Release form and composition
Dosage form Effezel - gel for external use: light yellow or white, opaque (in a polyethylene tube 2, 15, 30, 45, 60 and 90 g, in a cardboard box 1 tube; in a polypropylene bottle with a dosing device 30 g, carton box 1 bottle).
Gel composition (1 g):
- active ingredients: adapalene - 0.001 g; benzoyl peroxide - 0.025 g;
- auxiliary components: simulgel 600 PHA (acryloyldimethyl taurate, acrylamide copolymer, sorbitan oleate, polysorbate, isohexadecane, water), disodium edetate, sodium docusate, propylene glycol, poloxamer, glycerol, purified water.

Pharmacodynamics
Effezel contains two active mutually complementary substances that have different mechanisms of action.
Adapalene is a chemically stable derivative of naphthoic acid, a new generation synthetic retinoid. It helps to normalize the processes of differentiation and keratinization of the epidermis, has a pronounced comedolytic activity, anti-comedogenic, sebostatic and anti-inflammatory effects. In the most common form of acne, acne vulgaris, the main mechanism of action of adapalene is selective binding to specific nuclear receptors of epidermal cells, resulting in an increase in the final differentiation of keratinocytes and a decrease in the prerequisites for the formation of microcomedones. The anti-inflammatory effect of adapalene is due to the inhibition of lipoxygenase, cytokines, arachidonic acid. The active substance in vitro reduces the chemotactic (targeted) and chemokinetic (voluntary) activity of polymorphonuclear leukocytes, and also inhibits the AP-1 transcription factor and the expression of toll-like receptors 2. Benzoyl peroxide has an antimicrobial effect against the pathogen Propionibacterium acnes and other bacteria that are present in acne in sebaceous hair follicles. The substance cleanses the skin from horny layers, has an exfoliating, keratolytic effect, as well as a sebostatic effect - it reduces the excessive oiliness of the skin, which usually accompanies acne.
Pharmacokinetics
Adapalene is absorbed through the skin in small quantities (approximately 4% of the dose used), excreted from the body mainly in the bile.
The absorption of benzoyl peroxide through the skin is very low. The substance is completely transformed into benzoic acid, which is rapidly excreted from the body in the urine.
Indications for use
According to the instructions, Effezel is indicated for the treatment of acne with papules, comedones and pustules.
Contraindications
- children under 9 years of age;
- pregnancy;
- increased individual sensitivity to adapalene, benzoyl peroxide or any of the auxiliary components of the drug.
Instructions for the use of Effezel: method and dosage
Gel Effezel is used externally. Apply a thin layer to the affected skin surface with fingertips 1 time per day in the evening. The drug should be applied to clean and dry skin, avoid contact with eyes, corners of the nose, lips and mucous membranes.
The therapeutic effect appears after 1–4 weeks of using the gel. The duration of treatment is determined by the doctor depending on the clinical condition of the patient.
When signs of skin irritation appear, depending on their intensity, the number of applications of Effezel can be reduced (for example, every other day), or temporarily suspended (until the symptoms of irritation disappear) or treatment can be stopped completely. Gel therapy should be resumed after medical consultation. In case of repeated signs of skin irritation, it is recommended to use non-comedogenic cosmetics with a moisturizing effect.
Side effects
Classification of side effects: often - more than 1%, but less than 10% of cases, infrequently - more than 0.1%, but less than 1% of cases, with an indefinite frequency - if it is impossible to estimate the frequency from the available data. Possible adverse reactions:
- from the skin and subcutaneous tissues: often - irritation, burning sensation of the skin, contact dermatitis, dry skin, erythema, rash, peeling; infrequently - itching, sunburn; with an uncertain frequency (post-registration data) - facial swelling, allergic contact dermatitis, soreness, tingling sensations of the skin, blisters and vesicles;
- on the part of the organ of vision: with an uncertain frequency - edema of the eyelids;
- on the part of the respiratory system, organs of the chest and mediastinum: with an uncertain frequency - a feeling of constriction in the throat.
During the first week of treatment with Effezel, the following symptoms of mild to moderate severity are possible: skin redness, irritation, peeling, dryness, soreness, burning and tingling. As a rule, they go away spontaneously and do not require drug withdrawal.
If the side effects described above are aggravated or any other undesirable reactions are observed, you must inform your doctor about it.
Overdose
In case of accidental ingestion of Effezel gel inside, appropriate symptomatic treatment should be carried out.

Avoid contact of the drug with the eyes, nostrils, oral cavity and other mucous membranes. In case of contact with eyes, rinse immediately with warm water.
Effezel should not be applied to eczematous skin that has been changed or damaged as a result of trauma (abrasions or cuts).
Propylene glycol (E1520), contained in Effezel, can cause skin irritation symptoms. If you suspect hypersensitivity to any ingredient in the drug, it must be canceled.
During treatment with Effezel, exposure to excessive solar or artificial ultraviolet radiation should be avoided.
It is recommended not to allow the preparation to come into contact with colored materials (for example, colored hair or fabrics) due to possible discoloration or discoloration.
Application during pregnancy and lactation
Clinical experience with external use of a combination of adapalene and benzoyl peroxide in women during pregnancy is insufficient. The small amount of data collected from patients in early pregnancy and using Effezel does not indicate any negative effect of the drug.
In animal studies, when using adapalene in high doses, a toxic effect on the reproductive system was noted.
Since the available data are insufficient and there is a possibility of weak penetration of the drug through the skin, Effezel is not recommended for use during pregnancy. In case of pregnancy, the drug should be canceled.
The study of the penetration of active substances into milk after topical application of the gel has not been carried out. A negative effect on infants receiving breast milk is unlikely, since systemic exposure of adapalene and benzoyl peroxide in women during lactation is negligible. Therefore, Effezel can be used during breastfeeding. However, the baby should not be allowed to contact the drug and apply the gel to the breasts.
Pediatric use
The safety and effectiveness of Effezel's use in pediatrics has not been established, therefore, it is prohibited to prescribe it to children under 9 years of age.
Drug interactions
The study of the interaction of adapalene and benzoyl peroxide with other drugs has not been carried out. The mutual influence on the basis of the existing experience with the use of the active substances Effezel and other drugs has not been identified.
Nevertheless, other retinoids, benzoyl peroxide in the composition of other drugs, and drugs with a similar mechanism of action should not be used simultaneously.
The combined use of Effezel with cosmetics that have an exfoliating, drying or irritating effect requires caution due to the additional irritant effect.
The interaction of adapalene and benzoyl peroxide with systemic drugs is unlikely due to their insignificant absorption through the skin.
