Primafungin is a local antifungal drug for use in gynecology.
Release form and composition
Dosage form - vaginal suppositories: cylindrical-conical, from light yellow to yellow or yellow with a cream shade; on the cut, the presence of a funnel-shaped depression or an air rod is permissible (3 pcs. in a blister strip; in a cardboard box 1 or 2 packages and instructions for the use of Primafungin).
Composition of 1 suppository:
- active substance: natamycin - 100 mg;
- auxiliary components: cetyl alcohol - 690 mg; sorbitan trioleate (E496) - 460 mg; polysorbate 80 - 460 mg; sodium bicarbonate - 69 mg; adipic acid - 64 mg; semi-synthetic glycerides (Supposir AM) - in an amount sufficient to obtain a suppository weighing 2550 mg.
Pharmacological properties
Pharmacodynamics
Natamycin has a fungicidal effect. It belongs to the group of antifungal polyene antibiotics from the macrolide group with a wide spectrum of action. Natamycin binds sterols of cell membranes, which leads to a violation of their integrity and function and, as a result, to the death of microorganisms. It is active against most pathogenic yeast-like fungi, especially the genus Candida of albicans and non-albicans species. Dermatophytes show less sensitivity to natamycin.
In clinical practice, natamycin resistance is not found.
Pharmacokinetics
Natamycin does not have a systemic effect, since it is practically not absorbed from the surface of the mucous membranes.
Indications for use
Primafungin is recommended for vaginitis, vulvitis, vulvovaginitis, caused mainly by fungi of the genus Candida.
Contraindications
The use of the drug is contraindicated in patients with individual intolerance to any of its components.

Primafungin is used intravaginally.
In the supine position, the suppository is inserted as deep as possible into the vagina.
The recommended scheme of use for girls who have begun to have sex life and adult women: 1 pc. a day before falling asleep at night, a course of 3–6 days.
Suppositories should not be cut into pieces, since such a change in their storage conditions may lead to a violation of the distribution of the active substance.
Side effects
Possible local reactions: burning sensation, unexpressed irritation of the mucous membrane.
An increase in any of the indicated side effects, or the appearance of other reactions not specified in the instructions, must be immediately reported to the attending physician.

There are no reports of overdose.

Due to the presence in the composition of vaginal suppositories of the unique base of Suppositor AM, they melt under the influence of body temperature with the formation of a volumetric spreading foamy mass, which contributes to the uniform distribution of the active substance over the vaginal mucosa.
Therapy should be interrupted during menstruation.
There is no need to exclude sexual intercourse at the time of drug use, however, it is recommended to examine the sexual partners and prescribe a treatment course in cases of candidiasis.
Also, during the period of therapy, it is necessary to provide for the use of barrier contraceptives.
Cetyl alcohol, which is part of Primafungin, in the presence of hypersensitivity to it, can cause a slight burning sensation in the genital area.
Pediatric use
Primafungin is not prescribed to girls before sexual activity.
Application during pregnancy and lactation
It is allowed to use Primafungin at all stages of pregnancy and during breastfeeding.
