Pharmacological properties
Pharmacodynamics
Askofen-P is one of the combined analgesic drugs.
The mechanism of action is due to the properties of the active substances that make up Askofen-P:
- acetylsalicylic acid: has anti-inflammatory, antipyretic and analgesic effects, which is associated with the suppression of COX-1 and COX-2 (cyclooxygenase), which regulate the synthesis of prostaglandins; inhibits platelet aggregation;
- caffeine: increases the reflex excitability of the spinal cord, excites the vasomotor / respiratory centers, dilates the blood vessels of the brain, kidneys, heart, skeletal muscles, reduces platelet aggregation; helps to increase physical and mental performance, reduce drowsiness and fatigue;
- paracetamol: has analgesic and antipyretic effects.
Indications for use
The indication for Askofen-P is moderate / mild pain syndrome in adults:
- algodismenorrhea;
- thoracic radicular syndrome, lumbago;
- toothache and headache;
- migraine;
- neuralgia;
- myalgia;
- arthralgia.
Also, the drug is indicated for children from 15 years old and adults with acute respiratory and infectious-inflammatory diseases in order to reduce the increased body temperature.
Contraindications
Absolute:
- exacerbation of erosive and ulcerative lesions of the gastrointestinal tract;
- gastrointestinal bleeding;
- vitamin K deficiency;
- severe impairment of renal / hepatic function;
- asthma induced by the intake of acetylsalicylic acid, salicylates and other non-steroidal anti-inflammatory drugs;
- hemorrhagic diathesis (thrombocytopenia, hypoprothrombinemia, hemophilia, von Willebrand disease, telangiectasia, thrombocytopenic purpura);
- surgical interventions accompanied by bleeding;
- deficiency of glucose-6-phosphate dehydrogenase;
- dissecting aortic aneurysm;
- portal hypertension;
- glaucoma;
- sleep disturbance, hyperexcitability, anxiety disorders (panic disorder, agoraphobia);
- organic diseases of the cardiovascular system (acute myocardial infarction, severe ischemic heart disease, arterial hypertension), frequent ventricular premature beats, paroxysmal tachycardia;
- age up to 15 years (as an antipyretic) or 18 years (as an analgesic);
- I and III trimesters of pregnancy, lactation period;
- individual intolerance to the components of the drug, as well as xanthines and other non-steroidal anti-inflammatory drugs.

Instructions for use of Askofen-P: method and dosage
Askofen-P is taken orally, preferably after meals, washed down with milk, water, alkaline mineral water.
Usually, the drug is prescribed 1-2 tablets 2-3 times a day, at intervals of at least 4 or 6 hours (with normal or impaired renal / hepatic function).
Askofen-P is intended for short-term use - 3 days as an antipyretic drug or 5 days as an anesthetic.
Changing the regimen of taking the drug is possible only by agreement with the doctor.
Side effects
- cardiovascular system: tachycardia, increased blood pressure;
- digestive system: gastralgia, anorexia, vomiting, nausea, diarrhea, erosive and ulcerative lesions of the gastrointestinal tract, gastrointestinal bleeding, renal / hepatic failure;
- allergic reactions: Quincke's edema, skin rash, bronchospasm.
During a long course, the following disorders may develop: hypocoagulation, headache, dizziness, tinnitus, visual impairment, decreased platelet aggregation, hemorrhagic syndrome (including bleeding gums, nosebleeds, purpura), kidney damage with papillary necrosis, deafness, syndrome Stevens - Johnson and / or Lyell; in children - Reye's syndrome (manifests itself in the form of metabolic acidosis, mental and nervous system disorders, vomiting, hepatic dysfunction).
Overdose
The main symptoms are tachycardia, sweating, nausea, stomach pain, vomiting, pallor of the skin. Light intoxication is characterized by the appearance of ringing in the ears, severe - anuria, drowsiness, collapse, bronchospasm, convulsions, shortness of breath, bleeding. As intoxication intensifies, progressive respiratory paralysis and uncoupling of oxidative phosphorylation may develop, which can cause respiratory acidosis.
Therapy: gastric lavage, intake of activated charcoal.
In cases where there is a suspicion of poisoning, medical attention is required.
special instructions
With prolonged use of Askofen-P, it is necessary to monitor the peripheral blood and the functional state of the liver.
Acetylsalicylic acid, which is part of the drug as an active component, has an antiplatelet effect, therefore, before surgical intervention, the doctor must be warned about the therapy.
Low doses of acetylsalicylic acid reduce the excretion of uric acid, which in some cases can cause a gout attack.
During the period of therapy, it is recommended to stop drinking alcoholic beverages, since in this case the risk of gastrointestinal bleeding increases.
Application during pregnancy and lactation
The I and III trimesters of pregnancy, as well as the lactation period, are a contraindication to taking Askofen-P.
When used in the first trimester of pregnancy, acetylsalicylic acid can lead to fetal malformation in the form of cleavage of the upper palate, in the third trimester - to promote inhibition of labor, closure of the ductus arteriosus in the fetus, which causes hypertension in the pulmonary circulation and hyperplasia of the pulmonary vessels.
In cases where acetylsalicylic acid is used by lactating women, the child has an increased risk of bleeding (associated with impaired platelet function).
In the II trimester of pregnancy, a single dose of the drug in recommended doses is allowed only in cases where the expected benefit is higher than the possible harm.
Pediatric use
Due to the danger of developing Reye's syndrome, a contraindication to taking Askofen-P is:
- age up to 15 years - as an antipyretic agent;
- age up to 18 years - as an anesthetic.
With impaired renal function
According to the instructions, Askofen-P is not prescribed for severe renal dysfunction.
For violations of liver function
Even if there are indications, Askofen-P is not prescribed for severe liver dysfunction.
Drug interactions
With the combined use of Askofen-P with some drugs / substances, the following effects may develop:
- spironolactone, furosemide, antihypertensive and anti-gout drugs that promote the excretion of uric acid: reducing their effect;
- steroid hormones, heparin, indirect anticoagulants, hypoglycemic agents, reserpine: increasing their action;
- barbiturates, antiepileptic drugs, zidovudine, rifampicin, alcoholic beverages: increased likelihood of hepatotoxic effect (combination not recommended);
- glucocorticosteroids, sulfonylurea derivatives, methotrexate, non-narcotic analgesics and non-steroidal anti-inflammatory drugs: increased side effects;
- ergotamine: accelerating its excretion (due to caffeine);
- chloramphenicol: a significant increase in the time of its elimination (due to paracetamol);
- metoclopramide: accelerating the absorption of paracetamol;
- salicylamide and other stimulants of microsomal oxidation: an increase in the likelihood of the formation of toxic metabolites of paracetamol, which affect hepatic function;
- anticoagulants (dicoumarin derivatives): enhancing their action (with repeated administration of paracetamol).
