Zerkalin is an antibacterial drug for external use.
Release form and composition
Dosage form - 1% solution for external use: transparent colorless liquid with the smell of ethanol (30 ml in dark glass dropper bottles, each bottle is packed in a cardboard box).
1 ml of solution contains:
- active substance: clindamycin (in the form of hydrochloride) - 10 mg;
- additional components: purified water, propylene glycol, ethyl alcohol 96%.
Pharmacological properties
Pharmacodynamics
Clindamycin is an antibacterial agent from the lincosamide group that is active against all strains of Propionibacterium acnes. Its minimum inhibitory concentration (MIC) is 0.4 μg / ml.
The drug, due to interaction with the 50S-subunits of ribosomes, inhibits protein synthesis in the microbial cell.
After external application of Zerkalin, the amount of free fatty acids on the skin surface decreases from about 14% to 2%.
Pharmacokinetics
Clindamycin, rapidly accumulating in comedones, begins to have an antibacterial effect. The average content of the substance in the contents of comedones after application of the solution to the skin significantly exceeds the MIC of the causative agent of acne, Propionibacterium acnes.
Considering the external route of administration of the drug, the concentration of clindamycin in urine and blood plasma is very low.
Indications for use
Zerkalin is a drug designed to treat acne vulgaris (acne).
Contraindications
Absolute:
- ulcerative and pseudomembranous colitis (currently or in history);
- Crohn's disease;
- hypersensitivity to lincomycin;
- hypersensitivity to any component of the drug.
Relative (cases requiring special care):
- children under 12 years old;
- allergy;
- simultaneous use of muscle relaxants;
- period of breastfeeding.
Instructions for the use of Zerkalin: method and dosage
Zerkalin is applied externally. The solution must be applied to the affected skin areas, previously cleaned and dried, 2 times a day - in the morning and in the evening.
The duration of treatment in each case is determined individually. To obtain a positive result, at least 6-8 weeks of daily use of the drug are required. The maximum allowable duration of therapy is 6 months.
Side effects
According to the instructions, Zerkalin is usually well tolerated. In some cases, the following side effects occur: irritation at the site of application of the solution, itching, burning, dryness and peeling of the skin, increased secretion of sebum, erythema, contact dermatitis.
The likelihood of pseudomembranous enterocolitis during drug resorption is not excluded.
Overdose
Due to the peculiarities of local application of the solution, the probability of an accidental overdose is minimal.

Drug interactions
Due to the risk of cumulative irritant effects on the skin, Zerkalin for acne is not recommended to be prescribed in combination with other drugs for the treatment of acne, which include abrasive, emollient and exfoliating agents, for example, salicylic acid, resorcinol, benzoyl peroxide, sulfur, tretinoin.
You should not use erythromycin at the same time, because when it interacts with clindamycin, antagonistic properties are manifested.
Cross-resistance has been reported between clindamycin and lincomycin.
Zerkalin in the case of combined use can enhance the effect of muscle relaxants, as a result of which neuromuscular transmission may be impaired. For this reason, special care should be taken when prescribing such a combination.
