Release form and composition

Composition of 1 tablet:

• active substance: loperamide hydrochloride - 2 mg;
• auxiliary components: potato starch, milk sugar (lactose), talc, magnesium stearate, aerosil (colloidal silicon dioxide), polyvinylpyrrolidone (povidone).

Pharmacological properties

Pharmacodynamics

Loperamide binds to opioid receptors of the intestinal wall (modulation of choline and adrenergic neurons through guanine nucleotides), inhibits the release of acetylcholine and prostaglandins, thereby reducing the tone and motility of intestinal smooth muscles. Slowing down peristalsis, increases the transit time of intestinal contents. Increases the tone of the anal sphincter, reduces the urge to defecate, retains stool.

After taking the drug orally, the effect develops quickly and lasts up to 4-6 hours.

Pharmacokinetics

When administered orally, loperamide is absorbed by about 40%. The maximum plasma concentration reaches within 2.5 hours. It binds to plasma proteins by 97%. It does not penetrate the blood-brain barrier.

It is metabolized almost completely in the liver by conjugation. The half-life is from 9 to 14 hours. It is excreted from the body mainly in the feces through bile, in a small amount is excreted in the urine.

Indications for use

Vero-Loperamide is used for the symptomatic treatment of acute and chronic diarrhea of various origins: allergic, drug, emotional and radiation genesis; due to changes in the qualitative composition of food and / or diet; against the background of impaired absorption and metabolism.

As an adjuvant, Vero-Loperamide is used for infectious diarrhea.

For patients with ileostomy, the drug is prescribed to regulate the stool.

Contraindications

Absolute:

• diarrhea against the background of acute pseudomembranous enterocolitis;
• ulcerative colitis in the acute stage;
• intestinal obstruction;
• diverticulosis;
• dysentery and other infections of the gastrointestinal tract (when using the drug as monotherapy);
• children under 5 years old;
• I trimester of pregnancy;
• lactation period;
• hypersensitivity to loperamide or any auxiliary component.

Vero-Loperamide should be used with caution in patients with hepatic impairment, as well as in the II and III trimesters of pregnancy.

Vero-Loperamide, instructions for use: method and dosage

Vero-Loperamide tablets are taken orally.

The initial dose for adults is 4 mg (2 tablets), then 2 mg (1 tablet) after each bowel movement, while the stool remains loose. The maximum permissible daily dose of the drug is 16 mg (8 tablets).

In children over 5 years of age with acute diarrhea, the first and subsequent doses are 2 mg (1 tablet). The maximum daily dose is 6 mg (3 tablets).

Treatment with the drug is stopped when the stool is normalized or absent for more than 12 hours.

Side effects

• from the gastrointestinal tract (GIT): dry mouth, nausea, constipation, intestinal colic, vomiting, gastralgia; very rarely - intestinal obstruction;
• from the central nervous system (CNS): dizziness, drowsiness;
• allergic reactions: skin rash.

Overdose

In case of an overdose of Vero-Loperamide, intestinal obstruction may develop, as well as depression of the central nervous system (drowsiness, impaired coordination of movements, muscle hypertension, miosis, stupor, respiratory depression).

The antidote to loperamide is naloxone (it may need to be re-administered because loperamide has a longer duration of action). Treatment of an overdose is symptomatic: gastric lavage, intake of activated carbon. If necessary, artificial lung ventilation is performed. The patient must be under medical supervision for at least 48 hours.

Pediatric use

Vero-Loperamide is contraindicated in children under 5 years of age.

Use in the elderly

The bioavailability of loperamide is increased by ritonavir and co-trimoxazole (due to inhibition of its metabolism during the first passage through the liver).

Naloxone is an antidote to loperamide.
