Release form and composition

Dosage form Tsefekon D - rectal suppositories for children: torpedo-shaped, white or white with a creamy or yellowish tinge (5 pcs. In blisters, in a cardboard box 2 packs).

Composition of 1 suppository:

• active substance: paracetamol - 50, 100 or 250 mg;
• additional component: solid fat (supposir, witepsol) - until a suppository weighing 1250 mg is obtained.

Pharmacological properties

Pharmacodynamics

Cefekon D has an analgesic and antipyretic effect.

Paracetamol, acting on the centers of pain and thermoregulation, blocks cyclooxygenase in the central nervous system. Cellular peroxidases in the inflammation focus neutralize the effect of paracetamol on cyclooxygenase. This explains the lack of a pronounced anti-inflammatory effect.

Cefekon D has no negative effect on water-electrolyte metabolism and the mucous membrane of the gastrointestinal tract.

Pharmacokinetics

Paracetamol is rapidly and highly absorbed from the gastrointestinal tract. C _max (maximum concentration of the substance) is reached after 30-60 minutes. Plasma protein binding is 15%. Penetrates the blood-brain barrier.

Metabolized in the liver. 80% of the substance reacts with glucuronic acid and sulfates, resulting in the formation of inactive metabolites. 17% of paracetamol undergoes hydroxylation with the formation of active metabolites, which are subsequently conjugated with glutathione and form inactive metabolites. These metabolites, in case of a lack of glutathione, can block the enzyme systems of hepatocytes and lead to the development of necrosis. T _1/2 (half-life) - from 2 to 3 hours. 85–95% of paracetamol is excreted by the kidneys within 24 hours in the form of sulfates and glucuronides, unchanged - about 3%.

V _d (volume of distribution) and bioavailability in children (including neonates) are similar to those in adult patients. The main metabolite of paracetamol in newborns during the first 2 days of life and in children 3–10 years old is paracetamol sulfate, in children over 12 years old - conjugated glucuronide. There is no significant age-related difference in the rate of elimination of paracetamol and in the total amount of substance excreted in the urine.

Indications for use

• acute respiratory viral infection, influenza, childhood infections, post-vaccination reactions and other conditions accompanied by an increase in body temperature - as an antipyretic agent;
• pain syndrome of mild / moderate intensity, including headache, toothache, muscle pain, neuralgia, pain in burns and injuries - as a pain reliever.

Contraindications

Absolute:

• age up to 1 month;
• individual intolerance to the components of the drug.

Relative (Tsefekon D is prescribed under medical supervision):

• deficiency of the enzyme glucose-6-phosphate dehydrogenase;
• functional disorders of the kidneys and liver;
• diseases of the blood system (thrombocytopenia, anemia, leukopenia);
• syndromes of Gilbert, Dubin - Johnson, Rotor.

Instructions for the use of Cefekon D: method and dosage

Cefekon D is used rectally. Suppositories should be injected into the rectum after a cleansing enema or spontaneous bowel movement.

The scheme of application is set depending on age and weight. The average single dose is 10-15 mg / kg, the frequency of use is 2-3 times a day (observing the intervals of 4-6 hours). The maximum dose is 60 mg / kg per day.

Recommended single doses:

• 1-3 months (4-6 kg): Tsefekon D 50 mg (the drug can be administered once after vaccination to lower the temperature, the possibility of its use in other cases is determined by the doctor);
• 3–12 months (7–10 kg): Cefecon D 100 mg;
• 1-3 years (11-16 kg): 100-200 mg;
• 3-10 years (17-30 kg): Tsefekon D 250 mg;
• 10-12 years (31-35 kg): 500 mg.

As an antipyretic agent, Tsefekon D can be used for 3 days, as an anesthetic - 5 days. The duration of use should not be increased without the agreement of a doctor.

Side effects

• digestive system: vomiting, nausea, abdominal pain;
• hematopoietic system: rarely - leukopenia, thrombocytopenia, anemia, agranulocytosis;

• hepatotoxic drugs, ethanol, as well as phenytoin, barbiturates, flumecinol, phenylbutazone, rifampicin, tricyclic antidepressants and other inducers of microsomal oxidation in the liver: an increase in the production of hydroxylated active metabolites, which, even with a small overdose, can cause the development of severe intoxication;
• chloramphenicol: increasing its toxic effects;
• salicylates: increased risk of nephrotoxic effects;
• anticoagulants of indirect action: enhancing their action;
• drugs with uricosuric action: decrease in their effectiveness.
