Diclofenac-Akrikhin is a non-steroidal anti-inflammatory drug (NSAID) for external use.

Release form and composition

• gel for external use 1%: has a characteristic aroma, color from pure white to white with a yellowish or grayish tinge (40 or 50 g in aluminum tubes, in a cardboard box 1 tube with instructions for use);
• ointment for external use 1%: has a characteristic aroma, color from pure white to white with a yellowish or grayish tinge, possibly the presence of mother-of-pearl stains (30 g each in aluminum tubes, in a cardboard box 1 tube with instructions for use).

Each pack also contains instructions for the use of Diclofenac-Akrikhin.

Composition for 100 g of gel for external use:

• active ingredient: diclofenac sodium - 1 g;
• auxiliary ingredients: rectified ethyl alcohol (ethanol) - 25 g, gelling agent carbomer 940 (carbopol 980) - 0.9 g, propylene glycol - 5 g, diethanolamine - 1 g, vaseline oil (liquid paraffin) - 2.5 g, macrogol cetostearate (macrogol 20 cetostearyl ether) - 2 g, cocoyl caprilocaprate - 2.5 g, lavender oil - 0.05 g, neroli essential oil (from bitter orange flowers) - 0.05 g, purified water - up to 100 g.

Pharmacological properties

Pharmacodynamics

As an active component, Diclofenac-Akrikhin gel and ointment contains diclofenac, an NSAID, which has a pronounced analgesic and anti-inflammatory effect through non-selective inhibition of cyclooxygenase 1 and 2 (COX-1 and -2), which disrupts the metabolism of arachidonic acid, as well as the synthesis of prostaglandins, which are mediators of the inflammatory process.

Diclofenac-Akrikhin, when applied externally, eliminates pain syndrome and reduces swelling associated with inflammation, causes weakening or relieves joint pain when the patient is at rest and when moving. Reduces morning stiffness, joint swelling, improves the quality of life by increasing the range of motion.

In the postoperative and post-traumatic periods, it quickly relieves pain and suppresses inflammation in the area of application; in the treatment of post-traumatic hematoma, it promotes its resorption.

Due to the water-ethanol base, Diclofenac-Akrikhin gel has an additional local anesthetic effect.

Pharmacokinetics

• absorption: after topical application, diclofenac exhibits good transdermal permeability. The amount of substance absorbed through the skin is proportional to the area of application of the gel / ointment, the time of their contact with the skin, and also depends on both the total dose of the drug and the degree of skin hydration. With the dosage regimen recommended in the instructions, no more than 6% of diclofenac is absorbed into the systemic circulation. If applied for 10 hours under an occlusive dressing, the resorption of the substance increases 3 times;
• distribution: diclofenac is concentrated mainly in the foci of inflammation and in the synovial fluid. Up to 99% of the active ingredient binds to plasma proteins (mainly albumin);
• metabolism: occurs mainly in the liver through hydroxylation; as a result of the biotransformation of diclofenac, several of its metabolites are formed (including two pharmacologically active, but to a lesser extent than diclofenac);
• excretion: about 65% of diclofenac is excreted in the urine, about 35% in the bile (in the form of inactive compounds with glucuronic and sulfuric acids), less than 1% is excreted unchanged. The total (total) plasma clearance of diclofenac averages 263 ml / min, the final T _1/2 (half-life) is 1–2 hours.

Indications for use

• rheumatoid arthritis, ankylosing spondylitis, juvenile psoriatic arthritis (JAPA), other diseases of the musculoskeletal system of an inflammatory nature;
• bursitis, tendovaginitis, lesions of periarticular tissues, other rheumatic diseases of soft tissues;
• deforming osteoarthritis, osteochondrosis, other diseases of the musculoskeletal system associated with degenerative processes in the joints;
• post-traumatic inflammation of soft tissues and joints, as a result of sprains, bruises, overexertion;
• swelling of tissues and pain syndrome resulting from such diseases of muscles and joints as osteoarthritis, rheumatoid arthritis, radiculitis, sciatica, lumbago, muscle pain of rheumatic / non-rheumatic genesis.

Contraindications

Absolute:

• exacerbation of erosive and ulcerative lesions of the gastrointestinal tract (gastrointestinal tract);
• third trimester of pregnancy;
• period of breastfeeding (lactation);
• children under 6 years of age;
• damage to the integrity of the skin;
• history of attacks of bronchial obstruction, urticaria, rhinitis after taking acetylsalicylic acid or other NSAIDs;
• increased individual sensitivity to diclofenac or auxiliary components of Diclofenac-Akrikhin ointment or gel, as well as to acetylsalicylic acid or other NSAIDs.

The drug is used with caution in hepatic porphyria in the exacerbation phase, erosive and ulcerative diseases of the gastrointestinal tract, severe violations of hepatic / renal function, bronchial asthma, CHF (chronic heart failure), in the first / second trimesters of pregnancy and in old age.

Diclofenac-Akrikhin gel and ointment is applied externally by applying a thin layer over the focus of inflammation, followed by rubbing the product into the skin with light movements.

Recommended dosing regimen of Diclofenac-Akrikhin:

• children from 6 to 12 years old: up to 2 g of gel / ointment for 1 procedure, but not more than 2 times a day;
• children over 12 years old and adult patients: 2-4 g of gel / ointment (the size of a large cherry or small walnut) 3-4 times a day.

The amount of the drug is determined by the size of the painful area.

After the procedure (application of the drug), you need to thoroughly wash your hands if they have not been processed for the purpose of treatment.

Side effects

• reactions at the site of application: photosensitivity, eczema, contact dermatitis (redness, itching, swelling of the treated skin area, vesicles, papules, peeling);
• systemic reactions: generalized skin rash, hypersensitivity reactions (urticaria, bronchospasm, angioedema).

Overdose

Since Diclofenac-Akrikhin has a low systemic absorption, its external use makes an overdose of Diclofenac almost impossible.

The drug is applied only to intact skin; contact of the gel / ointment on open wounds should be avoided.

It is not recommended to use Diclofenac-Akrikhin under an occlusive dressing.

Avoid contact with eyes and mucous membranes.

Application during pregnancy and lactation
