Release form and composition

The dosage form of the drug is a transdermal patch: size - 70 × 100 mm (15 mg / day) or 100 × 140 mm (30 mg / day); rectangular with rounded edges; consists of a beige polyester base, on which an adhesive layer from pale yellow to colorless with a weak aromatic odor is evenly applied; the adhesive surface is covered with a transparent polyethylene protective film with two transverse cuts, one of which has a wavy line; there is a drawing on the protective film that shows the way to remove the film from the surface of the plaster (2, 5, 7 or 10 pcs. in aluminum foil bags with / without zip-lock for multiple opening, 1 bag in a cardboard box).

Composition of 1 patch:

  • active ingredient: diclofenac sodium - 15 or 30 mg;

Pharmacological properties

Pharmacodynamics

Voltaren Patch is a non-steroidal anti-inflammatory drug for external use. It is a base with an adhesive layer containing diclofenac, which has pronounced anti-inflammatory and analgesic properties. The mechanism of action of diclofenac is based on the inhibition of prostaglandin synthesis.

The drug provides an analgesic and anti-inflammatory effect at the site of application, as a result of which the pain syndrome is eliminated and the swelling associated with the inflammatory process decreases.

Pharmacokinetics

The amount of diclofenac absorbed systemically from the drug over 24 hours is similar to that when using an equivalent amount of Voltaren Emulgel (gel for external use).

The binding of the substance with plasma proteins is 99.7%, mainly with albumin (99.4%).

The total systemic clearance from plasma is 263 ± 56 ml / min.

Diclofenac and its metabolites are mainly excreted in the urine. The final T 1/2 (half-life) in blood plasma is from 1 to 2 hours. Four metabolites, including 2 active ones, also have a short T 1/2 - 1–3 hours. One of the metabolites - 3'-hydroxy-4'-methoxydiclofenac - has a longer T 1/2, but it is not active.

In case of impaired renal function, accumulation of diclofenac and its metabolites does not occur.

Indications for use

  • joint pain (including knee joints, finger joints) against the background of rheumatoid arthritis, osteoarthritis;
  • back pain associated with degenerative / inflammatory diseases of the spine (lumbago, osteoarthritis, radiculitis, sciatica);
  • swelling and inflammation of soft tissues and joints due to injuries and rheumatic diseases (bursitis, tendovaginitis, lesions of periarticular tissues);
  • muscle pain associated with sprains, overexertion, bruises, trauma.

Contraindications

Absolute:

  • a tendency to the appearance of skin rashes, attacks of bronchial asthma or acute rhinitis when using acetylsalicylic acid or other non-steroidal anti-inflammatory drugs;
  • violation of the integrity of the skin in the area of intended sticking of the patch;
  • III trimester of pregnancy and breastfeeding period;
  • age up to 15 years;
  • individual intolerance to the components of the drug.

Relative (the appointment of Voltaren Patch requires caution):

  • period of exacerbation of hepatic porphyria;
  • bronchial asthma;
  • severe impairment of hepatic / renal function;
  • erosive and ulcerative lesions of the gastrointestinal tract;
  • chronic heart failure;
  • elderly age.

Instructions for the use of Voltaren Patch: method and dosage

The drug is applied externally (as an application to the skin).

Voltaren Patch is applied to the skin over the painful area for 24 hours (140 cm 2 is intended for use on large painful areas). During the day, only 1 patch is permissible.

Recommended duration of therapy:

  • damage to soft tissues - up to 14 days;
  • joint / muscle diseases - up to 21 days.

If after 7 days of use there is no improvement, or if the condition worsens, medical advice is required.

Side effects

As a rule, adverse reactions are characterized by transient and moderately pronounced skin manifestations at the site of the Voltaren Patch application.

Possible adverse reactions (> 10% - very common;> 1% and 0.1% and 0.01% and <0.1% - rarely; <0.01% - very rare):

  • systemic reactions: very rarely - photosensitization reactions, generalized skin rash, allergic reactions (urticaria, hypersensitivity, including asthma attacks, angioedema, bronchospastic reactions);
  • local reactions: often - erythema, dermatitis, including contact dermatitis (symptoms manifest as erythema, itching, swelling of the treated area of the skin, rash, papules, vesicles, peeling), eczema; rarely, bullous dermatitis; very rarely - pustular eruptions.

Overdose

Due to the extremely low systemic absorption of the active substance, overdose is practically impossible.

Do not apply the patch to damaged skin, including open wounds.

Do not allow Voltaren Patch to get into mucous membranes (including the membranes of the eye).

With too long use, the likelihood of developing systemic adverse reactions cannot be completely ruled out.

In cases of use with other dosage forms of diclofenac, it should be taken into account that the maximum daily dose of the substance is 150 mg.

Influence on the ability to drive vehicles and complex mechanisms

The use of Voltaren Patch does not affect the ability to drive a car and complex mechanisms, as well as perform work that requires a high speed of psychomotor reactions.

Application during pregnancy and lactation

According to the instructions, Voltaren Patch should not be used during pregnancy, especially in the third trimester, as well as during lactation.

Pediatric use

For patients under 15 years of age, the use of Voltaren Patch is contraindicated.

With impaired renal function
