Furagin-Aktifur is a nitrofuran derivative, an antibacterial drug.

Release form and composition

The drug is available in the form of capsules: gelatinous, size No. 3 (25 mg each) or No. 1 (50 mg each), with a yellow hard shell, inside the body - orange powder (10, 15, 20 or 30 pcs. In contour cell packages, in a cardboard box 1, 2, 3, 4 or 5 packages and instructions for use of Furagin-Aktifur).

1 capsule contains:

  • active substance: sodium furazidine - 31.25 mg or 62.5 mg (in terms of 100% anhydrous substance - 25 mg or 50 mg, respectively);
  • auxiliary components: pregelatinized corn starch, basic aqueous magnesium carbonate, croscarmellose sodium, aerosil (colloidal silicon dioxide), talc;
  • capsule body and cap: gelatin, titanium dioxide, sunset yellow dye, quinoline yellow dye.

Pharmacological properties

Pharmacodynamics

Furagin-Aktifur is a uroantiseptic, antimicrobial agent with a wide spectrum of action. The active ingredient is furazidine, which belongs to the group of nitrofurans. Shows an antibacterial effect against many antibiotic-resistant microorganisms, including gram-positive and gram-negative microbes. Furazidin is active against gram-positive cocci (streptococci, staphylococci), pathogenic strains of Staphylococcus speciales (spp.), Gram-negative rod-shaped bacteria (Escherichia coli, Salmonella, Klebsiella, lterobacteria, Shigella), Protozoa. Unlike other nitrofurans and groups of antimicrobial agents, it is more effective in the treatment of staphylococcal infections, Escherichia coli, Aerobacter aerogenes, Bacterium citrovorum, Proteus morganii, Proteus mirabilis, Enterococcus faecalis, Staphylococcus spp. The advantage of furazidine is the slow development of resistance, which does not reach a high degree.

The mechanism of action of nitrofurans is due to their ability to suppress the respiratory chain and the tricarboxylic acid cycle (Krebs cycle) in microorganisms, to inhibit their other biochemical processes, which leads to the destruction of their cytoplasmic membrane or membrane. The bacteriostatic concentration against most bacteria can range from 1: 100,000 to 1: 200,000, the bactericidal concentration is approximately two times higher. Against the background of exposure to nitrofurans, there is less release of toxins by microorganisms, therefore, a general improvement in the condition is possible before the onset of pronounced suppression of bacterial growth. Nitrofurans do not have a depressing effect on the body's immune system. Unlike other antimicrobial agents, they have the property of increasing the titer of complement and the ability of leukocytes to phagocytose microorganisms that have entered the bloodstream,which causes the activation of the immune system during therapy. In therapeutic doses, nitrofurans stimulate leukopoiesis.

Pharmacokinetics

After taking Furagin-Aktifur, the absorption of furazidine occurs as a result of passive diffusion in the small intestine. With the simultaneous treatment of urogenital infections and gastrointestinal diseases (including chronic enteritis), it should be borne in mind that the absorption of furazidine from the distal segment of the small intestine is 2 times higher than the absorption from the proximal segment, and 4 times from the medial segment. In the colon, nitrofurans are poorly absorbed. The drug has a high degree of bioavailability, which is due to the presence of basic magnesium carbonate in its composition. Getting into the acidic environment of the stomach, sodium furazidine is not converted into poorly soluble furazidine.

The clinical effect is achieved due to the uniform distribution of furazidine in the body. High concentrations of the active substance in the lymph inhibit the spread of infection through the lymphatic tract. Its concentration in serum is several times lower than in bile and several times higher than in cerebrospinal fluid. The content of furazidine in saliva is 30% of serum concentrations. Due to its rapid excretion, its content in tissues and blood is relatively low. The concentration of furazidine in urine significantly exceeds its level in the blood. The maximum concentration of the active substance in the blood plasma is maintained for 3-8 hours, in the urine - 3-4 hours after taking the capsule.

Less than 10% of the administered dose undergoes biotransformation. The metabolic rate increases with reduced renal excretory function.

Furazidine does not affect the acidity of urine.

It is excreted through the kidneys up to 85% by glomerular filtration and tubular secretion, partially undergoes reverse reabsorption in the tubules. The filtration and secretion process occurs at low concentrations of furazidine in the urine. With a high content of the active substance, the secretion of furazidine decreases, and the reabsorption increases. Due to the low acidity, furazidine does not disintegrate in the acidic environment of urine, but undergoes intense reabsorption. This can increase the risk of developing systemic side effects. Alkalization of urine enhances the excretion of furazidine.

Indications for use

The use of Furagin-Aktifur is indicated for the treatment of gynecological and urogenital infectious diseases caused by microorganisms sensitive to furazidine, including acute cystitis, urethritis, pyelonephritis.

In addition, the drug is prescribed for the prevention of bacterial infections during cystoscopy, catheterization and urological operations.

Contraindications

  • severe chronic renal failure;
  • impaired renal function;
  • period of pregnancy;
  • lactation;
  • age up to 3 years;
  • established hypersensitivity to nitrofurans;
  • individual intolerance to the components of the drug.

Furagin-Aktifur should be prescribed with caution in case of glucose-6-phosphate dehydrogenase deficiency.

Furagin-Aktifur capsules are taken orally, after a meal, swallowing whole and drinking plenty of liquid.

Recommended dosage for the treatment of urogenital and gynecological infections:

  • adults: 50-100 mg 3 times a day;
  • children over three years old: 25-50 mg 3 times a day. The maximum daily dose should not exceed 5 mg per 1 kg of the child's body weight.

The duration of the course of treatment is from 7 to 10 days. A second course can be prescribed no earlier than 10-15 days after the end of the previous treatment.

If you accidentally skip the next dose, it cannot be reimbursed by taking a double dose. At the appointed time, you should take the usual dose of Furagin-Aktifur.

In order to prevent bacterial infection during cystoscopy, catheterization or urological operations, a single dose of the drug is prescribed 0.5 hours before the procedure in the following doses: adults - 50 mg, children - 25 mg.

Side effects

  • from the nervous system: often - headache; infrequently - dizziness; rarely - polyneuritis;
  • dermatological reactions: rarely - skin rash and / or papular rash of an allergic nature;

Overdose

Symptoms: ataxia, tremors, neurotoxic symptoms.

  • ristomycin, sulfonamides, chloramphenicol: increase the risk of inhibition of hematopoiesis, therefore, their combination with furazidine is not recommended;
  • calcium chloride, ascorbic acid: can increase the acidity of urine (drugs that can increase the acidity of urine should not be taken together with furazidine);
  • ethanol: may increase the side effects of furazidine.
