Pharmacological properties

Pharmacodynamics

Azithromycin is a broad-spectrum antibacterial drug, the first representative of azalides - a new subgroup of macrolides.

The drug binds to the 50S subunit of ribosomes and inhibits peptide translocase at the translation stage, slows down the reproduction and growth of bacteria, and suppresses protein synthesis. It has a bacteriostatic effect, in high concentrations - a bactericidal effect. Acts on extra- and intracellular pathogens.

Azithromycin-Akrikhin is active against the following microorganisms:

  • gram-negative bacteria: Campylobacter jejuni, Bordetella pertussis, Haemophilus influenzae, Haemophilus ducreyi, Bordetella parapertussis, Neisseria gonorrhoeae, Legionella pneumophila, Gardnerella vaginalis, Moraxella catarrhalis;
  • gram-positive bacteria: Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus agalactiae, Streptococcus pyogenes, Streptococcus viridans, Streptococcus pneumoniae, Streptococcus spp. (groups C, F and G, except for those resistant to erythromycin);
  • some anaerobes: Clostridium perfringens, Bacteroides bivius, Peptostreptococcus spp.;

Azithromycin is ineffective for infections caused by erythromycin-resistant gram-positive bacteria.

Pharmacokinetics

After oral administration from the gastrointestinal tract, azithromycin is rapidly absorbed, since it is stable in an acidic environment and lipophilic.

After applying a dose of 500 mg, the maximum plasma concentration (C max) is 0.4 mg / l and is noted after 2.5–3 hours. Bioavailability is 37%. Azithromycin is characterized by a first pass effect through the liver.

The drug penetrates well into the respiratory tract, soft tissues and skin, organs and tissues of the urogenital tract (in particular, the prostate gland), through the histohematogenous barriers. It accumulates in a low pH environment and in lysosomes, which plays an important role in the eradication of intracellular pathogens. It is also transported by phagocytes, macrophages and polymorphonuclear leukocytes. Penetrates through cell membranes, where it creates high concentrations.

The high concentration in tissues and cells (10-50 times higher than serum) and the long half-life (T ½) of azithromycin are due to its low binding. The volume of distribution is 31.1 l / kg.

In the foci of infection, compared with healthy tissues, the drug reaches significantly higher concentrations (by 24–34%), which correlate with the severity of inflammatory edema, and remains in effective concentrations for 5–7 days after taking the last dose. Plasma proteins bind 7-50% (inverse proportion to the concentration in the blood).

Undergoes the process of demethylation in the liver with the formation of pharmacologically inactive metabolites. Plasma clearance is 630 ml / min. T ½ when taking the drug at intervals of 8-24 hours is 14-20 hours, at intervals of 24-72 hours - 41 hours, which allows you to take Azithromycin-Akrikhin 1 time per day.

It is excreted: 50% - unchanged through the intestines, 6% - by the kidneys.

Indications for use

Azithromycin-Akrikhin is used to treat diseases caused by sensitive microorganisms:

  • infections of ENT organs and upper respiratory tract: sinusitis, laryngitis, tonsillitis, pharyngitis, otitis media;
  • infections of the lower respiratory tract: bronchitis, pneumonia (including atypical);
  • scarlet fever;
  • infections of the urogenital tract: cervicitis, urethritis (including gonorrheal);
  • infections of the skin and soft tissues: impetigo, erysipelas, secondarily infected dermatoses;
  • the initial stage (erythema migrans) of Lyme disease (borreliosis);
  • peptic ulcer of the stomach and duodenum associated with the bacterium Helicobacter pylori (as part of combination therapy).

Contraindications

Absolute:

  • liver failure;
  • renal failure;
  • age up to 12 years;
  • period of pregnancy and lactation;
  • hypersensitivity to the components of the drug or other antibiotics of the macrolide group.

Azithromycin-Akrikhin should be used with caution in patients with arrhythmias, as well as in children with impaired renal and / or liver function.

Azithromycin-Akrikhin should be taken orally, 1 hour before meals or 2 hours after meals.

Recommended doses for adults:

  • infections of the upper and lower respiratory tract: Azithromycin-Akrikhin 500 mg once a day for 3 days;
  • infections of the skin and soft tissues: the first day - 1000 mg once a day, from the second to the fifth day - 500 mg once a day;
  • acute urinary tract infections: 1000 mg once;
  • Stage I (erythema migrans) of Lyme disease: the first day - 1000 mg, from the second to the fifth day - 500 mg once a day;
  • peptic ulcer and 12 duodenal ulcer associated with Helicobacter pylori: 1000 mg 1 time per day for 3 days in addition to standard anti-Helicobacter pylori therapy.

For children from 12 years old, the dose of Azithromycin-Akrikhin is determined depending on body weight and clinical features of the disease. Possible treatment options:

  1. 10 mg / kg 1 time per day for 3 days.
  2. On the first day - 10 mg / kg, over the next 3-4 days - 5-10 mg / kg once a day (course dose - 30 mg / kg).

For the treatment of erythema migrans in children on the first day, 20 mg / kg is prescribed, from the second to the fifth day - 10 mg / kg once a day.

If the next dose is missed, the drug should be taken as soon as possible, and subsequent doses should be carried out at intervals of 24 hours.

Side effects

  • digestive system: diarrhea (5%), nausea (3%), abdominal pain (3%), flatulence, vomiting, increased activity of hepatic transaminases; in children - constipation, gastritis, anorexia;

Overdose

An overdose of Azithromycin-Akrikhin can manifest itself with the following symptoms: diarrhea, severe nausea, vomiting, temporary hearing loss.

  • slows down the excretion and increases the plasma concentration and toxicity of the following drugs (by inhibiting microsomal oxidation in hepatocytes with azithromycin): indirect anticoagulants, cycloserine, felodipine, methylprednisolone, as well as agents that undergo microsomal oxidation, such as ergot alkaloids, phenyphenadic acid, valphenytoin, terp. bromocriptine, hexobarbital, carbamazepine, disopyramide, cyclosporine, theophylline and other xanthine derivatives, oral hypoglycemic drugs.
