Release form and composition
The drug is produced in the form of a dosed nasal spray (0.1 mg + 5 mg / dose) and a dosed nasal spray for children (0.05 mg + 5 mg / dose): a clear, colorless solution [10 ml (100 doses) in vials from high density polyethylene equipped with a pump (dosing device), closed with a transparent protective cap; in a cardboard box 1 bottle and instructions for use of SeptaNazal].
Composition of 1 dose of nasal spray:
- active substances: xylometazoline (in the form of hydrochloride) - 0.1 mg, dexpanthenol - 5 mg;
- auxiliary components: purified water, sodium hydrogen phosphate dodecahydrate, potassium dihydrogen phosphate.
Composition of 1 dose of nasal spray for children:
- active substances: xylometazoline (in the form of hydrochloride) - 0.05 mg, dexpanthenol - 5 mg;

Pharmacological properties
Pharmacodynamics
SeptaNazal is a combined composition drug, the action of which is due to the properties of its active ingredients:
- dexpanthenol is a derivative of pantothenic acid, a vitamin of group B. Once in the body, dexpanthenol is converted into pantothenic acid, which is a component of coenzyme A (CoA). It is involved in the synthesis of corticosteroids, acetylcholine and porphyrins, fat and carbohydrate metabolism, and acetylation processes. It normalizes cellular metabolism, stimulates the restoration of mucous membranes and skin, increases the strength of collagen fibers, and accelerates mitosis. Has a metabolic, regenerating and weak anti-inflammatory effect;
- xylometazoline is a local vasoconstrictor (decongestant) with alpha-adrenomimetic activity. Promotes narrowing of the blood vessels of the nasal mucosa and restores the patency of the nasal passages, thereby eliminating nasal congestion and facilitating nasal breathing. The vasoconstrictor effect develops within a few minutes and lasts up to 10 hours.
Pharmacokinetics
Dexpanthenol after topical application is quickly absorbed by the skin and biotransformed into pantothenic acid, binds to blood plasma proteins, mainly albumin and beta-globulin. The concentration of pantothenic acid in the blood is 0.5–1 mg / l, in the blood serum - 0.1 mg / l. Not metabolized (except for inclusion in CoA). It is excreted from the body unchanged.
Xylometazoline is practically not absorbed. It is present in blood plasma in such low concentrations that it is impossible to determine them by modern analytical methods.
Indications for use
- sinusitis;
- vasomotor and acute allergic rhinitis;
- acute respiratory diseases accompanied by rhinitis;
- otitis media (as part of combination therapy to reduce swelling of the nasopharyngeal mucosa);
- impaired nasal breathing after surgery in the nasal cavity;
- rhinoscopy (to facilitate the procedure).
Contraindications
Absolute:
- dry rhinitis;
- atrophic rhinitis;
- severe atherosclerosis;
- glaucoma;
- tachycardia;
- arterial hypertension;
- hyperplasia of the prostate;
- hyperthyroidism;
- porphyria;
- surgical interventions on the meninges (in history);
- the period of pregnancy and breastfeeding;
- concomitant use of tricyclic antidepressants or monoamine oxidase (MAO) inhibitors;
- children under 6 years of age - for nasal spray 0.1 mg + 5 mg / dose, up to 2 years - for SeptaNazal spray for children 0.05 mg + 5 mg / dose;
- the presence of increased sensitivity to any of the components of SeptaNazal.
Relative:
- diseases of the cardiovascular system (including angina pectoris and ischemic heart disease);
- pheochromocytoma;
- diabetes;
- hypersensitivity to the action of adrenergic drugs, manifested by an increase in blood pressure (BP), arrhythmia, dizziness, insomnia, tremor.
SeptaNazal, instructions for use: method and dosage
SeptaNazal spray is applied intranasally.
The drug is prescribed 1 injection into each nasal passage 3-4 times a day.
Before using the spray for the first time and after a long break in its use, press the dispenser several times to achieve a uniform spray.
Nasal instillation procedure:
- Remove the protective cap.
- Insert the tip of the bottle vertically into the nasal passage.
- Taking a slow breath through the nose, press the pump once.
- Repeat the procedure for the other nostril.
- Wipe the tip with a paper towel and close the cap.
The duration of application of the spray 0.1 mg + 5 mg / dose is 3-5 days, the SeptaNazal spray for children is 0.05 mg + 5 mg / dose - 5-7 days.
Side effects
SeptaNazal is generally well tolerated. With frequent and / or prolonged use of the spray, the following phenomena may develop: tingling, burning, dryness and / or irritation of the nasopharyngeal mucosa, swelling of the nasal mucosa, hypersecretion, sneezing, dry rhinitis, headache, nausea, vomiting, blurred vision, insomnia, depression (with prolonged use of high doses), palpitations, increased blood pressure, tachycardia, allergic reactions.
Overdose
If you accidentally take SeptaNazal inside and in case of an overdose, the following symptoms may appear: cyanosis, mydriasis, fever, nausea, vomiting, hallucinations, convulsions, increased blood pressure, heart rhythm disturbances, tachycardia, cardiac arrest, respiratory failure, pulmonary edema. It is also possible to develop signs of depression of the central nervous system, including drowsiness, decreased body temperature, bradycardia, shock, respiratory arrest, coma.
If you accidentally take SeptaNazal, you should rinse the stomach and take activated charcoal. Treatment of manifestations is symptomatic. If necessary, oxygen ventilation of the lungs is carried out, antipyretic and anticonvulsant drugs are prescribed. To reduce elevated blood pressure, 5 mg of phentolamine in 0.9% sodium chloride solution is slowly injected intravenously or 100 mg of phentolamine is administered orally. The use of vasoconstrictors is contraindicated.

Pediatric use
Age restrictions:
- up to 6 years old - nasal spray 0.1 mg + 5 mg / dose;
- up to 2 years - nasal spray 0.05 mg + 5 mg / dose.
Drug interactions
In the case of the simultaneous use of MAO inhibitors or tricyclic antidepressants, an increase in blood pressure is possible, since these drugs have an effect on the cardiovascular system. The use of these combinations is contraindicated.
