Ibuprofen-Hemofarm is a non-steroidal anti-inflammatory drug (NSAID).

Release form and composition

Dosage forms of Ibuprofen-Hemofarm:

  • effervescent tablets: round, flat-cylindrical, from yellowish-white to white, beveled on both sides, with a slight lemon scent (10 or 20 pcs. in plastic tubes, 1 or 2 tubes in a cardboard box);
  • film-coated tablets: round, biconvex, from yellowish-white to white, with a dividing line on one side, the core of the tablet is from yellowish-white to white (10 pcs. in blisters, in a cardboard box 1, 2, 3 or 5 blisters).

1 effervescent tablet contains:

  • active substance: ibuprofen D, L-lysinate - 0.342 g, this is equivalent to 0.2 g of ibuprofen;
  • auxiliary components: xylitol, sodium carbonate, aspartame, sodium saccharinate, povidone K-25, simethicone aqueous emulsion (Silfar SE-4), lemon flavor.

Pharmacological properties

Pharmacodynamics

Ibuprofen-Hemofarm has analgesic, anti-inflammatory, antipyretic activity. The active substance is ibuprofen, its mechanism of action is due to inhibition of the synthesis of prostaglandins - mediators of inflammation, pain and hyperthermic reaction and indiscriminate blocking of cyclooxygenase 1 and 2. The manifestation of the therapeutic effect of the drug is the weakening of pain syndrome (including joint pain at rest and during movement), an increase in the range of motion, reduction of morning stiffness and swelling of the joints.

Pharmacokinetics

Ibuprofen is absorbed quickly, its maximum concentration (C max) in blood plasma when taken on an empty stomach is reached after 0.75 hours. Taking the drug after meals causes a slight decrease in the rate of absorption, C max is achieved in 1.5–2.5 hours. After 2-3 hours, the concentration of ibuprofen in the synovial fluid is higher than in the blood plasma.

Plasma protein binding - 90%.

Ibuprofen penetrates into the joint cavity slowly, lingering in the synovial tissue.

The biological activity of ibuprofen is associated with the S-enantiomer. Approximately 60% of the pharmacologically inactive R-form after absorption is slowly converted to the active S-form. Metabolized by the isoenzyme CYP2C9. The pre-systemic and post-systemic metabolism of the drug occurs in the liver.

The half-life (T 1/2) with biphasic elimination kinetics is 2–2.5 hours.

In the form of metabolites (unchanged - up to 1% ibuprofen), it is excreted to a greater extent through the kidneys, a small part - with bile.

Pharmacokinetic parameters do not change in elderly patients.

Indications for use

  • symptomatic therapy of inflammatory and degenerative diseases of the musculoskeletal system, including osteoarthritis, rheumatoid arthritis, juvenile chronic arthritis, psoriatic arthritis, gouty arthritis, ankylosing spondylitis (ankylosing spondylitis) - to reduce pain and reduce inflammation during use (the drug has no effect on disease progression);
  • myalgia, ossalgia, arthritis, arthralgia, sciatica, neuralgia, tendovaginitis, tendonitis, bursitis, headache (including menstrual syndrome), migraine, toothache accompanied by inflammation post-traumatic and postoperative condition, oncological pathologies - as a pain reliever;
  • algodismenorrhea, adnexitis and other inflammatory processes in the small pelvis;
  • colds and infectious diseases, accompanied by febrile syndrome.

Contraindications

  • acute period of gastric ulcer and duodenal ulcer, peptic ulcer, ulcerative colitis, ulcerative colitis, Crohn's disease and other erosive and ulcerative pathologies of the gastrointestinal tract (gastrointestinal tract), gastrointestinal bleeding, inflammatory bowel disease;
  • incomplete or complete combination of bronchial asthma, recurrent polyposis of the paranasal sinuses and nose, intolerance to acetylsalicylic acid or other NSAIDs (including history);

  • individual intolerance to acetylsalicylic acid and other NSAIDs;
  • hypersensitivity to the components of the drug.

It is recommended to take Ibuprofen-Hemofarm with caution in patients with hepatic failure, hyperbilirubinemia, liver cirrhosis with portal hypertension, renal failure (CC less than 60 ml / min), nephrotic syndrome, chronic heart failure, arterial hypertension, ischemic heart disease, peripheral arterial disease, cerebral diseases, dyslipidemia or hyperlipidemia, diabetes mellitus, a history of gastric ulcer and duodenal ulcer, with gastritis, the presence of Helicobacter pylori infection, colitis, enteritis, blood diseases of unknown etiology (anemia, leukopenia), long-term use of NSAIDs, severe somatic diseases, smoking, frequent alcohol consumption, breastfeeding, in the I and II trimesters of pregnancy,in old age.

Effervescent tablets

Ibuprofen-Hemofarm 200 mg Effervescent Tablets must not be swallowed whole, chewed or sucked.

The tablets are taken orally, after being dissolved in 200 ml of water, after meals.

Ibuprofen-Hemofarm 400 mg tablets are taken orally after meals, swallowing whole and drinking plenty of water.

The dose should be selected individually, taking into account clinical indications, using the smallest effective dose that will ensure the desired therapeutic effect.

Recommended dosage for patients over 12 years old: 1 pc. (0.4 g) with an interval of at least 4 hours. Maximum daily dose: adults - 3 pcs. (1.2 g), children 12-18 years old - 2.5 pcs. (1 d).

Overdose

Symptoms: nausea, vomiting, abdominal pain, drowsiness, lethargy, depression, tinnitus, headache, low blood pressure, tachycardia, bradycardia, atrial fibrillation, metabolic acidosis, acute renal failure, coma, respiratory arrest.

  • cefotetan, cefamandol, cefoperazone, plikamycin, valproic acid cause an increase in the incidence of hypoprothrombinemia;
  • insulin, oral hypoglycemic agents, including sulfonylurea derivatives, enhance their effect;
  • gold preparations, cyclosporine enhance the effect of ibuprofen on the synthesis of prostaglandins in the kidneys, causing an increase in nephrotoxicity; cyclosporine increases its plasma concentration and the risk of developing hepatotoxic effects;
  • inhibitors of microsomal oxidation reduce the likelihood of hepatotoxic effects;
  • cholestyramine, antacids help to reduce the absorption of ibuprofen;
  • slow calcium rope blockers, angiotensin-converting enzyme inhibitors (vasodilators) reduce their hypotensive activity;
  • agents that block tubular secretion, increase the plasma concentration of ibuprofen;
  • uricosuric agents reduce their therapeutic efficacy;
  • furosemide, hydrochlorothiazide reduce natriuretic and diuretic effects;
  • indirect anticoagulants, antiplatelet agents, fibrinolytics enhance their effect;
  • mineralocorticosteroids, glucocorticosteroids, ethanol, colchicine, estrogens enhance their ulcerogenic effect, increasing the risk of bleeding;
  • caffeine enhances the analgesic effect of ibuprofen;
  • digoxin, lithium preparations, methotrexate increase their level of concentration in the blood;
  • myelotoxic drugs cause an increase in the hematotoxic manifestations of the drug.
