Indications for use

Glucobay is used to treat type 2 diabetes mellitus (in combination with diet therapy).

Contraindications

  • severe renal failure (creatinine clearance <25 ml / min);
  • chronic diseases of the digestive system, accompanied by severe disorders of absorption and digestion;
  • conditions in which flatulence is possible (for example, peptic ulcer, Remheld syndrome, impaired intestinal patency, large hernias of any localization);
  • age up to 18 years;
  • period of pregnancy and lactation;
  • hypersensitivity to any component.

Glucobay should be used with caution in the following cases:

  • fever;
  • infectious diseases;
  • surgical interventions;
  • trauma.

Method of administration and dosage

The drug should be taken orally: swallow the tablets whole, without chewing, with sufficient water, immediately before meals, or chewed with the first portion of food.

The appropriate dose is selected for each patient individually.

At the beginning of treatment, it is usually prescribed 50 mg 3 times a day.

The average effective daily dose is 300 mg: 100 mg 3 times a day.

In case of insufficient severity of the therapeutic effect after 4–8 weeks of taking the drug, it is possible to increase the daily dose to 600 mg (maximum): 200 mg 3 times a day.

Elderly people (over 65 years old) and patients with impaired renal function do not need to adjust the dosage regimen.

Side effects

  • allergic reactions: rarely - exanthema, hyperemia, skin rash, urticaria;
  • from the gastrointestinal tract: often - epigastric pain, diarrhea, flatulence; sometimes nausea; rarely - intestinal obstruction, a significant increase in the level of liver enzymes (disappears after discontinuation of the drug), jaundice and / or hepatitis and associated liver damage. There are isolated cases of fatal fulminant hepatitis in Japan, but the connection with the use of acarbose has not been reliably established;
  • others: rarely - edema.

If Glucobay is taken with carbohydrate (di-, oligo- and polysaccharide) drinks and / or foods, diarrhea and flatulence may develop if the recommended daily dose is exceeded. If the drug is taken in excessive doses, regardless of meals, the development of adverse reactions is not expected. In case of an overdose, beverages and foods containing carbohydrates should be excluded from the diet for 4-6 hours.

special instructions

Admission Glukobay must be combined with a diet prescribed by a doctor.

Patients with diabetes mellitus should be warned that they cannot interrupt treatment on their own, since in this case an increase in blood glucose levels is possible.

In case of non-compliance with diet therapy, side effects from the gastrointestinal tract may increase. If, even with strict adherence to the diet, the symptoms intensify, it is necessary to reduce the dose of the drug (temporarily or permanently).

When used as a monopreparation, Glucobay does not cause hypoglycemia, however, if it is added to insulin or oral hypoglycemic drugs, hypoglycemia may develop, in rare cases - up to coma. Presumably, this is due to a decrease in the need for antidiabetic therapy. In this regard, it may be necessary to adjust the prescribed doses.

Acarbose slows down the conversion of sucrose to glucose and fructose, so glucose should be used to relieve acute hypoglycemia.

While taking the drug, an asymptomatic increase in the activity of hepatic transaminases is possible. For this reason, it is recommended to monitor the enzyme levels during the first 6-12 months of treatment.

A note on the use of Glucobay should be made in the outpatient card of a patient with diabetes mellitus.

Acarbose does not affect the cognitive and psychomotor functions of a person, therefore it does not reduce the ability to engage in potentially hazardous activities.

Drug interactions

In some cases, acarbose can alter the bioavailability of digoxin, as a result of which a dose adjustment will be required.

Glucobay is not recommended to be taken simultaneously with preparations containing digestive enzymes, gastrointestinal adsorbents and cholestyramine, since they may affect its effectiveness.

Analogs

Analogs of the drug Glucobay are: Acarbose, Glinoza, Siofor.
