Release form and composition

The drug is produced in the form of tablets (6 pcs. In a contour non-cell packaging, 2, 3, 5 packs in a carton pack; 10 pcs. In a contour cell-free packaging, in a carton pack, 2 packs and instructions for use of Becarbon).

Content of active substances in 1 tablet:

  • Belladonna (belladonna) extract thick - 0.01 g;
  • Sodium bicarbonate - 0.3 g.

Pharmacological properties

Pharmacodynamics

Bacarbon is a combined antispasmodic agent, the action of which is determined by the properties of its active components:

  • belladonna extract, due to its m-anticholinergic action, helps to reduce the tone and motor activity of smooth muscles of the gastrointestinal tract and other organs, to reduce the secretion of exocrine glands;
  • sodium bicarbonate: neutralizes free hydrochloric acid in the stomach.

Pharmacokinetics

  • belladonna: is well absorbed from the gastrointestinal tract (gastrointestinal tract). Its alkaloids scopolamine, atropine and hyoscyamine cross the placental barrier, and the latter two also easily cross the blood-brain barrier. It is not known whether belladonna alkaloids pass into breast milk. They are hydrolyzed to tropic acid and tropine. Excreted mainly in the urine, to a lesser extent in the feces;
  • sodium bicarbonate: its antacid effect occurs quickly, but is short-lived. During the neutralization reaction of hydrochloric acid, carbon dioxide is formed. Sodium bicarbonate absorbed into the gastrointestinal tract can contribute to the development of alkalosis. The substance is excreted in the urine.

Indications for use

Bacarbon is effective for gastritis with high acidity, dyspeptic disorders, heartburn, intestinal colic.

Contraindications

Absolute:

  • diseases of the cardiovascular system, in which an increase in heart rate (HR) may be undesirable: tachycardia, atrial fibrillation, chronic heart failure (CHF), ischemic heart disease (CHD), severe arterial hypertension, mitral stenosis;
  • diseases of the gastrointestinal tract, accompanied by intestinal obstruction, hypoacid gastritis;
  • angle-closure glaucoma;
  • delayed outflow of urine or a predisposition to this condition, including due to prostate adenoma with urination disorder;
  • myasthenia gravis (myasthenia gravis);
  • thyrotoxicosis;
  • hyperthermic syndrome;
  • acute bleeding;
  • the period of pregnancy and breastfeeding;
  • children and adolescents under 18 years of age;
  • hypersensitivity to belladonna or any other component of the drug.

Relative contraindications for which Becarbon tablets should be prescribed with caution:

  • autonomic (autonomic) neuropathy;
  • severe arterial hypotension;
  • hypertrophy of the prostate gland without obstructive retention of the urinary tract;
  • Down's disease, cerebral palsy, other brain disorders;
  • hepatic / renal failure;
  • chronic lung diseases, accompanied by low secretion and difficult sputum separation, especially in debilitated patients;
  • reflux esophagitis, combined with a hernia of the esophageal opening of the diaphragm;
  • inflammatory bowel disease, including ulcerative colitis (NUC) and Crohn's disease;
  • megacolon;
  • dry mouth (xerostomia);
  • old age (due to increased sensitivity to such antimuscarinic effects as constipation, dry mouth, urinary retention, drowsiness, confusion and / or psychomotor agitation, even with usual doses);
  • weakened condition of patients;
  • high ambient temperature (due to the increased risk of heat stroke due to suppression of the activity of the sweat glands).

Bakarbon, instructions for use: method and dosage

The drug is taken orally, before meals, 1 tablet 2-3 times a day. The maximum allowable dose for adult patients is 5 tablets at a time or 15 tablets per day.

Side effects

  • nervous system and mental disorders: dizziness, headache, sleep disturbances, drowsiness / insomnia, nervousness, weakness, ataxia (impaired coordination of movements), speech disorders, seizures, psychotic disorders (hallucinations, delirium, mainly in older patients - confusion and / or psychomotor agitation);
  • urinary system: difficulty / retention of urination;
  • digestive tract: dry mouth, thirst, taste disturbance, dysphagia, belching, flatulence, abdominal cramps / pain, decreased intestinal motility to atony, decreased tone of the biliary tract and gallbladder, constipation, nausea, vomiting; taking high doses of the drug for a short period can cause compensatory hypersecretion of gastric juice;
  • organ of vision: photophobia, dilated pupils, accommodation paralysis, temporary visual impairment (loss of sharpness), decrease in lacrimal secretion, increase in intraocular pressure;
  • cardiovascular system: hot flushes (including to the face), palpitations, arrhythmias (including extrasystole, transient bradycardia, tachycardia), myocardial ischemia, possibly a drop / increase in blood pressure;
  • the respiratory system and mediastinal organs: a decrease in the secretory activity and tone of the bronchi (the formation of viscous, hard-to-cough up sputum);
  • immune system: hypersensitivity reactions, including anaphylactic reactions / shock;
  • skin and subcutaneous tissue: rash, urticaria, itching, hyperemia, dyshidrosis, exfoliative dermatitis, decreased sweating, dry skin;
  • other reactions: dysarthria, dryness of mucous membranes, edema, increased existing hypokalemia.

If any undesirable reactions appear, you must immediately stop taking Becarbon and consult a specialist.

Overdose

The main symptoms are acute urinary retention, convulsions, psychomotor agitation, accommodation paresis.

Therapy: gastric lavage is indicated, the use of activated carbon; in some cases, symptomatic treatment is carried out.

special instructions

Because Becarbon contains sodium bicarbonate, it should be taken with caution in patients on a low sodium diet.

In undiagnosed and therefore untreated angle-closure glaucoma, the use of anticholinergics (for example, belladonna preparations) can cause an increase in intraocular pressure. In this connection, the appearance of pain and redness of the eyes, accompanied by deterioration / loss of vision, requires an ophthalmologist's consultation.

During therapy with Becarbon, it is required to refrain from drinking ethanol-containing drinks.

Belladonna alkaloids can alter the results of the following laboratory tests:

  • determination of stomach acidity: it is possible to counteract the activity of histamine and pentagastrin; Bekarbon should be discontinued 24 hours before the start of laboratory testing;
  • radioisotope testing of gastric emptying: it is possible to slow down the process of evacuation of gastric contents;
  • phenolsulfophthalein test of urine secretion: it is possible to slow down the elimination of phenolsulfophthalein.

Bekarbon should not be used for longer than the prescribed period without medical advice.

If the symptoms of the disease persist or the health condition worsens, it is required to suspend therapy and seek the advice of a specialist.

Influence on the ability to drive vehicles and complex mechanisms

During treatment with Bekarbon, the patient is advised to observe increased caution when driving a car, engaging in other activities that require increased attention and visual acuity.

Application during pregnancy and lactation

Bacarbon is not prescribed during pregnancy / lactation.

In the first trimester of pregnancy belladonna alkaloids have a teratogenic effect; in the second and third trimesters, the use of Becarbon tablets is not recommended due to the lack of information confirming its safety and effectiveness.

There is no information on the penetration of belladonna alkaloids into breast milk.

Pediatric use

For patients under 18 years of age, the drug is not prescribed.

Drug interactions

The use of any other drugs during Becarbon therapy must be reported to the doctor.

Possible interaction of Becarbon with concurrently taken substances / preparations:

  • other drugs of anticholinergic action: it is possible to increase the effectiveness of Becarbon, the inhibition of peristalsis under its influence can change the absorption of other drugs;
  • antiparkinsonian drugs (amantadine), M-anticholinergics (tiotropium bromide, ipratropium bromide), antiarrhythmic anticholinergic drugs (quinidine, disopyramide), tricyclic antidepressants, nefopam, glutethimide, antispasmodics: their anticholinergic effect is enhanced;
  • opioid analgesics, phenothiazines, clozapine, other drugs that depress the central nervous system (central nervous system): dangerous inhibition of the central nervous system is possible, as well as a decrease in the duration and depth of the effect of narcotic substances, inhibition of the analgesic effect of opiates;
  • butyrophenones (haloperidol) and organic nitrates: increased intraocular pressure;
  • corticosteroids for systemic use: their effect decreases, an increase in intraocular pressure is possible;
  • parasympathomimetics: anticholinergic drugs can inhibit their effect, for example, reduce the effect of pilocarpine in the treatment of glaucoma;
  • furosemide: its use with Becarbon can cause orthostatic hypotension;
  • reserpine, guanethidine, antacids, ascorbic acid, attapulgite: reduce the effect of Becarbon;
  • levodopa: the systemic concentration of levodopa decreases, which requires a correction of its dose;
  • some antihistamines (including meclizine, diphenhydramine, diprazine): their antihistaminic effect and the risk of developing antimuscarinic side reactions increase;
  • β-adrenergic receptor blockers: their action is inhibited;
  • hormonal contraceptives: Bekarbon reduces their contraceptive effect;
  • monoamine oxidase (MAO) inhibitors, including nonselective monoamine reuptake inhibitors: the sedative effect is mutually enhanced, and cardiac arrhythmias may develop;
  • non-steroidal anti-inflammatory drugs, griseofulvin, digitoxin, doxycycline, metronidazole: their therapeutic efficacy decreases;
  • penicillins: mutual potentiation of the medicinal action;
  • ketoconazole: Bekarbon reduces its absorption by increasing the pH of gastric juice, therefore Bekarbon must be taken no earlier than 2 hours after ketoconazole;
  • sulfonamides: their antibacterial effectiveness decreases, the risk of kidney damage increases;
  • indapamide, nizatidine: their effect is enhanced;
  • spironolactone, minoxidil: their effect is weakened;
  • potassium chloride: damage to the gastrointestinal mucosa is possible;
  • domperidone, metoclopramide: anticholinergic drugs inhibit their effect on gastrointestinal activity;
  • ethanol: Becarbon enhances the sedative effect of ethanol;
  • lithium preparations: sodium bicarbonate increases the excretion of lithium, lowering its concentration in the blood plasma.

Since sodium bicarbonate, which is part of Bekarbon, has an antacid effect, in order to avoid impaired absorption, it is advisable not to take other drugs simultaneously with it.
