Flamax forte is a non-steroidal anti-inflammatory drug (NSAID).

Release form and composition

Dosage form Flamax forte - film-coated tablets: blue, biconvex, round (in blisters of 10 pcs., In a cardboard box 2 blisters).

Composition of 1 tablet:

  • active substance: ketoprofen - 0.1 g;
  • auxiliary components: brilliant blue, titanium dioxide, talc, macrogol 6000, hypromellose, colloidal silicon dioxide, sodium lauryl sulfate, magnesium stearate, microcrystalline cellulose, povidone, croscarmellose sodium, rice starch, lactose monohydrate.

Pharmacological properties

Pharmacodynamics

Flamax forte has antiaggregatory, antipyretic, analgesic and anti-inflammatory properties, and exhibits antibradykinin activity. The drug inhibits the activity of cyclooxygenase-1 (COX-1) and COX-2, which regulate the synthesis of prostaglandins, stabilizes lysosomal membranes.

The tablets have an analgesic effect, which is due to both central and peripheral mechanisms.

Pharmacokinetics

Pharmacokinetic characteristics of ketoprofen:

  • absorption: quickly absorbed; bioavailability is 90%; With max (maximum concentration in blood plasma) is achieved after ½ – 2 hours after administration due to low Vd (volume of distribution); absolute bioavailability varies within 64–80%; food intake has no effect on its absorption; equilibrium concentration (Css) in plasma is reached after 1 day after the start of regular intake; With max in synovial fluid - 30% of the concentration in serum, after 4-6 hours it increases; well penetrates into connective tissues and synovial fluid; penetrates the blood-brain barrier;
  • distribution: Vd - 0.1–0.2 l per 1 kg; almost 99% of absorbed ketoprofen binds to proteins (mainly albumin);
  • metabolism: through glucuronidation, it is almost completely metabolized in the liver; has the effect of the first passage through the liver;
  • excretion: mainly excreted by the kidneys and intestines (1–8%); T 1/2 (half-life) varies from 1.6 to 1.9 hours; T 1/2 in elderly patients increases to 3-5 hours, in patients with renal insufficiency - approximately 3.5 hours; does not cumulate.

Indications for use

According to the instructions, Flamax forte is used to treat inflammatory and degenerative pathologies of the musculoskeletal system:

  • pain syndrome;
  • toothache and headache;
  • post-traumatic and postoperative pain syndrome, accompanied by inflammation;
  • oncological diseases;
  • algodismenorrhea;
  • arthritis: rheumatoid, psoriatic, gouty;
  • ankylosing spondylitis (ankylosing spondylitis);
  • osteoarthritis;
  • radiculitis, tendinitis, bursitis;
  • otitis;
  • adnexitis;
  • myalgia, neuralgia, arthralgia, ossalgia.

Contraindications

Absolute:

  • hemophilia and other blood clotting disorders;
  • insufficiency of the circulatory system;
  • severe hepatic / renal failure;
  • cerebrovascular, gastrointestinal and other bleeding (or suspicion of them);
  • a history of bronchial asthma, the development of which was the use of ketoprofen, another NSAID, including acetylsalicylic acid;
  • peptic ulcer;
  • diverticulitis, Crohn's disease, ulcerative colitis;

Instructions for the use of Flamax forte: method and dosage

Flamax forte tablets are taken orally, along with food.

The adult dose is 1 pc. 2-3 times a day every 8 hours. The maximum dose is 3 pcs. in a day.

Side effects

  • digestive system: hepatitis, increased activity of hepatic transaminases in blood serum, hemorrhoidal / gastrointestinal / gingival bleeding, ulceration and perforation of the gastrointestinal mucosa, taste changes, stomatitis, dyspepsia, abdominal pain, NSAID-gastropathy;
  • nervous system: peripheral neuropathy, migraine, forgetfulness, confusion or loss of consciousness, asthenia, depression, drowsiness, nervousness, agitation, insomnia, dizziness, headache;
  • sensory organs: vertigo, hearing loss, conjunctival hyperemia, eye pain, dryness of the mucous membrane of the eye, conjunctivitis, blurred vision, noise or ringing in the ears;
  • cardiovascular system: tachycardia, increased blood pressure;

Overdose

In case of an overdose, it is possible to develop abdominal pain, vomiting, nausea, drowsiness, impaired renal and liver function, and bleeding.

Therapy: symptomatic treatment, including gastric lavage and / or intake of activated charcoal (or other sorbents).

Strict medical supervision is required when combined therapy with ketoprofen with coumarin anticoagulants (including warfarin), lithium salts.

During the period of application of the drug in patients with a history of ulcerative pathologies of the gastrointestinal tract, renal / hepatic insufficiency, control the picture of peripheral blood and the functional state of the liver and kidneys.

  • tricyclic antidepressants, phenylbutazone, rifampicin, barbiturates, ethanol, phenytoin (inducers of microsomal oxidation in the liver): increase the production of its hydroxylated active metabolites;
  • cholestyramine, antacids: reduce its absorption;
  • myelotoxic drugs: increase the manifestations of its hematotoxicity.

With the simultaneous use of ketoprofen with other NSAIDs, corticotropin, ethanol, glucocorticosteroids, ulceration, the development of gastrointestinal bleeding, an increased risk of renal dysfunction is possible; with sodium valproate - violation of platelet aggregation; with cefotetan, cefamandol, cefoperazone, antiplatelet agents, thrombolytics, heparin, oral anticoagulants - an increased risk of bleeding.
