
- Diseases of the uterine appendages of inflammatory etiology.
Suppositories (optional)
- Human papillomavirus infection;
- Incomplete recovery after acute viral and bacterial diseases (immunorehabilitation and immunoprophylaxis).
Solution for injection (optional)
- Traumatic injuries of the peripheral and central nervous system;
- Herpesvirus eye infection;
- Sluggish tuberculosis in the resorption phase;
- Chronic skin conditions, including psoriasis;
- Iridocyclitis;
- Corneal opacity;
- Uveitis;
- The need for pyrotherapy (as a pyrogenic drug).
Contraindications
All dosage forms
- Febrile diseases in acute course;
- Pregnancy;
- Hypersensitivity to the components of Pyrogenal.
Solution for injection (optional)
- Chronic diseases in the stage of decompensation and exacerbation, except for diseases that are an indication for the appointment of Pyrogenal;
- Diseases of the blood;
- Lactation period.
For chronic diseases of the cardiovascular system, kidneys, as well as diabetes, Pyrogenal is prescribed under the supervision of a physician.
In the presence of anamnestic data on febrile seizures or in convulsive readiness, the use of the drug is possible against the background of anticonvulsant treatment.
Pirogenal, instructions for use: method and dosage
Suppositories
In the form of rectal suppositories, Pyrogenal is used rectally.
Single dose - 1 suppository per day. The drug, depending on the doctor's prescription, is used daily or every other day.
Single initial / maximum dose - 50/200 mcg.
If there are no other doctor's prescriptions, Pyrogenal suppositories are used with a gradual increase in the dose: 50, 100, 150 and 200 μg, while suppositories of the same concentration should be used 3 times (a total of 12 pieces per course).
If the drug is used as monotherapy for immunorehabilitation and immunoprophylaxis with incomplete recovery after acute bacterial and viral diseases, concentrations of 50 or 100 μg are usually prescribed (5-10 suppositories per course).
According to the doctor's prescription, combined use with injection of Pyrogenal is possible.
Injection
Pyrogenal solution in ampoules is administered intramuscularly.
Frequency rate of application - 1 time per day. Injections should be given every other day. If necessary, before administration, the drug is diluted with 0.9% sodium chloride solution for injection. The solution cannot be stored in an opened ampoule.
The doctor prescribes the therapeutic dose individually.
The initial adult single dose is 2.5 mcg. In the future, it is gradually increased by 2.5-5 μg. The maximum is 100 mcg per day.
For children, Pyrogenal is prescribed in a single dose of 0.5-1.5 μg. In the future, it is gradually increased by 0.5-1.5 μg or 2.5 μg (depending on age). The maximum is 25-30 mcg per day.
The duration of the course is 10-30 injections for adult patients and 10-15 injections for children. Repeated courses are possible in 2-3 months.
If Pyrogenal is prescribed for pyrotherapy purposes, the solution should be administered 1 time per day. Injections as prescribed by a doctor are carried out in 1-2 days. The initial dose is 5-10 mcg, it is gradually increased to 100-150 mcg.
Side effects
As a rule, Pyrogenal is tolerated without the development of adverse reactions.
Suppositories
In some cases, a slight increase in temperature (up to 37-37.6 ° C) and the development of mild malaise are possible.
Injection
Possible side effects:
- Rarely: an increase in temperature up to 37.5 ° C, proceeding with mild malaise and chills. This condition can last 3-8 hours. The dose that caused it should be repeated in the following days until the reaction to its introduction stops (usually 1-3 injections), then the dose is gradually increased;
- Very rare: an increase in temperature up to 39.5 ° C, proceeding with severe chills, headache, back pain. This condition can last 6-8 hours, then it goes away on its own. In this case, the dose must be reduced.
The reactions described above when using Pyrogenal for pyrotherapy are allowed.
Overdose
There is no information.
special instructions
For children under 5 years of age and patients over 60 years of age, Pyrogenal injection solution should be prescribed in a reduced dose. Its increase is possible only under the supervision of a specialist and with good tolerability of the drug.
Application during pregnancy and lactation
Pyrogenal solution in ampoules during pregnancy / lactation is not prescribed.
Suppositories are contraindicated for use during pregnancy.
Pediatric use
For children under 5 years of age, Pyrogenal is prescribed in a reduced dose. Its increase can be carried out with good tolerance of the drug under medical supervision.
With impaired renal function
In the presence of chronic kidney disease, Pyrogenal should be used under medical supervision.
Use in the elderly
For patients over 60 years of age, Pyrogenal is prescribed in a reduced dose. Its increase can be carried out with good tolerance of the drug under medical supervision.
Drug interactions
Pyrogenal can be combined with all drugs that are usually used to treat these diseases.
When used as part of a complex therapy, Pyrogenal enhances the effect of specific drugs used in chemotherapy.
