Sermion is a drug that improves cerebral and peripheral circulation.
Release form and composition
Dosage forms of Sermion release:
- lyophilisate for preparation of solution for injection: white powder or porous mass; applied solvent - colorless, transparent (in colorless glass bottles, 4 bottles in a cardboard box, complete with solvent - 4 ampoules);
- coated tablets: round, convex, orange (5 mg), white (10 mg) or yellow (30 mg) (15 or 25 pcs. in blisters, 2 blisters in a cardboard box).
Composition of 1 bottle of lyophilisate:
- active substance: nicergoline - 4 mg;
- auxiliary components: tartaric acid - 1.04 mg, lactose monohydrate - 40 mg.
Solvent: benzalkonium chloride - 0.2 mg, sodium chloride - 34 mg, water for injection - up to 4 ml.
Composition of 1 tablet:
- active substance: nicergoline - 5, 10 or 30 mg;
- auxiliary components (5/10/30 mg): calcium hydrogen phosphate dihydrate - 100 / 94.3 / 72.69 mg, magnesium stearate - 1.3 / 2 / 3.61 mg, microcrystalline cellulose - 22.4 / 22.4 / 22.4 mg, sodium carboxymethylcellulose - 1.3 / 1.3 / 1.3 mg;

Indications for use
- cerebral vascular and metabolic disorders in acute and chronic course caused by diseases and conditions such as atherosclerosis, arterial hypertension, thrombosis or embolism of cerebral vessels, including acute transient cerebrovascular accidents, vascular dementia and vasospasm-induced headache;
- peripheral vascular and metabolic disorders in acute and chronic course (Raynaud's disease, functional and organic arteriopathies of the extremities, syndromes caused by impaired peripheral blood flow);
- hypertensive crisis (as part of complex treatment; for parenteral administration).
Contraindications
Absolute:
- acute bleeding;
- orthostatic hypotension;
- severe bradycardia;
- acute myocardial infarction;
- pregnancy and lactation (the safety profile for this category of patients has not been studied, parenteral administration during pregnancy is possible only according to strict indications and under the direct supervision of a specialist);
- hypersensitivity to the components of the drug.
Additionally for Sermion, film-coated tablets:
- fructose intolerance, isomaltase / sucrase deficiency, glucose-galactose malabsorption;
- age up to 18 years.
A relative contraindication for the appointment of Sermion in all dosage forms is a history of gout or hyperuricemia and / or combined use with drugs that disrupt the metabolism or excretion of uric acid.
Method of administration and dosage
Injection
Sermion is administered intravenously (intravenously), intramuscularly (intramuscularly) or intravenously (intraarterially).
The dosage regimen (dose, duration and route of administration) is determined by the nature of the disease. In some cases, it is preferable to start the course with parenteral administration, after which, for maintenance therapy, they switch to taking the drug inside.
Recommended dosage regimen:
- IV: slow infusion; infusion solution - 4-8 mg Sermion in 100 ml of 0.9% sodium chloride solution or 5-10% dextrose solution; it is possible to administer such a dose several times a day;
- i / m: 2 times a day, 2-4 mg;
- i / a: solution - 4 mg in 10 ml of 0.9% sodium chloride solution; injection time - 2 minutes.
It is recommended to use the reconstituted Sermion solution immediately after preparation.
Pills
Sermion is taken orally.
Usually the drug is prescribed 3 times a day, 5-10 mg, or 2 times a day, 30 mg. There should be equal breaks between taking single doses. Long-term therapy is indicated.
Features of the dosage regimen:
- chronic disorders of cerebral circulation, vascular cognitive impairment, post-stroke conditions: 3 times a day, 10 mg for a course of at least 3 months. As a rule, the healing effect develops gradually;
- vascular dementia: 2 times a day, 30 mg. Every 6 months it is recommended to consult with a specialist about the advisability of continuing to take Sermion;
- acute disorders of cerebral circulation, ischemic stroke due to atherosclerosis, thrombosis and embolism of cerebral vessels, transient cerebral circulation disorders (hypertensive cerebral crises, transient ischemic attacks): it is preferable to start the course with parenteral administration of Sermion, and then continue the drug intake;
- peripheral circulatory disorders: 3 times a day, 10 mg for a long time (a course of at least several months).
In case of functional impairment of the kidneys (serum creatinine content ≥2 mg / dL) Sermion in all dosage forms is recommended to be used in lower doses.
Side effects
- nervous system: rarely - insomnia or drowsiness;
- cardiovascular system: rarely - a marked decrease in blood pressure, a feeling of heat, dizziness;
- metabolism: an increase in the concentration of uric acid in the blood (the violation does not depend on the dose and duration of use);
- others: rarely - skin rashes, dyspepsia.
As a rule, adverse reactions are mild to moderate in nature.
special instructions
As a rule, if the recommended dosage regimen is observed, Sermion has no effect on blood pressure, however, with arterial hypertension, therapy can lead to a gradual decrease in blood pressure.
Due to the likelihood of arterial hypotension, after parenteral administration of Sermion, it is recommended to be in a horizontal position for several minutes, especially at the beginning of therapy.
Since the effect of the drug develops gradually, it should be used for a long time, while periodically (at least every six months) it is necessary to assess the feasibility of further treatment.
The influence of Sermion on the ability to drive vehicles has not been studied, therefore, despite the fact that therapy helps to improve concentration of attention, the nature of the underlying disease should be taken into account and caution should be exercised when driving / working with mechanisms.
Drug interactions
With the combined use of Sermion with certain drugs / substances, the following effects may develop:
- drugs with antihypertensive action: enhancement of their action;
- acetylsalicylic acid: increased bleeding time.
Nicergoline is metabolized by the isoenzyme CYP2D6, therefore, it is assumed that Sermion may interact with drugs that are metabolized with the participation of the same enzyme.
