Clenil is a topical glucocorticosteroid (GCS).

Release form and composition

Dosage forms: metered dose aerosol for inhalation and solution for inhalation (200 doses each in aluminum cans with a dispensing device, in a cardboard box 1 can, complete with an inhaler).

Active ingredient: beclomethasone dipropionate, in 1 dose - 50 or 250 mcg.

Additional components: norflurane (1,1,1,2-tetrafluoroethane), glycerol, ethanol.

Indications for use

Mild to moderate bronchial asthma;
Moderate and severe bronchial asthma, including hormone-dependent and refractory to treatment with bronchodilators and / or sodium cromoglycate.

Contraindications

Absolute:

Tuberculosis;
Children under 4 years of age - for the drug 50 μg / dose, up to 6 years - for the drug 250 μg / dose;
Lactation (or feeding should be stopped);
Individual hypersensitivity to Klenil's components.

Relative (patients with the following conditions / diseases should be under close medical supervision during treatment, since there is a risk of complications):

Cirrhosis of the liver;
Epilepsy;
Glaucoma;
Cerebral pathology;
Hypothyroidism;
Osteoporosis;
Systemic infections (fungal, viral, bacterial, parasitic);
Adrenal insufficiency.

With caution and only if the expected benefit outweighs the possible risks, the drug should be used during pregnancy, especially in the II and III trimesters.

Method of administration and dosage

Clenil is used by inhalation.

The initial dose of the drug is selected by the doctor depending on the severity of the disease. If necessary, the dose can be reduced to the minimum effective, or increased until the clinical effect is achieved.

Recommended doses for adult patients (including the elderly) and children over 12 years old:

Mild bronchial asthma severity (forced expiratory volume in 1 second (FEV ₁) or peak expiratory flow rate (PSV) - more than 80%, and the daily range of PSV values does not exceed 20%): 200-600 μg per day;
Average severity of bronchial asthma (FEV ₁ or PSV - 60-80%, daily range of PSV indicators - 20-30%): 600-1000 mcg per day;
Severe bronchial asthma (FEV ₁ or PSV - 60%, daily variation in PSV values - 30%): 1000-2000 mcg per day (for severe disease, Clenil is prescribed, containing 250 mcg in 1 dose).

The maximum daily dose for this category of patients should not exceed: with an average severity of the disease - 1000 mcg, with a severe degree - 2000 mcg.

The daily dose is divided into 3-4 inhalations.

The initial dose for children aged 4 to 12 years is 50-100 mcg 2 times a day; in severe cases, the daily dose can be increased to 400 mcg in several doses.

The maximum allowable daily dose for children 4-12 years old is 500 mcg. In this age group of patients, Clenil is mainly used with a content of 50 mcg in 1 dose. The drug 250 mcg / dose can only be prescribed if the required daily dose exceeds 500 mcg.

Method of using aerosol for inhalation:

Before the first use, as well as every time in the case of a 3-day break in the use of the inhaler, it is necessary to remove the protective cap from the mouthpiece and make a single spray into the air;
Take the inhaler with your index and thumb;
Remove the protective cap;
Put the mouthpiece in your mouth and, tightly wrapping your lips around it, exhale completely through your nose;
Take a long deep breath, while simultaneously pressing on the end of the can with your index finger;
Hold your breath as long as possible;
Rinse mouth with water.

It is recommended to rinse the mouthpiece with warm water as it gets dirty.

Side effects

Local reactions: sore throat, hoarseness, cough; when using the drug in high doses and / or for a long time - candidiasis of the oral cavity and upper respiratory tract (in this case, local antifungal therapy is prescribed);
Systemic reactions: in isolated cases - allergic reactions; with long-term treatment in high doses (more than 1500 mcg) - a decrease in the function of the adrenal cortex, an increase in intraocular pressure, cataracts;
Others: in isolated cases - eosinophilic pneumonia, paradoxical bronchospasm (stopped by the use of inhaled bronchodilators).

special instructions

Clenil is not intended for the relief of acute asthmatic attacks. Also, the drug should not be used in severe attacks of bronchial asthma requiring intensive therapy.

Infectious and inflammatory diseases of the nasal cavity and paranasal sinuses, with the use of appropriate therapy, are not a contraindication to the appointment of Klenil.

If there is a risk of developing paradoxical bronchospasm, a bronchodilator (for example, salbutamol) is inhaled 10-15 minutes before using beclomethasone.

In the case of the development of candidiasis of the oral cavity and upper respiratory tract, local antifungal therapy is prescribed, while there is no need to cancel Clenil.

The drug with a content of 250 μg / dose is not prescribed for children under the age of 12, except in cases where the advisability of using high doses is justified by the severity of the patient's condition.

Newborn babies whose mothers received beclomethasone during pregnancy should be carefully examined for adrenal insufficiency.

Drug interactions

Cases of clinically significant interactions of beclomethasone with other drugs have not been established.

Keep in mind:

Beta2-adrenostimulants, estrogens, methandrostenolone, theophylline and oral glucocorticosteroids enhance the effect of beclomethasone;
Microsomal oxidation inducers (including phenobarbital, rifampicin, phenytoin) reduce the effectiveness of beclomethasone;
Beclomethasone enhances the action of beta2-adrenostimulants;
Previous inhalation use of beta2-adrenostimulants increases the clinical efficacy of Clenil.

Since the drug contains a small amount of ethanol, in patients with hypersensitivity, there is a theoretical possibility of Clenil's interaction with metronidazole and disulfiram.

Terms and conditions of storage
