Release form and composition

Dosage form - tablets: flat-cylindrical, chamfered and scored, light yellow or light yellow with a greenish tinge (2, 3 or 6 pcs. In polymer cans; 2 or 3 pcs. In blisters, in cardboard pack 1 or 2 packs).

Composition of 1 tablet:

  • active substance: mifepristone - 200 mg;
  • additional components: sodium carboxymethyl starch, calcium stearate, talc, microcrystalline cellulose.

Pharmacological properties

Pharmacodynamics

Mifepristone - the active substance of Miropristone - is a synthetic steroidal antigestagenic agent (it blocks the action of progesterone at the receptor level), which does not possess gestagenic activity.

Increases the contractility of the myometrium, stimulates the release of interleukin-8 in choriodecidual cells, increases the sensitivity of the myometrium to prostaglandins.

Miropristone promotes desquamation (flaking) of the decidua and the removal of the ovum.

Due to competition at the level of binding to receptors, mifepristone has antagonism with glucocorticosteroids.

Pharmacokinetics

After a single oral dose of 600 mg mifepristone, a maximum concentration of 1.98 mg / L is reached after 1.3 hours. Absolute bioavailability - 69%.

Mifepristone has a high binding to plasma proteins (98%), namely acid alpha1-glycoprotein and albumin.

After the distribution phase, excretion begins slowly, in the period between 12–72 hours, the concentration decreases by 2 times, then the rate of excretion increases.

The half-life is 18 hours.

Indications for use

  • termination of uterine pregnancy in the early stages (up to 42 days of amenorrhea) - Miropristone is used in combination with misoprostol;
  • preparation and induction of labor.

Contraindications

  • uterine fibroids;
  • the presence of severe extragenital pathology;
  • acute inflammatory diseases of the female genital organs;
  • disorders of hemostasis (including previous treatment with anticoagulants);
  • anemia (hemoglobin level less than 100 g / l);
  • acute or chronic renal and / or hepatic failure;
  • adrenal insufficiency;
  • porphyria;
  • long-term glucocorticosteroid therapy;
  • smoking over the age of 35 without first consulting a therapist;
  • a history of hypersensitivity to mifepristone.

Additional contraindications for medical abortion:

  • pregnancy exceeding 42 days of amenorrhea;
  • pregnancy not confirmed by clinical trials;

Instructions for the use of Miropriston: method and dosage

Miropriston tablets are used only in medical institutions equipped with the necessary equipment, under the close supervision of a qualified doctor.

Early medical termination of pregnancy

In the presence of a doctor, a woman drinks 3 tablets once (600 mg of mifepristone in total) with 100 ml of boiled water. The drug should be taken 1–1.5 hours after a light breakfast. After 36–48 hours, the patient must report to the medical facility to receive misoprostol at a dose of 0.4 mg, after which she remains under dynamic medical supervision for 2 hours.

After 10-14 days, the woman undergoes a clinical and ultrasound examination, if necessary, the level of chorionic gonadotropin is determined to confirm that the pregnancy has ended.

If after 14 days the effect of the drug is absent (i.e. the pregnancy persists or an incomplete abortion has occurred), mandatory vacuum aspiration and subsequent histological examination of the aspirate are indicated.

Preparation and induction of labor

In the presence of a doctor, a woman takes 1 tablet of Miropriston once, after 24 hours, the reception is repeated. After another 48–72 hours, the condition of the birth canal is assessed, if necessary, oxytocin or prostaglandins are prescribed.

Side effects

Miropristone can cause the following side effects: headache, general weakness, dizziness, hyperthermia, nausea and vomiting, discomfort in the lower abdomen, urticaria.

The procedure for medical abortion can be accompanied by pain in the lower abdomen and bloody discharge from the genital tract. It is also possible to exacerbate the inflammatory processes of the uterus and appendages.

Overdose

When taken in doses up to 2000 mg, mifepristone does not cause adverse events.
