Argosulfan is an antibacterial agent for external use.
Release form and composition
The drug is produced in the form of a cream for external use 2%: a soft homogeneous mass of white or white with a shade from light gray to pink (15 or 40 g each in aluminum tubes, in a cardboard box 1 tube).
The active ingredient is silver sulfathiazole, its content in 1 g of cream is 20 mg.
Excipients: white petrolatum, cetostearyl alcohol, liquid paraffin, methyl hydroxybenzoate, glycerol, potassium dihydrogen phosphate, sodium lauryl sulfate, propyl hydroxybenzoate, sodium hydrogen phosphate, water for injection.
Pharmacological properties
Argosulfan is a topical antibacterial agent. It provides effective protection of wound surfaces from infection, promotes healing of trophic, burn and purulent wounds, reduces the time of therapy and the period of preparation of the wound for skin transplantation. In many cases, there is an improvement in the condition, eliminating the need for transplantation.
Pharmacodynamics
The active component of the drug, silver sulfathiazole, is an antimicrobial bacteriostatic agent with a broad spectrum of antibacterial action against gram-positive and gram-negative bacteria.
The mechanism of the antimicrobial action of sulfathiazole is based on inhibition of the growth and reproduction of microbes, which is associated with competitive antagonism with para-aminobenzoic acid and inhibition of dihydropteroate synthetase. As a result of the application, there is a violation of the synthesis of dihydrofolic acid and, as a consequence, tetrahydrofolic acid (its active metabolite), which is required for the synthesis of pyrimidines and purines of the microbial cell.
Silver ions, which are part of Argosulfan, enhance the antibacterial effect of sulfonamide. They help to inhibit the growth and division of bacteria by binding to deoxyribonucleic acid of a microbial cell. Also, the use of silver ions leads to a weakening of the sensitizing properties of sulfanilamidanilamide.
Argosulfan has no toxic effect on the body (due to its minimal resorption).
Pharmacokinetics
Silver sulfathiazole contained in Argosulfan has little solubility. Due to this, the concentration of the active substance in the wound after topical application is continuously maintained at the same level. Silver sulfathiazole only in a small amount appears in the bloodstream, after which it undergoes acetylation in the liver.
In the urine, the substance is present in the form of inactive metabolites and partially unchanged. After application to extensive wound surfaces, the absorption of silver sulfathiazole increases.
Indications for use
- Purulent wounds;
- Burns of any etiology (all degrees of severity): solar, thermal, chemical, radiation, electric shock;
- Streptostaphyloderma, microbial eczema, impetigo, simple contact and infected dermatitis;
- Frostbite;
- Trophic ulcers of the lower leg of various origins, including diabetic angiopathies, chronic venous insufficiency, erysipelas, obliterating endarteritis;
- Bedsores;
- Cuts, abrasions and other minor household injuries.
Contraindications
- Prematurity, children's age up to 2 months (due to the high risk of developing "nuclear" jaundice);
- Congenital deficiency of glucose-6-phosphate dehydrogenase;
- Hypersensitivity to sulfonamides (including sulfathiazole).
Argosulfan cream is prescribed with caution to patients: with extensive burns, in a state of shock, due to the impossibility of collecting a full allergic anamnesis; for violations of liver and / or kidney function.
Instructions for the use of Argosulfan: method and dosage
Argosulfan cream is applied externally, treatment is prescribed by an open method or using occlusive dressings.
The cream is applied to the affected area and spread in an even layer of 2-3 mm. Manipulations are performed under sterile conditions 2-3 times a day until the wound is completely healed or skin transplanted. During therapy, the cream should completely cover all areas of the lesion; if part of the wound opens, the layer of coverage should be restored.
If exudate is formed during the treatment of infected wounds with Argosulfan, before re-applying the cream, the wound must be cleaned of it and treated with an antiseptic solution (aqueous solution of chlorhexidine 0.1%).
The maximum permissible daily dose of the cream is 25 g. The maximum duration of treatment is no more than two months.
Side effects
The use of the drug can cause a burning sensation, itching and irritation at the site of application, the development of allergic reactions.
With prolonged use of the cream, side effects can be desquamatous dermatitis, leukopenia.
Overdose
There is no information.
special instructions
When treating patients with renal and / or hepatic impairment, it is recommended to monitor the serum sulfathiazole content.
Influence on the ability to drive vehicles and complex mechanisms
According to the instructions, Argosulfan does not affect the ability to control complex mechanisms and drive vehicles.
Application during pregnancy and lactation
Argosulfan can be used during pregnancy / lactation only after assessing the ratio of the expected benefit with the possible risk in cases where the burn area does not exceed 20% of the body surface.
Pediatric use
It is contraindicated to use the cream for the treatment of premature babies and infants under 2 months of age.
With impaired renal function
Argosulfan for impaired renal function should be used under medical supervision.
For violations of liver function
Argosulfan for violations of hepatic function should be used under medical supervision.
Drug interactions
The cream is not recommended to be applied simultaneously with other external medications.
The combination with folic acid and its structural analogs reduces the antimicrobial properties of the drug.
