Pharmacological properties

Pharmacodynamics

The mucolytic and expectorant effects of carbocisteine are associated with the activation of sialic transferase, an enzyme of goblet cells localized on the bronchial mucosa. The substance normalizes the quantitative ratio of neutral and acidic sialomucins contained in bronchial secretions. The drug reduces the viscosity of the discharge from the paranasal sinuses and bronchial secretions, improves the discharge of mucus and sputum, and reduces the severity of the cough reflex.

Carbocisteine provides regeneration of the mucous membrane, restoring its structure, and intensifies the activity of the ciliated epithelium. It also improves the production of IgA immunoglobulin, which is responsible for specific protection, and normalizes mucociliary clearance and the number of sulfhydryl groups of mucus components responsible for nonspecific protection.

Pharmacokinetics

When taken orally, the absorption rate of carbocysteine is quite high, but it has a low bioavailability (less than 10% of the dose taken). Its maximum level in the mucous membrane of the respiratory tract and in the blood serum is recorded 2-3 hours (for capsules) or 1.5-2 hours (for syrup) after taking Bronchobos inside and remains in the mucous membrane for 8 hours. Cumulation of the drug is observed in the blood, middle ear and liver. Also in high concentrations, it is determined in bronchial secretions (17.5% of the dose taken).

The substance is metabolized in the liver and has a first pass effect through the liver. Carbocisteine is excreted in the urine (60-90% is excreted unchanged, the rest is excreted as metabolites). The half-life is approximately 2 hours, and complete excretion is achieved 3 days after taking the drug.

Indications for use

According to the instructions, Bronchobos is indicated in the case of:

  • Chronic and acute bronchopulmonary diseases, which are accompanied by a violation of the formation and excretion of sputum (bronchitis, tracheitis, tracheobronchitis, bronchial asthma, etc.);
  • Inflammatory diseases of the paranasal sinuses and middle ear (sinusitis, otitis media, rhinitis);
  • Preparing the patient for bronchography or bronchoscopy.

Contraindications

Bronchobos in any form of release cannot be taken with:

  • Hypersensitivity to carbocisteine and / or drug excipients;
  • Cystitis;
  • Peptic ulcer of the duodenum and stomach in the acute stage;
  • Chronic glomerulonephritis in the acute phase.

Syrup of any concentration is contraindicated in:

Overdose

An overdose of Bronchobos is determined by such signs as diarrhea, nausea, gastralgia. Symptomatic treatment is recommended.

special instructions

When taking the drug in the form of a syrup, it is important to take into account that the alcohol content in it is 125 mg in one measuring spoon (in 5 ml of liquid). This may affect the patient's ability to perform work requiring special concentration of attention or drive vehicles and complex mechanisms due to a decrease in the speed of psychomotor responses due to the presence of alcohol in the blood.

Bronchobos in capsules is prescribed in low doses if the patient has a history of gastric ulcer and / or duodenal ulcer. In this case, to enhance the effect of treatment, taking the drug should be combined with physiotherapy procedures.

Influence on the ability to drive vehicles and complex mechanisms

At the moment, it is not known how the reception of Bronchobos affects the ability to drive vehicles and perform potentially dangerous work, requiring increased concentration and immediate psychomotor reactions, due to the lack of relevant studies.

Drug interactions

Carbocisteine makes antibacterial and glucocorticosteroid therapy more effective, used in diseases of the upper and lower respiratory tract of an inflammatory nature, and enhances the bronchodilator effect of theophylline. The combination of Bronchobos with antitussive and atropine-like agents helps to weaken the activity of carbocisteine.

Analogs

Direct analogues of Bronchobos in terms of active ingredient (carbocisteine): Fluifort, Libeksin Muko, Fluditek, Mukosol, Mukodin.

Terms and conditions of storage

Store at a temperature of 15-30 ° C, out of the reach of children.
