Release form and composition
The drug is available in the form of eye drops based on brinzolamide. The drops are sold in 5 ml Drop Tainer plastic bottles with a dropper dispenser.
1 ml of 1% ophthalmic suspension contains 10 mg of brinzolamide.
Excipients:
- Benzalkonium chloride;
- Mannitol;
- Sodium chloride;
- Tyloxapol;
- Purified water;
- Hydrochloric acid and / or sodium hydroxide;
- Disodium edetad;
- Carbomer (974P).
Brinzolamide enters the systemic circulation and is adsorbed in the red blood cells. It is excreted in the urine.
Pharmacological properties
Pharmacodynamics
Brinzolamide, the active ingredient of the drug, is a carbonic anhydrase II inhibitor. Inhibition of carbonic anhydrase II leads to a slowdown in the formation of bicarbonate ions and a subsequent decrease in the transport of fluid and sodium, which ultimately leads to a decrease in the formation of intraocular fluid in the ciliary body of the eye and a decrease in IOP (intraocular pressure).
Pharmacokinetics
After instillation of the drug, brinzolamide enters the systemic circulation. As a result of selective binding, it is adsorbed in erythrocytes. The resulting metabolite (N-desethylbrinzolamide) also accumulates in erythrocytes and binds to carbonic anhydrase. N-desethylbrinzolamide in the presence of brinzolamide mainly binds to carbonic anhydrase I.
Plasma concentrations of brinzolamide and its metabolites are below the level of quantitation. The drug is excreted in the urine both unchanged and in the form of metabolites (low concentrations of N-desethylbrinzolamide, O-desmethyl and N-desmethoxypropyl are found in urine).
Indications for use
Azopt eye drops are prescribed to lower intraocular pressure in ocular hypertension or open-angle glaucoma.
The action of the drops is based on the ability of the active substance of the drug to reduce the production of intraocular fluid in the ciliary body of the eye, resulting in a decrease in eye pressure.
Contraindications
In accordance with the instructions, it is forbidden to use Azopt in case of individual sensitivity to any component of the drug.
A contraindication is severe renal dysfunction, since the active substance and its metabolites are excreted by the kidneys.
There are no data on the safety and efficacy of brinzolamide treatment in childhood, so it is not prescribed for children.
Caution is required when prescribing to patients with severe liver disease or angle-closure glaucoma, since the use of Azopt for these pathologies has not been studied.
Instructions for the use of Azopt: method and dosage
Azopt is used 1 drop 2 times a day by instillation into the conjunctival sac of the affected eye.
Shake the bottle before use.
Side effects
Azopt eye drops can cause the following side effects:
- A bitter or sour taste in your mouth, blurred vision;
- Feeling of a foreign body or dry eyes, blepharitis, dermatitis, redness, headaches, mucous discharge from the eyes and nose, itching, pain;
- Conjunctivitis, keratoconjunctivitis, lacrimation, corneal lesions, crust on the edge of the eyelid;
- Shortness of breath, dizziness, chest pain, increased blood pressure, diarrhea, nausea, indigestion, dry mouth, lower back pain;
- Hives, pharyngitis, allergic reactions, alopecia.
If the instructions are violated, there is a risk of developing sensitization of the body with sulfonamides.
With the accidental use of drops inside, the development of acidosis, electrolyte imbalance and the work of the nervous system is possible. In this case, symptomatic therapy and control over the level of electrolytes and blood pH are necessary.
There are no data on drug overdose.
Overdose
With local application of the drug, no information about an overdose has been reported.
If Azopt is accidentally swallowed, an acidosis state, electrolyte imbalance and reactions from the nervous system may develop. Blood pH and potassium and other electrolytes should be monitored.

The pipette tip of the opened bottle should not touch any surfaces, including the eyelids and skin of the hands.
Benzalkonium chloride can be absorbed by contact lenses. Before using drops, lenses should be removed and reinstalled after 15-20 minutes.
Immediately after eye instillation, blurred vision may temporarily occur, so care should be taken when driving vehicles and when working with equipment.
Application during pregnancy and lactation
The use of the drug during pregnancy is not recommended, since adequate and strictly controlled studies of the safety of its use in pregnant women have not been conducted. The use of Azopt is possible only in cases where the expected benefit to the mother outweighs the possible risk to the fetus.
There are no data on the excretion of brinzolamide in breast milk, therefore, when prescribing the drug during breastfeeding, the question of stopping breastfeeding is decided.
Pediatric use
According to the instructions, Azopt is contraindicated for use in children and adolescents under 18 years of age, since the safety and effectiveness of its use in children and adolescents have not been established.
With impaired renal function
In case of severe renal dysfunction (with creatinine clearance less than 30 ml / min), the use of the drug is not recommended.
For violations of liver function
Azopt is used with caution in patients with severely impaired liver function.
Drug interactions
The drug is not recommended to be used concomitantly with oral carbonic anhydrase inhibitors, since systemic side effects may increase.
High doses of salicylates also increase the risk of developing systemic adverse reactions.
Azopt can be used in combination with other ophthalmic preparations for topical use, but the interval between instillation of different agents should be at least 10 minutes.
Analogs
Brinzolamide is not included in other antiglaucoma drugs.
Azopt's analogues in terms of the mechanism of action are:
