Release form and composition
Dosage forms of release of the drug Celestoderm-B with garamycin:
- ointment for external use: soft, from light yellow to almost white, homogeneous, without foreign inclusions (in aluminum tubes of 15 or 30 g, in a cardboard box 1 tube);
- cream for external use: white, homogeneous, soft, free from foreign inclusions (in aluminum tubes of 15 or 30 g, in a cardboard box 1 tube).
Active substances in the composition of 1 g of ointment / cream:
- betamethasone 17-valerate - 0.001 22 g (corresponds to the content of betamethasone - 0.001 g);
- gentamicin sulfate - 0.001 g [equivalent to 0.001 g, or 1000 IU (international units) gentamicin].
Auxiliary components that make up Celestoderm-B with garamycin:

Pharmacodynamics
The preparation Celestoderm-B with garamycin contains betamethasone valerate, a glucocorticosteroid with anti-inflammatory, antipruritic and vasoconstrictor action, and gentamicin, an antibiotic with broad-spectrum antibacterial activity. Bacteria susceptible to gentamicin include
sensitive strains of Staphylococcus aureus (coagulase-positive, coagulase-negative and some penicillinase-producing strains), as well as gram-negative bacteria - Klebsiella pneumonia, Proteus vulgaris, Escherichia coli, Enterobacter aerogenes, Pseudomonas aeruginosa.
Indications for use
Celestoderm-B cream and ointment with garamycin is used for local therapy of skin pathologies that can be treated with glucocorticosteroids, in the presence of a secondary infection caused by microorganisms susceptible to gentamicin, or in the presence of suspicions of such an infection:
- anogenital and senile itching;
- intertrigo;
- neurodermatitis;
- eczema (children's, atopic, coin-like);
- psoriasis;
- dermatitis (contact, seborrheic, solar, exfoliative, radiation).
The preparation in the form of a cream is prescribed for patients with diseases of wet or oily skin; Celestoderm-B ointment - with skin lesions with eczema or psoriasis.
Contraindications
Absolute:
- fungal skin diseases, herpes simplex, chicken pox, skin manifestations of syphilis, skin tuberculosis;
- post-vaccination skin reactions and vaccination period;
- age less than 6 months;
- long-term therapy of large areas of skin during pregnancy;
- period of breastfeeding;
- individual intolerance to the components contained in the preparation.
Relative (diseases / conditions in the presence of which the appointment of the drug Celestoderm-B with garamycin requires caution):
- pregnancy, especially the first trimester;
- long-term therapy of large areas of the skin, especially in children;
- the presence of skin cracks;
- the use of occlusive dressings.
Instructions for the use of Celestoderm-B with garamycin: method and dosage
The ointment / cream is applied externally: a thin layer is applied to the affected skin. The frequency of use of the drug is 2 times a day (morning and evening).
The frequency of use is established by the doctor taking into account the severity of the pathology. In mild cases, it is usually sufficient to use Celestoderm-B with garamycin once a day, in more severe cases, more often.
The duration of the course of therapy directly depends on the patient's response to the drug, the location of the lesion and the degree of its severity.
Side effects
During the period of therapy with Celestoderm-B with garamycin, a violation of skin pigmentation, the development of a rash and hypersensitivity reactions are possible.
Side effects due to betamethasone (especially when using occlusive dressings):
- irritation, burning sensation, dryness, maceration, or atrophy of the skin;
- folliculitis;
- hypertrichosis;
- steroid acne;
- hypopigmentation;
- perioral dermatitis;
- allergic contact dermatitis;
- secondary infection;
- striae;
- prickly heat.
On the background of the use of gentamicin, transient skin irritation may occur, which usually does not require discontinuation of therapy.
In cases of prolonged use of Celestoderm-B with garamycin or its application to large areas, systemic side effects characteristic of glucocorticosteroids may develop:
- violation of the menstrual cycle;
- insomnia;
- excitation;
- hyperglycemia;
- exacerbation of hidden foci of infection;
- ulceration of the mucous membrane of the gastrointestinal tract;
- swelling;
- increased blood pressure;
- osteoporosis;
- increase in body weight.
Children may also develop side effects due to betamethasone:
- increased intracranial pressure (bilateral edema of the optic nerve head, headache, bulging fontanelle);
- lag in weight gain;
- growth retardation;
- Itsenko-Cushing's syndrome;
- suppression of the function of the hypothalamic-pituitary-adrenal system (including the lack of response to stimulation of adrenocorticotropic hormone, a decrease in the concentration of cortisol in the plasma).
Overdose
The main symptoms: excessive or prolonged use of local glucocorticosteroids or the use of doses exceeding the recommended ones can serve to suppress the function of the pituitary-adrenal system, which may cause symptoms of hypercortisolism, including Itsenko-Cushing's syndrome, and develop secondary adrenal cortex insufficiency. With a single overdose of gentamicin, no symptoms were observed. Prolonged use of the drug or its use in doses exceeding the recommended ones can lead to a significant growth in the lesion focus of insensitive microflora, including fungal ones.
Therapy: symptomatic treatment is prescribed. Acute symptoms of hypercortisolism are reversible in most cases. If necessary, correct electrolyte imbalance. With chronic toxic effects, treatment is gradually canceled. If there is an uncontrolled growth of insensitive microorganisms or the development of a fungal infection, the therapy is stopped and the appropriate treatment is selected.

With topical use, the systemic absorption of gentamicin may increase when applied to large areas of the skin, especially with prolonged therapy or the presence of skin cracks. In such cases, side effects characteristic of gentamicin with its systemic use may develop, and therefore its careful use is recommended, especially in children, which requires a doctor's determination of the frequency of use and the duration of treatment.
There are reports of cross-allergic reactions to antibacterial agents from the aminoglycoside group.
With prolonged local use of gentamicin, the growth of insensitive microflora, including fungal ones, is rarely observed. In such cases, as well as in the event of irritation, hypersensitivity reactions and superinfection, therapy is canceled and appropriate treatment is prescribed.
Celestoderm-B with garamycin is intended exclusively for external use and should not be used in ophthalmology.
Application during pregnancy and lactation
According to the instructions, Celestoderm-B with garamycin is contraindicated during breastfeeding. During pregnancy, long-term drug therapy of large areas of the skin is contraindicated.
The drug is used with caution during pregnancy, especially in the first trimester.
Pediatric use
The drug is contraindicated in children under 6 months of age.
