Release form and composition

Phlebofu is produced in the form of tablets: biconvex, oblong, on one side with a risk, from gray-brown to gray-yellow color with possible inclusions of lighter and darker shades (10 pieces in a blister strip, in a cardboard box 1-10 packs; 10, 20, 30, 40, 50, 60 or 100 pcs.   • active substance: diosmin - 0.6 g (in terms of the anhydrous substance);

Pharmacological properties

Pharmacodynamics

Diosmin is a benzopyrone derivative; belongs to the group of bioflavonoids, has a phlebotonic and venoprotective effect, reduces the permeability of the vascular wall. The substance helps to reduce the extensibility and increase the tone of the veins, to reduce venous stasis in the lower extremities. The venotonic effect of the drug is dose-dependent, its mechanism is not fully understood. It is assumed that the active substance of Phlebofa blocks the activity of catechol-O-methyltransferase, leading to the breakdown of norepinephrine, which increases venous tone. This hypothesis was confirmed by experimental studies under ex vivo conditions, during which diosmin increased the vasoconstrictor effect of serotonin, adrenaline, and norepinephrine on separated fragments of the great saphenous vein and superficial veins of the forearm. Also, due to the effect of the substance, a decrease in the serum concentration of enzymes that regulate the metabolism of mucopolysaccharides in the venous wall was noted.

According to the data of clinical studies, diosmin helps to reduce the capacity of the venous bed and the volume of venous stasis (according to the results of occlusive plethysmography), to reduce the average pressure in the superficial and deep veins of the legs (according to the results of Doppler ultrasound) and to increase the systolic and diastolic blood pressure (BP) against the background of postoperative orthostatic hypotension. By increasing the frequency of contraction and tone of the lymphatic capillaries, increasing their functional density and reducing lymphatic pressure, the drug enhances lymphatic drainage.

Due to the effect of diosmin, the processes of blood microcirculation are improved - the resistance of the capillaries increases (it has a dose-dependent nature) and their permeability decreases. The substance helps to reduce the adhesion of leukocytes to the venous wall and their movement into paravenous tissues, improves oxygen diffusion and perfusion in the skin tissue. Possessing anti-inflammatory effect, Phlebofa inhibits the production of prostaglandins and thromboxane and the production of free radicals.

Pharmacokinetics

After taking Phlebof's tablets, diosmin is rapidly absorbed from the gastrointestinal tract (GIT) and is detected in the blood plasma 2 hours after administration. Oral bioavailability of the substance is about 40-57.9%. In the process of metabolic transformation of a part of the drug with the participation of cecum bacteria, hippuric and benzoic acids are formed. The maximum concentration of the substance in the plasma is observed 5 hours after ingestion. The accumulation of diosmin occurs in all the walls of the saphenous veins of the lower extremities and vena cava, to a lesser extent in the lungs, liver, kidneys and other tissues.

The volume of distribution is 62.1 liters, the maximum value of the selective accumulation of diosmin and / or its metabolites is noted in the vein wall 9 hours after administration and remains for 96 hours. The agent is rapidly biotransformed in the liver to form the main metabolite, hydroxyphenylpropionic acid. Diosmin metabolites are excreted mainly by the kidneys in the form of conjugates with glucuronic acid. 79% of the taken substance is excreted by the kidneys, 11% by the intestines, 2.4% by the bile.

Phlebofa is characterized by enterohepatic circulation. After ingestion of radioactively labeled diosmin, about 86% is excreted by the intestines and kidneys within 48 hours.

Indications for use

  • varicose veins of the lower extremities (to eliminate symptoms);
  • chronic form of lymphovenous insufficiency of the lower extremities (to eliminate symptoms);
  • acute hemorrhoids (in combination therapy to relieve symptoms);
  • disorders of microcirculation processes (for example, in the case of idiopathic edema).

Contraindications

  • age up to 18 years;
  • I trimester of pregnancy;
  • lactation period;
  • hypersensitivity to the constituents of the product.

Instructions for use of Phlebofe: method and dosage

Phlebof tablets are taken orally.

The recommended dosage regimen, taking into account the indications:

  • varicose veins of the lower extremities: take on an empty stomach in the morning, 0.6 g (1 tablet), the course is on average 2 months;
  • chronic lymphovenous insufficiency: take 0.6 g on an empty stomach in the morning, average course - 2 months; in severe forms of the disease (convulsions, pain, swelling, etc.), it is possible to prescribe a longer course, but the total duration of continuous therapy should not be more than 4 months, repeated courses are allowed at intervals of 2-3 months; when treating a disease in the II and III trimesters of pregnancy, the drug is taken once a day at a dose of 0.6 g, the duration of admission is no more than 1 month, therapy must be completed 2-3 weeks before childbirth;
  • exacerbation of hemorrhoids: taken with meals, daily dose - 1.2-1.8 g (2-3 tablets), course - 7 days, further, if necessary, it is possible to take 0.6 g once a day (1 tablet) for 1–2 months;
  • microcirculation disorders: take 0.6 g on an empty stomach in the morning, course - 1-2 months.

In case of missing one or more receptions of Phlebofa, treatment should be continued as usual at the recommended dosages.

Side effects

  • central nervous system: rarely - dizziness, malaise, headache;
  • Gastrointestinal tract: often - vomiting, heartburn, nausea, constipation, diarrhea, dyspeptic disorders; infrequently - colitis;
  • allergic reactions: rarely - urticaria; with an unknown frequency - isolated edema of the lips, eyelids, face; extremely rare cases - angioedema;
  • skin and subcutaneous fatty tissue: rarely - itching, skin rashes.

Overdose

Overdose cases have not been established. Due to the wide therapeutic range of diosmin, the probability of intoxication with its overdose is insignificant. There is no specific antidote.
