Benzonal is a synthetic drug with an anticonvulsant effect.
Release form and composition
The drug is produced in the form of tablets (10 pcs. In blisters, 3 or 5 packages in a cardboard box; 50 pcs. In vials).
Active ingredient: benzobarbital, in 1 tablet - 0.05 g (tablets for children) or 1 g.
Pharmacological properties
Pharmacodynamics
Benzobarbital is a derivative of barbituric acid, on the basis of which many anticonvulsant, narcotic and hypnotic drugs are obtained. This compound is characterized by anticonvulsant, sedative, hypnotic, and anesthetic effects due to binding to barbiturate receptors. This enhances the effect on the body of gamma-aminobutyric acid and intensifies the flow of chlorine ions through the neuronal membrane, causing its partial hyperpolarization and decreased permeability. Since the barbiturate receptor is located near the receptors for gamma-aminobutyric acid and benzodiazepines, benzobarbital improves the interaction of these compounds with their receptors. Benzobarbital also enhances the depression of the central nervous system.
Benzonal refers to antiepileptic drugs for which hypnotic action is uncharacteristic. The metabolite of benzobarbital is phenobarbital, which enhances the inhibition processes in the central nervous system associated with gamma-aminobutyric acid. This is especially true of the thalamus and the ascending activating reticular formation of the brainstem at the level of interneurons.
Benzobarbital reduces the permeability of nerve fiber membranes for sodium ions and prevents the propagation of impulses from the focus of epileptic activity. The drug begins to work 20-60 minutes after ingestion. Benzobarbital is considered an inducer of liver microsomal enzymes and lowers the level of bilirubin in blood serum.
Pharmacokinetics
Benzobarbital is rapidly absorbed in the stomach. The maximum content of benzobarbital in the stomach is observed 1-3 hours after ingestion. Stable blood concentration is established on the 3rd day. The therapeutic concentration in blood plasma is 15–45 mg / l.
Benzobarbital weakly binds to plasma proteins, being evenly distributed in various organs and tissues. The substance accumulates in high concentrations in the kidneys, liver and brain, and also penetrates the histohematogenous barriers and into breast milk.
Benzobarbital in the body is rapidly metabolized by microsomal liver enzymes with the formation of phenobarbital, which has an antiepileptic effect. Its half-life is 3-4 days, and the compound is excreted in the urine both unchanged and in the form of metabolites.
Indications for use
- Epilepsy of various origins;
- Generalized and partial seizures;
- Epileptic syndrome.
Contraindications
- Hypersensitivity to the components included in the tablets;
- Chronic heart failure II-III degree;
- Liver failure (including chronic);
- Adrenal insufficiency;
- Bronchial asthma;
- Thyrotoxicosis;
- Hyperkinesis;
- Anemia;
- Porphyria;
- Diabetes;
- Respiratory failure;
- Depressive states (with suicidal attempts).
Instructions for the use of Benzonal: method and dosage
Benzonal tablets should be taken 3 times a day (after meals).
A single dosage for adults is 0.1 g, the maximum single dosage is 0.3 g. More than 0.8 g of the drug cannot be taken per day.
Children are usually prescribed (single dose / daily dose):
- 3-6 years old - 0.025-0.05 g / 0.1-0.15 g;
- 7-10 years old - 0.05-0.1 g / 0.15-0.3 g;
- 11-14 years old - 0.1 g / 0.3-0.4 g each;
- Over 14 years old - 0.15 g / 0.45 g.
As a rule, to achieve a therapeutic effect, long-term continuous treatment should be carried out, the duration of which is determined by the doctor.
Side effects
- Drug dependence, addiction, withdrawal syndrome;
- Drowsiness, lethargy, lethargy;
- Headache;
- Bronchospasm;
- Decrease in blood pressure;
- Nystagmus;
- Decreased appetite;
- Anemia;
- Thrombocytopenia;
- Thrombophlebitis;
- Slowing down of mental reactions;
- Difficulty speaking, ataxia;
- Allergic reactions.
Overdose
Overdose symptoms are:
- depression of the central nervous system (nystagmus, drowsiness, visual disturbances, dysarthria, ataxia) up to a coma;
- oppression of the respiratory center;
- pulmonary edema;
- a sharp decrease in blood pressure;
- hemorrhages in places of pressure;
- cyanosis;
- dysfunction of the kidneys;
- tachy or bradycardia;
- general weakness;
- headache;
- constriction of the pupils;
- excitation;
- decrease or increase in body temperature.
Chronic drug intoxication is expressed in confusion, irritability, sleep disturbances, and a decrease in the ability to critically assess what is happening.
In case of an overdose of Benzonal, gastric lavage is performed, activated charcoal is prescribed, as well as symptomatic and resuscitation therapy aimed at restoring and maintaining the normal functioning of the liver, kidneys, central nervous system, respiratory and cardiovascular systems.
special instructions
It should be borne in mind that Benzonal:
- Strengthens the effect of narcotic analgesics, drugs for general anesthesia, hypnotics and antipsychotics (neuroleptics), anxiolytics, ethanol, tricyclic antidepressants;
- Reduces the effect of tetracyclines, anticoagulants, vitamin D, paracetamol, griseofulvin, glucocorticosteroids, quinidine, cardiac glycosides, xanthines.
If the patient has taken other barbiturates before starting therapy, sleep disturbance may occur. In this case, 50-100 mg of phenobarbital or other hypnotic drugs are usually prescribed.
Currently, in the treatment of epilepsy, Benzonal tablets are usually prescribed in cases where other antiepileptic drugs (carbamazepine, valproic acid) are ineffective.
Influence on the ability to drive vehicles and complex mechanisms
During the period of treatment with Benzonal, patients experience a slower speed of psychomotor reactions. It is recommended to refrain from performing complex work requiring increased concentration of attention and immediate response.
Application during pregnancy and lactation
Since benzobarbital crosses the placental barrier and into breast milk, it is not used during pregnancy and lactation.
Drug interactions
According to the instructions, Benzonal enhances the effect on the body of hypnotics, narcotic analgesics, ethanol, neuroleptics, tranquilizers, tricyclic antidepressants, drugs for general anesthesia. When the drug is taken together with xanthines, paracetamol, vitamin D, anticoagulants, quinidine, tetracyclines, cardiac glycosides, griseofulvin, mineralocorticoids, glucocorticosteroids, the effect of the latter decreases.
Analogs
According to the mechanism of action, benzonal analogs are: Hexamidine, Primidone, Phenobarbital.
Terms and conditions of storage
The drug is dispensed by prescription. Shelf life - 4 years at temperatures up to 25 ° C.
Terms of dispensing from pharmacies
Dispensed by prescription.
Reviews about Benzonal
Reviews about Benzonal are numerous, but rather contradictory. Some patients like this drug, being inexpensive and effective, while others are forced to stop taking it due to unwanted side effects. Most often, patients report such symptoms accompanying the course of treatment as nausea, dizziness, speech disorders, drowsiness, lethargy, irritability. Those who have approached the drug are satisfied with the result of therapy and have been taking it for many years.
The price of Benzonal in pharmacies
In pharmacies, the average price for Benzonal 100 mg is 105–130 rubles (50 pcs per pack).
