Release form and composition
The dosage form of Khimopsin is a lyophilisate for preparing a solution for local and external use: white with a slight yellowish tinge or white lyophilized powder (or porous mass) in the form of lumps or tablets (in bottles of 0.05 g, 1 bottle in a cardboard box).
The active component of the drug is a mixture of trypsin and chymotrypsin, contained in 1 vial with lyophilisate in an amount of 0.05 g.
Pharmacological properties
Pharmacodynamics
Chymopsin is an enzyme with proteolytic action. Due to the presence of specific antienzymes in necrotic tissue, it breaks down it without affecting adipose tissue.
Also, the drug liquefies viscous exudates and secretions.
Indications for use
- suppurative bronchopulmonary pathologies: exudative pleurisy, atelectasis, lung abscess, bronchiectasis (for sanitation of the airways and liquefaction of thick viscous sputum);
- chronic rhinitis, eustachitis with viscous exudate, purulent sinusitis or acute / subacute / chronic otitis media, as well as after tracheostomy to facilitate the removal of thick viscous exudate (pathology of ENT organs);
- purulent wounds, trophic ulcers, bedsores, burns (to accelerate healing, cleanse from purulent-necrotic masses);
- sluggish healing of eyelid skin wounds, obstruction of the lacrimal ducts, burns of the mucous membrane of the eye, corneal pathology (herpetic keratitis, ulcer).
Contraindications
Absolute:
- hemorrhagic diathesis;
- pancreatitis;
- viral hepatitis;
- dystrophy and cirrhosis of the liver;
- pulmonary tuberculosis (open form);
- pulmonary emphysema with respiratory failure;
- malignant neoplasms;
- chronic heart failure II – III degree;
- ulcerated surfaces of malignant tumors, bleeding cavities;
- individual intolerance to the components contained in the preparation.
A relative contraindication (diseases / conditions in the presence of which the appointment of Himopsin requires caution) is the empyema of the pleura of tuberculous origin, since the resorption of exudate can serve the development of bronchopleural fistula.
Instructions for the use of Himopsin: method and dosage
The solution prepared from the lyophilisate is applied externally, used for inhalation, irrigation or instillation into the nasal cavity.
Dosing regimen:
- pathology of the respiratory system: 0.025-0.03 g of lyophilisate is dissolved in 5 ml of distilled water, then through a tracheostomy tube, endotracheal probe, bronchoscope or inhalation apparatus is introduced into the respiratory tract 1-3 times a day for several days (depending on indications) … It is allowed to add antibacterial or bronchodilator agents to the solution that enhance the effect of the drug;
- chronic rhinitis: 0.005 g of lyophilisate is diluted in 5 ml of 0.9% sodium chloride solution and instilled or irrigated the nasal cavity 2-3 times a day. In cases of chronic purulent otitis media complicated by cholesteatoma, after washing with 0.9% sodium chloride solution, 0.5% Himopsin solution (with 0.9% sodium chloride solution) is instilled into the ear 2-3 times a day;
- thermal burns of the III degree: a thin layer of lyophilisate (1 g per 1 m of the wound surface) is applied to the scab, the top is covered with a bandage soaked in 0.25% procaine solution or 0.9% sodium chloride solution. To slow down the drying of the dressing, apply a moisture-proof dressing over it. Both dressings are changed once a day;
- purulent wounds and bedsores: 0.025–0.05 g of the drug is dissolved in 10–50 ml of 0.25% procaine solution. In the resulting solution, sterile gauze tampons are moistened, then they are applied to the wound surface for 2-24 hours (depending on the thickness of the purulent-necrotic masses). Dressings are changed every 3-5 days;
- ulcers, burns of the cornea and keratitis: as an eye bath for Himopsin, a solution of 1: 500 is used daily for 2-3 days, or 2 drops of a 0.25% solution prepared ex tempore are used 4 times a day for 1-2 days;
- obstruction of the lacrimal passages, wounds of the skin of the eyelids: 1% solution of lyophilisate, prepared ex tempore, is used to wash the lacrimal passages and irrigate the wound.
Side effects
During the period of therapy with Himopsin, allergic reactions may develop. In rare cases, after inhalation of the drug, a rapidly passing low-grade fever, irritation of the mucous membrane of the respiratory tract, and hoarseness are observed.
When using the drug in ophthalmology, swelling and irritation of the conjunctiva is sometimes noted (requires a decrease in the concentration of the drug).
Overdose
There are no data on an overdose of Himopsin.
special instructions
Before using the drug in patients with a tendency to develop allergic reactions, a single therapeutic dose of antihistamines should be administered (intramuscularly or subcutaneously).
Patients should be warned about the need to carefully cough up sputum or remove it by suction for the first few hours after inhalation of Himopsin.
For violations of liver function
According to the instructions, Himopsin is contraindicated in dystrophy, liver cirrhosis and viral hepatitis.
Drug interactions
There are no data on the interaction of Himopsin with other drugs.
