Celanide is a drug belonging to the group of cardiotonic drugs, cardiac glycoside. Dispensed exclusively by prescription. Celanide contains the active ingredient - lanatoside C. The drug is produced in the form of a solution for injection, tablets, solution for oral administration.

The mechanism of action of Celanide is to block Na + / K + -ATPase. As a result, the concentration of Na ions in cardiomyocytes increases, the release of Ca ions from the sarcoplasmic reticulum is accelerated, as a result, the troponin complex is inhibited, which has a depressing effect on the interaction of myosin and actin. The drug increases the strength and rate of myocardial contraction in a different way from the Frank-Starling mechanism (does not depend on the level of myocardial pre-stretching). The systole period becomes shorter and more economical. Celanide increases myocardial contractility, VOC and IOC. At the same time, CSR and EDV are reduced, which, against the background of an increase in myocardial tone, leads to a significant decrease in its size and a decrease in oxygen demand. The drug has a negative dromotropic effect,which is manifested in an increase in the refractoriness of the AV node. As a result of direct and indirect influence on the processes of regulation of the heart rate, the drug causes a decrease in heart rate. Celanide has a direct vasoconstrictor effect. In patients suffering from chronic heart failure, an indirect vasodilating effect of the drug, a decrease in venous pressure, an increase in urine output, a decrease in edema, and shortness of breath, is also manifested. The positive batmotropic effect is manifested when using subtoxic and toxic dosages.decrease in venous pressure, increase in urine output, decrease in edema, shortness of breath. The positive batmotropic effect is manifested when using subtoxic and toxic dosages.decrease in venous pressure, increase in urine output, decrease in edema, shortness of breath. The positive batmotropic effect is manifested when using subtoxic and toxic dosages.

When administered intravenously, the effect of Celanide begins after 10 minutes, the maximum is reached after 2 hours.

Indications for the use of Celanide

According to the instructions for Celanide, this drug is indicated for:

• atrial fibrillation;

• atrial flutter (to reduce heart rate or transfer flutter to fibrillation with controlled conduction of impulses through the atrioventricular node);

• paroxysmal supraventricular tachycardia;

• chronic heart failure;

• acute left ventricular failure;

• chronic cor pulmonale.

Contraindications to the use of Celanide

In the instructions for Celanide, the following contraindications for use are noted:

• hypersensitivity;

• intoxication with cardiac glycosides.

Celanide should be used with caution when:

• bradycardia;

• AV blockade;

• SSSU without pacemaker;

• HCMP;

• paroxysmal ventricular tachycardia;

• isolated mitral stenosis;

• acute myocardial infarction;

• WPW syndrome;

• unstable angina pectoris;

• chronic heart failure with impaired diastolic function (amyloidosis, restrictive CMP, cardiac tamponade, constrictive pericarditis);

• extrasystoles;

• cardiac asthma with mitral stenosis (in the absence of tachysystolic atrial fibrillation);

• severe dilatation of the heart cavities;

• electrolyte disturbances: hypokalemia, hypomagnesemia, hypernatremia, hypercalcemia;

• hypothyroidism;

• alkalosis;

• arteriovenous shunt;

• old age.

Side effects

When using Celanide, the following side effects have been reported:

• from the gastrointestinal tract: vomiting, nausea, loss of appetite, diarrhea;

• from the CCC side: AV blockade, arrhythmias;

• on the part of the NS: delirious psychosis, drowsiness, sleep disturbance, dizziness, headache, confusion;

• from the hematopoietic system: thrombocytopenic purpura, thrombocytopenia;

• allergic reactions: urticaria, angioedema;

• others: nosebleeds, decreased visual acuity, gynecomastia, petechiae.

Overdose

Celanide tablets
Celanide tablets

With an overdose of Celanide, symptoms of digitalis intoxication occur: paroxysmal ventricular tachycardia, nodal extrasystole, ventricular extrasystole, atrial fibrillation, atrial flutter, CA blockade, AV blockade, vomiting, abdominal pain, diarrhea, changes in visual perception, neuritis.

Overdose treatment: withdrawal of this drug, administration of specific antidotes (EDTA, unitiol), antiarrhythmic drugs, symptomatic treatment. With complete AV block with episodes of Morgagni-Adams-Stokes, temporary pacing is used.

Method of application of Celanide and dosage

Celanide is intended for parenteral and oral administration. The dosage in each clinical case is determined by the doctor. If it is necessary to achieve a quick effect, Celanide is administered intravenously at a dosage of 0.2-0.4 mg, the frequency of administration is 1-2 times a day. The initial dosage for oral administration is 0.25-0.5 mg or 10-25 drops 3-4 times a day. The maintenance dosage is 0.4-0.2 mg (for intravenous administration), 0.5-0.25 mg or 40-10 drops (for oral administration). Maximum dosages: daily - 1 mg, single - 0.5 mg for oral administration; daily - 1 mg, single - 0.8 mg for intravenous use.

Before using the drug, it is necessary to study the instructions for Celanide, as well as the instructions of the attending physician noted in the prescription for Celanide.

special instructions

With GOKMP, Celanide causes an increase in obstruction. With mitral stenosis, Celanide is prescribed in case of accession of right ventricular failure or mitral tachyarrhythmia. The use of Celanide for AV block can aggravate its severity and lead to the occurrence of Morgagni-Adams-Stokes attacks. In WPW syndrome, Celanide promotes the conduction of cardiac impulses through accessory pathways, thereby provoking the development of paroxysmal tachycardia. To monitor the level of digitalization, the determination of the plasma concentration of the drug can be used.

Interaction of Celanide with other drugs

Antacids, cholestyramine, sulfasalazine reduce the absorption of Celanide. Verapamil and beta-blockers aggravate the decrease in atrioventricular conduction. Methyldopa, quinidine, spironolactom, verapamil, amiodarone increase the concentration of Celanide in the blood as a result of a competitive decrease in secretion in the proximal renal tubules. Diuretics, glucocorticoids increase the likelihood of electrolyte disturbances. Calcium preparations, thiazide diuretics increase the risk of hypercalcemia. Calcium preparations, catecholamines, diuretics, when taken simultaneously with Celanide, increase the risk of glycosidic intoxication.

Storage conditions

Celanide should be stored at room temperature.

This drug belongs to list A. Celanide is dispensed by prescription.
