Betahistine Canon is a drug that improves the microcirculation of the labyrinth, used in the treatment of pathology of the vestibular apparatus.
Release form and composition
Dosage form - tablets: round flat-cylindrical shape, almost white or white with a cream shade, slight marbling is permissible, with a chamfer (dosage 8 mg) or with a score and a chamfer (dosage 16 and 24 mg) (in a cardboard box 1, 2, 3 or 6 blister packs of 10 tablets of 8, 16 or 24 mg each, or 2 or 4 blister packs of 15 tablets of 24 mg each, or 1 or 2 blister packs of 30 tablets of 8 or 16 mg each and instructions for use of Betahistine Canon).
Composition of 1 tablet:
- active substance: betahistine dihydrochloride - 8, 16 or 24 mg;
- auxiliary components (8/16/24 mg): lactose monohydrate - 41.6 / 83.2 / 124.8 mg; copovidone - 5.2 / 10.4 / 15.6 mg; microcrystalline cellulose - 64.2 / 128.4 / 192.6 mg; magnesium stearate - 1/2/3 mg.
Pharmacological properties
Pharmacodynamics
The mechanism of action of betahistine has been studied only partially. There are several possible hypotheses that are supported by clinical and preclinical data:
- effect on the histaminergic system. Betahistine is a partial agonist of H 1 -histamines and an antagonist of H 3 -histamine receptors of the vestibular nuclei of the central nervous system, has little activity towards H 2 -histamine receptors. Due to the use of the substance, an increase in the metabolism of histamine and its release is observed, which is provided by blocking the presynaptic H 3 -histamine receptors and a decrease in the number of H 3 -histamine receptors;
- increased blood flow to the cochlear region and the entire brain. Betahistine, according to the results of preclinical studies, helps to improve blood circulation in the vascular strip of the inner ear due to the relaxation of the precapillary sphincters of the vessels of the inner ear. In addition, it was found that the use of betahistine leads to an increase in cerebral blood flow in humans;
- facilitating the process of central vestibular compensation. Betahistine therapy helps to accelerate the restoration of vestibular function after unilateral vestibular neurectomy in animals, facilitating and accelerating neutral vestibular compensation due to antagonism with H 3 -histamine receptors. When treating vestibular neurectomy with betahistine, the recovery time in humans is also reduced;
- excitation of neurons in the vestibular nuclei. Betahistine dose-dependently reduces the generation of action potentials in the neurons of the medial and lateral vestibular nuclei. The pharmacological properties of the substance revealed in animals provide a positive therapeutic effect of betahistine in the vestibular system. The efficacy of the substance was demonstrated in patients with Meniere's syndrome and vestibular dizziness, which was manifested by a decrease in the frequency and severity of dizziness.
Pharmacokinetics
Betahistine after oral administration is rapidly and almost completely absorbed in the gastrointestinal tract. After absorption, the drug is also rapidly metabolized almost completely, with the formation of a metabolite of 2-pyridylacetic acid (it has no pharmacological activity). The plasma concentration of betahistine in the blood is very low; therefore, the pharmacokinetic analysis is based on measuring the concentration of the metabolite of 2-pyridylacetic acid in urine and plasma. When taking Betahistine Canon on an empty stomach, the value of C max (maximum concentration of the substance) in the blood is higher than when taken with food. In this case, the total absorption of the substance is the same in both cases. This indicates that the intake of food only slows down the absorption of betahistine.
Betahistine binds to blood plasma proteins at a level of up to 5%.
With max 2-pyridylacetic acid in blood plasma (or urine) is reached 60 minutes after ingestion. T 1/2 (half-life) is approximately 3.5 hours.
2-pyridylacetic acid is rapidly excreted in the urine. When using 8-48 mg of Betahistine Canon, approximately 85% of the dose of betahistine is found in urine. Excretion of the substance through the intestines or kidneys is negligible.
With oral administration of Betahistine Canon in the dose range from 8 to 48 mg, the excretion rate is constant, which indicates the linearity of the pharmacokinetic processes of betahistine and allows us to make the assumption that the involved metabolic pathway remains unsaturated.
Indications for use
- Meniere's syndrome, which is characterized by dizziness (combined with vomiting and nausea), tinnitus and hearing loss (treatment);
- vestibular dizziness (vertigo) (symptomatic treatment).
Contraindications
Absolute:
- lactose intolerance, lactase deficiency, glucose-galactose malabsorption;
- pheochromocytoma;
- pregnancy and lactation;
- age up to 18 years;
- individual intolerance to the components of the drug.
Relative (Betagistin Canon tablets are used under medical supervision):
- burdened history of gastric ulcer or duodenal ulcer;
- bronchial asthma;
- arterial hypotension.
Betahistin Canon, instructions for use: method and dosage
Betagistin Canon tablets are taken orally, preferably with meals.
The daily dose is in the range of 24–48 mg: 3 times a day, 1-2 tablets with a dosage of 8 mg or ½ – 1 tablet with a dosage of 16 mg, or 2 times a day, 1 tablet with a dosage of 24 mg.
The therapy is long-term, its duration is determined individually.
Sometimes improvement is observed after several weeks of use, in some cases the best results are noted after several months of treatment.
Elderly patients, as well as with impaired renal / hepatic function, do not need to adjust the dose of Betahistine Canon.
Side effects
Possible adverse reactions (> 10% - very common;> 1% and 0.1% and 0.01% and <0.1% - rarely; <0.01% - very rare; with an unknown frequency - based on available data it is not possible to establish the frequency of side effects):
- digestive system: often - dyspepsia, nausea; with an unknown frequency - abdominal pain, a feeling of heaviness in the epigastrium, vomiting, bloating;
- nervous system: often - headache;
- skin: with an unknown frequency - urticaria, Quincke's edema, pruritus, rash;
- allergic reactions: with an unknown frequency - hypersensitivity reactions, including anaphylactic.
Overdose
The main symptoms of a drug overdose:
- dose application up to 640 mg - abdominal pain, nausea, drowsiness;
- using a dose of more than 640 mg or taking Betahistine Canon simultaneously with other drugs - cardiopulmonary complications, convulsions.
Therapy: prescribed gastric lavage, activated carbon intake, symptomatic treatment.

Influence on the ability to drive vehicles and complex mechanisms
Betahistine Canon is indicated for the treatment of Meniere's syndrome, which is characterized by a triad of main symptoms: dizziness, hearing loss and tinnitus, as well as with vestibular dizziness. These conditions can have a negative impact on the ability to drive vehicles.
According to the results of clinical studies, betahistine does not affect the ability to drive vehicles or does not significantly affect.
Application during pregnancy and lactation
Betahistine Canon is not prescribed during pregnancy / lactation, which is due to the lack of sufficient data on its safety and effectiveness when used by women during this period of their lives.
Whether betahistine is excreted in breast milk is unknown.
Pediatric use
The safety profile of Betahistine Canon in patients under 18 years of age has not been studied, therefore the use of the drug is contraindicated.
Drug interactions
Special studies aimed at studying the interaction of Betahistine Canon with other drugs have not been conducted. It is assumed that there is no inhibition of the activity of cytochrome P 450 isoenzymes.
When combined with monoamine oxidase (MAO) inhibitors, including MAO-B, care must be taken, since it is possible to inhibit betahistine metabolism under the influence of drugs that inhibit MAO, including MAO subtype B (for example, selegiline).
In theory, the interaction of betahistine and H 1 -histamine receptor blockers could affect the effectiveness of one of these drugs.
