Strepsils Intensive is a non-steroidal anti-inflammatory drug (NSAID) for topical use in ENT practice.
Release form and composition
Dosage forms of Strepsils Intensive:
- dosed spray for topical application: clear solution from colorless to pale yellow [15 ml (at least 15 doses) in a white opaque polyethylene bottle, with a dosing device and a cap on top; in a cardboard box 1 bottle];
- honey-lemon or orange lozenges (sugar-free): round, from a translucent caramel mass, from pale yellow to pale brown (honey lemon) or from white to pale yellow (orange), tablets on both sides with the image of the letter S; in the caramel mass there may be bubbles and a slight unevenness of the edges, the appearance of a white bloom is allowed (4, 6, 8, 10 or 12 pieces in a blister; in a cardboard box 1, 2, 3, 4 or 5 blisters).

- active substance: flurbiprofen - 8.75 mg;

Pharmacological properties
Pharmacodynamics
Flurbiprofen belongs to the group of NSAIDs, propionic acid derivatives. The active substance has an analgesic, antipyretic and anti-inflammatory effect as a result of inhibition of cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2), with a certain selectivity in relation to COX-1, which leads to a decrease in the production of prostaglandins - inflammatory mediators, pain and hyperthermic reaction.
After injecting a single dose of the spray, its active substance, acting on the mucous membrane of the mouth and throat, after 5 minutes, relieves difficulty in swallowing, a feeling of irritation and soreness in the throat; after 20 minutes - significantly reduces the intensity of pain; after 30 minutes - reduces swelling. The analgesic and decongestant effect of the spray lasts up to 6 hours.
The tablet dissolves completely in the oral cavity within 5–12 minutes. The onset of Strepsils Intensive action is marked from the second minute. A pronounced decrease in the intensity of sore throat is recorded from 22 minutes after administration, the maximum effect is achieved after 70 minutes and lasts up to 4 hours.
Pharmacokinetics
After injecting a single dose of spray (3 presses on the dispenser), as well as after dissolving the tablet, the active substance is quickly and almost completely absorbed, being detected in the blood plasma after 2–5 minutes. The maximum concentration of flurbiprofen (C max) in the blood is observed 30 minutes after spraying the spray and amounts to 1.6 μg / ml, which is 4 times less than when 50 mg of the drug is taken orally. In the oral cavity, flurbiprofen can be absorbed through passive diffusion. When Strepsils Intensive is used in the form of tablets, its C max is fixed 40–45 minutes after resorption; when flurbiprofen is taken orally in the same dose, a longer period is required to reach C max.
By quickly binding to blood plasma proteins, the substance is intensively distributed throughout the body. It is metabolized mainly by hydroxylation in the liver and excreted by the kidneys, to a lesser extent - with bile.
The half-life (T 1/2) of flurbiprofen is 3–6 hours. In small doses (less than 0.05 μg / ml) it is excreted in breast milk. About 20-25% of an oral dose is excreted unchanged by the kidneys.
Indications for use
Strepsils Intensive is recommended for use as a symptomatic remedy for relieving sore throat against the background of infectious and inflammatory diseases of the upper respiratory tract (for spray), oral cavity and pharynx (for tablets).
Contraindications
Absolute:
- erosive and ulcerative lesions of the gastrointestinal tract (GIT), including gastric ulcer and duodenal ulcer, ulcerative bleeding in the active phase or history (2 or more recorded episodes of peptic ulcer or bleeding ulcer);
- a history of hypersensitivity reactions (bronchospasm, bronchial asthma, rhinitis, recurrent polyposis rhinosinusitis, urticaria, Quincke's edema) provoked by the use of acetylsalicylic acid or other NSAIDs;
- perforation of an ulcer or bleeding of the gastrointestinal tract associated with the treatment of NSAIDs, in addition to the spray - ulcerative colitis, a history of hemorrhagic and hematopoietic disorders;
- decompensated heart failure; the period after the operation of coronary artery bypass grafting;
- renal failure, severe, with creatinine clearance (CC) <30 ml / min, diagnosed with hyperkalemia;
- active liver disease or severe liver failure;
- hemophilia and other blood clotting disorders (including hypocoagulation), hemorrhagic diathesis;
- deficiency of glucose-6-phosphate dehydrogenase;
- glucose-galactose malabsorption, fructose intolerance, sucrase / isomaltase deficiency;
- III trimester of pregnancy;
- lactation period (for spray);
- age up to 12 years;
- hypersensitivity to any of the constituents of the spray or Strepsils Intensive tablets.
Relative (in the presence of these diseases / conditions, you should consult a doctor before using the drug):
- the presence of 1 episode of gastric ulcer or gastrointestinal ulcer bleeding in the anamnesis;
- a history of gastrointestinal diseases (Crohn's disease, ulcerative colitis), enteritis, gastritis, colitis, the presence of Helicobacter pylori infection;
- concurrent use of other NSAIDs;
- bronchial asthma, allergic lesions in the exacerbation phase or in history, due to the possible development of bronchospasm;
- systemic autoimmune diseases of connective tissue, including systemic lupus erythematosus, due to an increased risk of aseptic meningitis (with short-term use of Strepsils Intensive, the risk is negligible);
- arterial hypertension (including in history) and / or heart failure, edema, since Strepsils Intensive is able to provoke fluid retention, increased blood pressure (BP);
- cirrhosis of the liver with portal hypertension, hepatic failure, hyperbilirubinemia;
- renal failure, including against the background of dehydration (CC below 60 ml / min), nephrotic syndrome;
- alcoholism;
- I – II trimester of pregnancy;
- lactation period (for tablets);
- elderly age.
Strepsils Intensive, instructions for use: method and dosage
Regardless of the form of release, Strepsils Intensive is recommended only for a short-term course of treatment - no more than 3 days.
The lozenges are kept in the mouth until they are completely dissolved. Teenagers over 12 years old and adults are usually prescribed to use 1 tablet every 3-6 hours. The maximum daily dose should not exceed 5 tablets.
Dosed spray Strepsils Intensive is injected to adolescents over 12 years old and adults at intervals of 3–6 hours, spraying 1 dose (3 presses on the dispenser) along the back wall of the oropharynx, but not more than 5 doses per day. Do not inhale the solution when spraying.
Before using the spray for the first time, it is necessary to make at least 4 clicks on the dispenser, spraying the solution in the opposite direction from you until a uniform spray appears. In the same way, proactively spray the drug into the air, making at least 1 press on the dispenser, before each subsequent spraying of the spray in the oral cavity.
If, within 3 days of using Strepsils Intensive, symptoms persist or worsen, you should consult a doctor.
Side effects
The likelihood of developing undesirable manifestations can be minimized by using Strepsils Intensive in a short course and in the lowest effective dose required to relieve symptoms.
Side effects from systems and organs, which were recorded against the background of short-term treatment with Strepsils Intensive:
- cardiovascular system: with an unknown frequency - increased blood pressure, edema, heart failure;
- blood and lymphatic system: with unknown frequency - anemia, thrombocytopenia;
- immune system: rarely - anaphylactic reactions;
- nervous system: often - headache, dizziness, paresthesia; infrequently - drowsiness / insomnia;
- respiratory system, chest and mediastinal organs: often - a feeling of irritation in the throat; infrequently - wheezing, shortness of breath, exacerbation of bronchial asthma and bronchospasm; decreased sensitivity, blisters in the mouth and pharynx;
- Gastrointestinal tract: often - nausea, diarrhea, discomfort in the oral cavity (burning or tingling sensation, feeling of warmth in the mouth), pain in the mouth and pharynx, ulceration, paresthesia of the oral cavity; infrequently - dry mouth, flatulence, dysesthesia of the oral cavity, dysgeusia, glossalgia (burning mouth syndrome), dyspepsia, vomiting, abdominal pain, constipation, bloating;
- hepatobiliary system: with unknown frequency - hepatitis;
- skin and subcutaneous fat: infrequently - itching, skin rash; with an unknown frequency - severe skin reactions such as bullous lesions, including Stevens-Johnson syndrome and toxic epidermal necrolysis (Lyell's syndrome);
- others: infrequently - pain, fever.
Overdose
Symptoms of an overdose of Strepsils Intensive may include: pain in the epigastric region, nausea, vomiting; less often - headache, tinnitus, diarrhea, gastrointestinal bleeding. With more serious intoxication, such disorders as drowsiness may appear; rarely, blurred vision, agitation, seizures, disorientation, or coma. In cases of severe NSAID poisoning, the following symptoms were recorded: an increase in prothrombin time, metabolic acidosis, a decrease in blood pressure, damage to the hepatic parenchyma, acute renal failure, respiratory depression and cyanosis. In persons with bronchial asthma, the risk of exacerbation of this disease is aggravated.
Therapy is prescribed symptomatic, with mandatory measures to maintain airway patency, monitoring the electrocardiogram (ECG) and vital signs until the condition normalizes. Oral administration of activated charcoal or gastric lavage is recommended for one hour after using an excessive dose of flurbiprofen. For frequent / prolonged seizures, intravenous lorazepam or diazepam is given. In case of aggravation of bronchial asthma, bronchodilators are prescribed. There is no specific antidote.

Orange lozenges do not contain sugar, and each honey-lemon tablet contains approximately 2.5 g of sugar - 0.21 bread units (XE), which should be taken into account by patients with diabetes.
Since orange tablets contain isomalt and liquid maltitol (the maximum dose of which per day exceeds 2 g), they can have a laxative effect.
If, against the background of treatment, symptoms of gastropathy develop, careful monitoring is shown, including esophagogastroduodenoscopy, general blood test (determination of hemoglobin level), and fecal occult blood test.
During the period of therapy, patients should stop drinking ethanol-containing drinks.
In case of aggravation of existing symptoms or the emergence of new ones, including with the development of signs of a bacterial infection, it is necessary to immediately contact a specialist to review the therapy.
If irritation occurs in the oral cavity, skin rash and other manifestations of an allergic reaction, you need to stop treatment with the drug and consult a doctor.
If it is necessary to determine the concentration of 17-ketosteroids, flurbiprofen must be discontinued 48 hours before this test.
Influence on the ability to drive vehicles and complex mechanisms
With the development of drowsiness, dizziness, lethargy or visual impairment against the background of treatment, patients should refuse to drive vehicles and control other complex mechanisms.
Application during pregnancy and lactation
In the third trimester of pregnancy, drug treatment is contraindicated. In the I – II trimesters of using Strepsils Intensive, it is recommended to avoid, and if necessary, its use should be consulted with a doctor.
Women planning pregnancy should take into account that Strepsils Intensive inhibits COX and prostaglandin production, and may affect ovulation, leading to impaired female reproductive function (which is reversible after discontinuation of therapy).
It has been established that flurbiprofen is excreted in small amounts in breast milk without causing any negative consequences for children's health. Nevertheless, due to the possible development of undesirable effects in a child, the use of Strepsils Intensive during lactation is not recommended.
Pediatric use
For patients under 12 years of age, therapy with Strepsils Intensive is contraindicated.
With impaired renal function
Strepsils Intensive is contraindicated in patients with severe renal failure (CC <30 ml / min).
With mild / moderate severity, including in the case of dehydration (CC <60 ml / min) and the development of nephrotic syndrome, due to the existing risk of deterioration of renal function (against the background of short-term use, the risk is insignificant), use the drug with caution.
For violations of liver function
Strepsils Intensive is contraindicated for patients with active liver disease or severe liver failure.
In the presence of functional liver failure of moderate severity, cirrhosis of the liver with portal hypertension, the drug should be used with caution, due to the possible deterioration of kidney function. Against the background of short-term treatment, this probability is minimal.
Use in the elderly
Elderly people are prescribed Strepsils Intensive therapy with caution due to the existing risk of deterioration of renal function.
Drug interactions
- acetylsalicylic acid (at a dose of more than 75 mg per day), other NSAIDs (including ibuprofen and selective COX-2 inhibitors): the threat of side effects is aggravated; it is recommended to avoid combinations of flurbiprofen with these drugs;
- antiplatelet agents (including clopidogrel), selective serotonin reuptake inhibitors (including fluoxetine, citalopram, paroxetine, sertraline), ethanol: increase the risk of gastrointestinal bleeding;
- anticoagulants (including warfarin): flurbiprofen enhances the effect of these drugs;
- angiotensin-converting enzyme (ACE) inhibitors, angiotensin II antagonists and other antihypertensive drugs, diuretics: there is a decrease in the effectiveness of these drugs, which can provoke nephrotoxicity as a result of COX inhibition, especially in persons with impaired renal function (it is required to monitor adequate fluid replacement);
- tacrolimus, cyclosporine: flurbiprofen increases the risk of developing nephrotoxicity;
- methotrexate: an increase in its plasma concentration is possible during treatment with NSAIDs, therefore, it is necessary to take the last 24 hours before or after using methotrexate;
- glucocorticosteroids (including prednisone): the likelihood of gastrointestinal ulcers and gastrointestinal bleeding is aggravated;
- mifepristone: flurbiprofen reduces its effectiveness, the use of NSAIDs should be started no earlier than 8–12 days after the end of mifepristone use;
- lithium preparations: an increase in the level of lithium in the blood plasma is possible;
- quinolone antibiotics: the risk of seizures increases;
- phenytoin: an increase in the serum content of this substance may be recorded (it is recommended to monitor the level of phenytoin in the serum, if necessary, change its dose);
- sulfinpyrazone, probenecid: slowing down the elimination of flurbiprofen is likely;
- potassium-sparing diuretics: the likelihood of developing hyperkalemia increases;
- oral hypoglycemic agents: a change in the concentration of glucose in the blood is possible, it is recommended to monitor its plasma level more often;
- zidovudine: the risk of hematotoxicity increases;
- tolbutamide, antacids: to date, no interactions have been identified between these agents and flurbiprofen.
