Lidaza is a drug of keloidolytic action. Dissolves scar tissue and increases capillary permeability.

Release form and composition

Lidase is available in the form of a lyophilisate for the preparation of a solution for injection and topical application, which is a powder or a porous mass compacted into a tablet of white or almost white color with a beige, yellowish, brownish, pinkish or cream shade [64 CU each in glass ampoules or vials with a capacity of 5 ml, in a cardboard box 10 ampoules or vials; 5 ampoules in blisters, in a cardboard box 2 packages (if necessary, an ampoule knife or a scarifier is placed in a cardboard box); in the form of a set: for 5 ampoules with the drug, 5 ampoules with a solvent, in a cardboard box 5 sets and an ampoule knife or scarifier if necessary].

One ampoule or one bottle of Lidase contains 64 IU (conventional units) of hyaluronidase.

Solvent (packaged as a kit): 0.9% sodium chloride solution (2 ml) or novocaine solution 5 mg / ml (5 ml).

Indications for use

  • long-term non-healing ulcers (including ulcers after radiation therapy);
  • traumatic, burn and postoperative scars;
  • osteoarthritis, joint contractures (after injuries, inflammatory processes), severe diseases of the lumbar discs, joint stiffness, ankylosing spondylitis;
  • Dupuytren's contracture;
  • cutaneous manifestations of scleroderma;
  • chronic tendovaginitis;
  • superficial soft tissue hematomas;
  • preparation for dermal and plastic surgery for scarring;
  • traumatic lesions of peripheral nerves and nerve plexuses (neuritis, plexitis);
  • obstructive phenomena in inflammatory processes in the bronchi and upper respiratory tract;
  • pulmonary tuberculosis with nonspecific bronchial lesions;
  • hemophthalmus of the eye, hemorrhage in the eye, retinopathy of various origins.

Contraindications

Absolute:

  • recent hemorrhages;
  • infectious and inflammatory diseases in the acute phase;
  • pulmonary hemorrhage, pulmonary tuberculosis with severe respiratory failure, hemoptysis (for inhalation use);
  • fresh vitreous hemorrhage (for inhalation use);
  • malignant tumors (for inhalation use);
  • simultaneous use of estrogens;
  • children and adolescents up to 18 years old;
  • increased individual sensitivity to hyaluronidase.

Relative (Lidaza is used with caution):

  • period of pregnancy;
  • period of breastfeeding.

Method of administration and dosage

Lidase is used in the form of subcutaneous or intramuscular injections, subconjunctival, parabulbar, inhalation, electrophoresis and topically by applying dressings soaked in a solution of the drug.

To prepare solutions for injections, use 0.9% sodium chloride solution or 0.5% novocaine (procaine) solution, for inhalation use - 0.9% sodium chloride solution, and for administration by electrophoresis - distilled water.

For cicatricial lesions, Lidaza is administered intramuscularly (near the site of the lesion) or subcutaneously (under the scar tissue), 1 ml (64 AU) once a day, every day or every other day. The course of treatment is 10–20 injections.

For neuritis and plexitis, the drug is injected subcutaneously into the area of the affected nerve. A single dose is 64 IU in a solution of procaine. Injections are performed once a day every other day. One course of treatment requires 12-15 injections. If necessary, a second course is prescribed.

In patients with pulmonary tuberculosis, Lidase is used in complex treatment in order to increase the concentration of antibacterial drugs in the lesions. The route of administration is in the form of inhalation and / or injection. For one inhalation, the contents of one vial are dissolved in 5 ml of isotonic sodium chloride solution. Inhalations are carried out once a day every day. One course of treatment requires 20-25 inhalations. A repeated course is carried out if necessary, but not earlier than in 1.5–2 months.

When used in ophthalmic practice, the contents of one bottle of Lidase are dissolved in 20 ml of water for injection (0.1% solution). Then the drug is injected parabulbarly at 0.5 ml, subconjunctivally at 0.3 ml or by electrophoresis.

For use by electrophoresis, the contents of one vial are dissolved in distilled water (60 ml), 0.1% hydrochloric acid solution (2-3 drops) is added and injected from the anode to the affected area. The duration of the procedure is 20-30 minutes. One course of treatment requires 15–20 sessions. The prepared solution should be used within 24 hours.

Outwardly, Lidaza is used in the form of dressings moistened with a solution of the drug. Each 64 EU (1 bottle) is dissolved in 10 ml of boiled water cooled to room temperature, or 0.9% sterile sodium chloride solution. The resulting solution is moistened with a bandage folded in 4 or 5 layers, and then applied to the affected area and covered with wax paper. A soft bandage is used to fix the application. The duration of one procedure is 15-18 hours. The course of treatment is 15-60 days. With therapy for a long time, a break of 3-4 days should be taken every 2 weeks. Application use of Lidase can be alternated with the method of electrophoresis.

Side effects

Allergic reactions are possible, and with prolonged use, Lidaza can have a local irritating effect.

Lidase solution should not be injected through a catheter previously used to administer cationic solutions.

Before using the drug, it is advisable to conduct a sensitivity test. To do this, the patient is injected with 20 μl of hyaluronidase intradermally.

The drug should not be injected into the areas of tumor and infectious inflammation.

Lidaza does not affect the patient's ability to drive vehicles and engage in other potentially hazardous work that requires a quick reaction and high concentration.

Drug interactions

Lidase enhances the effect of local anesthetic drugs. When used simultaneously with drugs administered intramuscularly or subcutaneously, it improves their absorption.
