Pharmacological properties
Pharmacodynamics
Pencrofton is a synthetic steroidal antigestagenic drug. The mechanism of action of the active substance, mifepristone, is due to the blocking of progesterone activity at the receptor level, which causes desquamation of the decidua. As a result, there is an expulsion of the ovum.
The drug increases the contractility and sensitivity of the myometrium to prostaglandins. Does not possess gestagenic activity.
Pharmacokinetics
The maximum concentration after a single oral administration of mifepristone at a dose of 600 mg is reached after 1.5 hours and is 1.98 mg / l. Absolute bioavailability - 69%.
Plasma protein binding (albumin and acid alpha-1-glycoprotein) - 98%.
The half-life is 18 hours.
The distribution phase lasts up to 12 hours, then between 12–72 hours after administration, a slow elimination of mifepristone occurs, during this period its concentration decreases by 2 times. Further, a faster elimination of the drug occurs.
Indications for use
According to the instructions, Pencrofton is indicated for medical interruption of uterine pregnancy in the period up to 42 days of amenorrhea.
In addition, the drug is used in preparation for childbirth and for induction of labor during full-term pregnancy.
Contraindications
- acute or chronic form of renal and / or hepatic failure;
- hemostatic disorders, including previous anticoagulant therapy;
- adrenal insufficiency;
- long-term use of glucocorticosteroids;
- porphyria;
- anemia;
- uterine fibroids;
- inflammatory diseases of the genital organs;
- the presence of a scar on the uterus;
- severe extragenital pathology;
- smoking over the age of 35 - without consulting a therapist;
- individual intolerance to the components of the drug.
Additional contraindications when using the drug for abortion:
- the gestation period exceeds 42 days of amenorrhea;

Instructions for the use of Pencrofton: method and dosage
Pencrofton tablets are taken orally.
The use of the drug is carried out in the presence of a doctor in specialized medical institutions equipped with the necessary resuscitation equipment. After taking Pencrofton, the woman's condition should be carefully monitored.
Recommended dosage:
- early medical termination of pregnancy: 3 tablets (600 mg) once. During the first two hours, the patient must be provided with medical supervision. After 1.5–2 days, an ultrasound examination (ultrasound) is shown. In order to confirm the termination of pregnancy, a clinical examination is carried out after 8-14 days, which includes an ultrasound scan and determination of the level of beta-chorionic hormone. If, on the 14th day after taking the drug, an ongoing pregnancy or an incomplete abortion is diagnosed, vacuum aspiration is performed and subsequent histological examination of the aspirate;
- preparation and induction of labor in full-term pregnancy: 1 tablet 1 time per day for two days. After 2-3 days, an assessment of the condition of the birth canal should be carried out. If necessary, the appointment of prostaglandins or oxytocin is possible.
Side effects
- postpartum complication: subinvolution of the uterus, lochiometer, metrorrhagia, discomfort and pain in the lower abdomen;
- from the urinary and reproductive systems: violation of the menstrual cycle, exacerbation of inflammatory diseases of the uterus, appendages and / or urinary tract, amenorrhea;
- others: weakness, nausea, vomiting, hyperthermia, diarrhea, chills, headache, dizziness, urticaria.
Overdose
Symptoms: Taking more than 10 tablets (2 g of mifepristone) may cause symptoms of adrenal insufficiency.
The appointment of symptomatic therapy is recommended.

The use of Pencrofton should be carried out in compliance with all abortion-related requirements, including examination to prevent Rh alloimmunization.
The patient should be informed that if there is no effect from the use of the drug for medical abortion within 10-14 days, the ongoing pregnancy must be terminated in another way, since there is a high probability of fetal malformations.
In women with infective endocarditis or an artificial heart valve, mifepristone should be given in combination with antibiotics for prophylaxis.
