Release form and composition
Dosage form:
- Solution for infusion (in ampoules of 5 ml, in a carton pack of 10 ampoules; in a blister strip of 5 ampoules, in a carton pack of 1 or 2 packs);
- Concentrate for the preparation of a solution for infusion (in ampoules of 5 ml, in a cardboard box of 10 ampoules; in a blister strip of 10 ampoules, in a cardboard box of 1 or 2 packages);
- Tablets 250 mg (10 pcs. In a blister, in a cardboard box of 3 or 6 packs; 30 pcs. In a blister, in a cardboard box of 1 or 2 packages);

Pharmacological properties
Pharmacodynamics
Arginine glutamate - the active substance Glutargin is a compound of arginine and glutamic acid. These substances are necessary to ensure the biochemical processes of neutralization and subsequent removal from the body of ammonia, a highly toxic metabolite of the metabolism of nitrogenous substances. The hypoammonoemic effects of Glutargin are caused by the following factors:
- Neutralization of ammonia in the ornithine cycle of urea synthesis;
- Binding of ammonia to non-toxic glutamine;
- Strengthening the excretion of ammonia from the central nervous system and its removal from the body.
As a result, there is a decrease in the general toxic, including neurotoxic, effects of ammonia.
Glutargin has a hepatoprotective effect, having antioxidant, membrane stabilizing and antihypoxic activity, and has a positive effect on the processes of energy supply in hepatocytes.
When used against the background of alcohol intoxication, Glutargin has the following effects:
- Stimulates the utilization of alcohol in the monooxygenase system of the liver, prevents the inhibition of alcohol dehydrogenase - the key enzyme for the utilization of ethanol;
- Promotes inactivation and elimination of toxic products of ethanol metabolism as a result of increased formation and oxidation of succinic acid;
- Reduces the depressing effect of alcohol on the central nervous system, which is provided by the neurotransmitter properties of excitatory glutamic acid.
Due to the endothelium-protective effect, when used for pathologies of pregnancy, there is a decrease in impaired vascular permeability and thromboresistance, prevention of hypercoagulation, decreased sensitivity of vessels to vasoconstrictor agents (angiotensin, endothelium, thromboxane), which cause generalized vasospasm.
Arginine, after the end of biotransformation into nitric oxide, has a vasodilating effect, has a positive effect on the functional properties of circulating blood elements and blood coagulation. As a result of antihypoxic and vasodilating effects, there is an improvement in fetoplacental hemodynamics, a decrease in intrauterine fetal hypoxia.
Glutargin in the pathology of pregnancy also exhibits hepatoprotective and antitoxic activity, acts as a non-specific metabolic regulator of metabolic processes. Due to these properties, there is a decrease in the level of immune complexes circulating in the blood, a decrease in the severity of immunotoxicosis and metabolic intoxication syndrome, an increase in the compensatory-adaptive reactions of the body.
Glutargin does not have gonadotoxic, embryotoxic, teratogenic and mutagenic effects, and does not lead to the development of immunotoxic and allergic reactions.
Indications for use
- Poisoning with hepatotropic poisons - chemicals, the poison of the pale toadstool, drugs;
- Hepatitis of various origins in acute or chronic form;
- Liver damage with leptospirosis;
- Cirrhosis of the liver;
- Hepatic encephalopathy;
- A state of severe and moderate acute alcohol poisoning, incl. alcoholic encephalopathy and coma;
- Post-intoxication pathologies caused by alcohol intake;
- Precoma and coma, with hyperammonemia.
In addition, the use of tablets Glutargin 750 mg is indicated for the prevention of alcohol intoxication, hangover syndrome.
Contraindications
Absolute:
- Age under 18;
- I and II trimesters of pregnancy and the period of breastfeeding;
- Individual intolerance to the components of the drug.
Relative (Glutargin is prescribed under medical supervision):
- Increased excitability;
- Feverish condition;
- Severe violations of nitrogen-excreting (filtration) renal function (in patients with creatinine clearance less than 30 ml / min).
Glutargin, instructions for use: method and dosage
A solution and concentrate for the preparation of a solution for infusion Glutargin in ampoules is used by intravenous (iv) administration at a rate of 60-70 drops per minute. The infusion solution is prepared in the proportion: 2 g of the drug / 150-250 ml of isotonic 0.9% sodium chloride solution. The procedures are carried out 2 times a day, the maximum daily dose is 8 g.
Glutargin tablets 250 mg are prescribed for adult patients orally, 3 pcs. 3 times a day, regardless of food intake. The course of treatment is 15-20 days. In severe clinical situations, a single dose can be increased to 8 pcs. The maximum daily dose is 32 pcs. (8 g).
When using a 750 mg tablet, a single dosage and treatment period depend on the severity of the disease and can be from 1 to 3 pcs. 3 times a day for 14-21 days. Appointment in the treatment of alcohol poisoning - 1-1.5 pcs. 4 times a day every 1-2.5 hours on the first day and 1 pc. 2 times a day for the next 2-3 days. When treating a severe degree of alcohol poisoning, the tablets are taken after a course of intravenous administration of Glutargin in ampoules, 1 pc is prescribed. 2 times a day for 20 days. In order to prevent alcohol intoxication, it is recommended to take 2.5 tablets 1-1.5 hours before drinking alcohol. The drug is taken regardless of food intake.
Side effects
When using tablets, side effects can sometimes be observed in the form of a temporary feeling of nausea and mild discomfort in the intestine or stomach immediately after taking the drug.
Overdose
The main symptoms are atrioventricular block, chest pain.
Therapy: the introduction of intravenous corticosteroids.
