Pharmacological properties
Pharmacodynamics
The amino acids that make up Aminovena Infant are among the physiological components. After parenteral administration of the solution, they are included in the pool of free amino acids of the body and take part in all metabolic processes, including used for protein biosynthesis.
Pharmacokinetics
When administered intravenously, the bioavailability of Aminovena Infant is 100%. After inclusion in the total pool of free amino acids of the body, distribution occurs in the interstitial fluid and the intercellular space of tissues and organs.
The concentration of free amino acids in the cytoplasm of cells and blood plasma is regulated within narrow limits and depends on several factors, including age, nutritional status and general condition of the patient. Aminoven Infant, when administered correctly (at a constant rate and slowly), does not lead to an imbalance of amino acids. In patients with severely impaired liver and kidney function, there is a dysregulation of the amino acid balance. In such cases, special prescriptions of amino acid solutions should be used for parenteral nutrition.
Only a small part of amino acids is eliminated by the kidneys. The value of the half-life of a drug from plasma is largely determined by age.
Indications for use
Aminoven Infant is prescribed for partial parenteral nutrition of newborns, premature infants and young children.
Complete parenteral nutrition is provided with simultaneous use with fat emulsions, carbohydrate solutions, as well as preparations of vitamins, trace elements and electrolytes.
Contraindications
Absolute:
- overhydration;
- metabolic acidosis;
- metabolic disorders of amino acids;
- hypokalemia.
Relative (the use of Aminovena Infant requires individual dosing):
- liver failure;
- renal failure.
Aminoven Infant, instructions for use: method and dosage
The drug is intended for long-term drip intravenous administration. The duration of use is determined by the need for parenteral nutrition.
Aminoven Infant 6%
The drug is injected into the central or peripheral veins.
Maximum daily dose:
- up to 1 year: 1.5-2.5 g / kg, or 25-40 ml / kg;
- 2-5 years: 1.5 g / kg, or 25 ml / kg.
The maximum injection rate is up to 0.1 g / kg, which is 1.67 ml / kg per hour.
Aminoven Infant 10%
The drug is injected mainly into the central veins.
Maximum daily dose:
- up to 1 year: 1.5–2.5 g / kg, or 15–25 ml / kg;
- 2–5 years: 1.5 g / kg, or 15 ml / kg;
- 6-14 years: 1 g / kg, or 10 ml / kg.
The maximum injection rate is up to 0.1 g / kg, which is 1 ml / kg per hour.
Side effects
Adverse events with the correct use of Aminovena Infant were not found.
In cases of infusion of the drug into peripheral veins, signs of a local reaction in the form of redness, phlebitis, thrombosis are possible. It is recommended to monitor the puncture areas daily.
Overdose
The main symptoms (if the rate of administration of the drug or dosage is exceeded): chills, vomiting, nausea, renal aminoacidosis. Acute circulatory disorders are also possible.
Therapy: immediate cancellation of the administration of Aminovena Infant. In case of hyperkalemia, a 5% glucose solution is injected at a dose of 200-500 ml, with 1-3 IU (international units) of insulin added for every 3-5 g of glucose.
special instructions
If infused too quickly, the loss of amino acids through the kidneys is possible, resulting in an imbalance of amino acids.
In young children, with parenteral nutrition, the following indicators should be taken into account: urine nitrogen, content of electrolytes, ammonia, glucose, triglycerides (with additional administration of fat emulsions), serum osmolarity, liver enzymes, water-salt metabolism and acid-base balance.
With impaired renal function
In renal failure, the dose is selected individually.
For violations of liver function
In case of liver failure, the dose is selected individually.
Drug interactions
Due to the increased risk of microbial contamination, Aminoven Infant should not be mixed with other drugs.
The addition of other substances / drugs to Aminoven Infant should be avoided, due to the likelihood of toxic reactions. In any case, you should be sure of the compatibility of the drugs, thorough mixing and maintaining sterility. Solutions with the addition of other medicines are not subject to storage.
