Release form and composition
Cerecard is available in the form of a solution for intramuscular and intravenous administration: a clear light yellow or colorless liquid (2 or 5 ml in glass ampoules, in blisters of 5 ampoules, in a cardboard box of 1 or 2 blisters).
1 ml of solution contains:
- active substance: ethylmethylhydroxypyridine succinate - 50 mg;
- excipient: water for injection.
Pharmacological properties
Pharmacodynamics
Cerecardia belongs to the 3-hydroxypyridine group. The drug inhibits free radical processes and is a membrane protector with antihypoxic, stress-protective, antiepileptic, anxiolytic and nootropic action.
The active substance of Cerecard is ethylmethylhydroxypyridine succinate, a pyridine derivative from the 3-hydroxypyridine group. An inhibitor of biochemical reactions involving free radicals, which protects cell membranes from various kinds of damage, and also has anxiolytic, nootropic, antihypoxic, stress-protective and antiepileptic effects.
The mechanism of action of the drug is due to its membrane-protective and antioxidant properties, due to which: lipid peroxidation (LPO) is inhibited; the activity of superoxide oxidase increases; the lipid-protein ratio increases; improves the function of the cell membrane and its structure. Cerecard modulates the activity of vector (membrane-bound) enzymes, such as calcium independent phosphodiesterase, adenylate cyclase, acetylcholinesterase, as well as receptor complexes - γ-aminobutyric acid, benzodiazepine and acetylcholine. Such efficiency contributes to their binding to ligands, preservation of the structural and functional features of biological membranes and the transmission of neurotransmitters, and improvement of neurotransmission. Cerecardia increases the concentration of dopamine in the brain.
With systemic use of ethylmethylhydroxypyridine succinate:
- reduces the degree of inhibition of oxidative processes in the Krebs cycle and increases the compensatory activation of aerobic glycolysis under hypoxic conditions with an increase in the level of adenosine triphosphoric acid (ATP) and creatine phosphate, activates the energy-synthesizing function of mitochondria;
- increases the body's resistance to the action of various pathogenic and damaging factors (biological, chemical and physical) in pathological processes;
- improves metabolic processes and blood supply in the brain, enhances microcirculation and rheological properties of blood, reduces platelet aggregation; stabilizes the cell membranes of platelets and erythrocytes, reducing the risk of hemolysis; has a hypolipidemic effect, reducing the content of low density lipoproteins and total cholesterol;
- helps in myocardial infarction to improve the functional state of the heart muscle in the areas of ischemia and contractile function of the heart, as well as to reduce the risk of impaired systolic and diastolic function of the left ventricle;
- contributes to the preservation of the structural and functional organization of biological membranes of cardiomyocytes, in conditions of a critical weakening of coronary blood flow, stimulates the activity of membrane enzymes - adenylate cyclase, phosphodiesterase, acetylcholinesterase. Supports the activation of aerobic glycolysis that develops in conditions of acute ischemia and promotes the restoration of mitochondrial redox processes under conditions of hypoxia; increases the synthesis of ATP and creatine phosphate. Maintains the integrity of the physiological functions and morphological structures of the myocardium in the areas of ischemia;
- improves the picture of the clinical course of myocardial infarction and the effectiveness of the treatment, accelerates the regeneration of the functional activity of the left ventricle, reduces the incidence of arrhythmias and intracardiac conduction disorders. In the ischemic myocardium, it helps to normalize metabolic processes, restore / improve electrical activity and myocardial contractility, reduce the zone of necrosis, and increase coronary blood flow in the ischemic zone. Strengthens the antianginal activity of nitro drugs; in case of acute coronary insufficiency, it reduces the consequences of reperfusion syndrome; improves the rheological properties of blood.
Pharmacokinetics
With intramuscular injection of ethylmethylhydroxypyridine succinate, the time to reach its maximum concentration (Cmax) is from 0.3 to 0.58 hours. The use of the drug in a dose of 400-500 mg intramuscularly provides a Cmax value in the range of 2.5-4 μg / ml.
The rate of distribution of the substance to organs and tissues is high, the average time of its retention in the body after intramuscular administration is from 0.7 to 1.3 hours.
Metabolism of Cerecardia occurs in the liver by glucuronidation with the formation of five main metabolites: the first is 3-hydroxypyridine phosphate, which decomposes with the participation of alkaline phosphatase into 3-hydroxypyridine and phosphoric acid; the second is pharmacologically active, it is found in the urine 1-2 days after administration; the third - is excreted in the urine in large quantities; the fourth and fifth are conjugated with glucuronic acid.
Cerecardia is rapidly excreted from the body in the urine, mainly in the form of metabolites (up to 50% in 12 hours) and slightly unchanged (up to 0.3% in 12 hours). The maximum rate of elimination is observed in the first 4 hours after administration. The rates of excretion of unchanged drug and its metabolites in the urine are characterized by significant individual variability.
Indications for use
Cerecard is recommended for use in the complex treatment of the following conditions / diseases:
- vegetative vascular dystonia;
- anxiety in neurosis-like and neurotic disorders;
- encephalopathy;
- acute disorders of cerebral circulation (ischemic stroke);
- mild cognitive impairment of atherosclerotic genesis;
- acute intoxication while taking antipsychotic drugs;
- withdrawal alcohol syndrome with a predominance of vegetative-vascular and neurosis-like pathologies;
- acute myocardial infarction.
Contraindications
- pregnancy;
- lactation period;
- acute renal and / or hepatic failure;
- age up to 18 years;
- hypersensitivity to the active substance of the drug.
Cerecard is prescribed with caution if there is a history of allergic diseases.
Instructions for the use of Cerecard: method and dosage
Cerecard is applied intramuscularly or intravenously (60 drops / min), or jet (for 5-7 minutes). With the infusion method, a solution of sodium chloride 0.9% (200 ml) is used for preliminary dilution of the drug.
The initial dose of ethylmethylhydroxypyridine succinate is 50-100 mg 1 to 3 times a day. The maximum daily dose is 800 mg.
The recommended dosage regimen of Cerecard:
- dyscirculatory encephalopathy in the decompensation phase: intravenous stream or drip, 100 mg 2-3 times a day for 14 days, and then intramuscularly 100 mg for 14 days;
- acute cerebrovascular accident: intravenous drip of 200-300 mg 1 time per day for the first 2-4 days, and then intramuscularly 100 mg 3 times a day;
- course prevention of discirculatory encephalopathy: intramuscularly, 100 mg 2 times a day for 10-14 days;
- vegetative-vascular dystonia, neurosis-like and neurotic states: intramuscularly, 50-400 mg 1 time per day for 14 days;
- acute intoxication with antipsychotic drugs: intravenous injection of 50-300 mg 1 time per day for 7-14 days;
- alcohol withdrawal syndrome: intramuscular injection of 100-200 mg 2-3 times a day or intravenous drip of 100-200 mg 1-2 times a day for 5-7 days;
- mild cognitive impairment: intramuscularly, 100–300 mg once a day for 14–30 days;
- acute myocardial infarction: in the first 5 days by intravenous infusion for 0.5-1.5 hours (in 100-150 ml of 5% dextrose solution or 0.9% sodium chloride solution) or intravenous jet for 5 minutes. In the next 9 days, the drug is administered intramuscularly 3 times a day (every 8 hours). Recommended doses: single - from 2 to 3 mg / kg, daily - from 6 to 9 mg / kg. Maximum doses: single - 250 mg, daily - 800 mg.
Side effects
When taking Cerecard, the appearance of a feeling of lack of air caused by a too high rate of introduction (most often of a short-term nature), dryness and a metallic taste in the mouth, sensations of spreading heat over the body, sore throat, unpleasant odor, discomfort in the chest, allergic reactions. Especially often these phenomena are observed in the case of administration of the drug intravenously.
With prolonged use, there is a possibility of developing nausea, flatulence, sleep disturbances (drowsiness and sleep disorder).
Overdose
In case of an overdose, insomnia or drowsiness (sometimes) may occur. With intravenous administration, a slight and short-term (up to 2 hours) increase in blood pressure is possible.
Overdose symptoms usually disappear within 24 hours and do not require special treatment. With a pronounced increase in blood pressure, antihypertensive drugs should be used and / or therapy should be supplemented with nitro drugs.
For insomnia, it is sometimes recommended to prescribe hypnotics from the group of benzodiazepine derivatives (oxazepam 10 mg, diazepam 5 mg, nitrazepam 10 mg).

During the period of treatment with the drug, care should be taken when driving moving mechanisms and vehicles.
Application during pregnancy and lactation
Cerecard is contraindicated for use during pregnancy and lactation.
Pediatric use
The drug is prohibited for use in children under 18 years of age.
With impaired renal function
Cerecard is contraindicated for use in acute renal failure.
For violations of liver function
According to the instructions, Cerecard is contraindicated for use in acute liver failure.
Drug interactions
Cerecard enhances the effect of antiparkinsonian (levodopa) and antiepileptic (carbamazepine) drugs, as well as anxiolytics from the benzodiazepine and nitrate group.
The drug reduces the toxic effects of ethanol.
