Tobrazon is a combined drug with antibacterial and anti-inflammatory action for topical use in ophthalmology.
Release form and composition
Torazon is produced in the form of eye drops: a clear solution from a slightly yellow to colorless (in polyethylene dropper bottles, 5 ml of solution, in cardboard boxes 1 dropper bottle).
1 ml of solution contains:
- active ingredients: dexamethasone (in the form of dexamethasone sodium phosphate) - 1 mg, tobramycin (in the form of tobramycin sulfate) - 3 mg;
- auxiliary components: disodium salt of ethylenediaminetetraacetic acid (EDTA), disodium hydrogen phosphate dihydrate, benzalkonium chloride, sodium chloride, sodium metabisulfite, propylene glycol, creatine, purified water, sulfuric acid or sodium hydroxide (to adjust the pH level).

Torazon contains a local glucocorticosteroid (GCS) and an antibiotic aminoglycoside as active ingredients, due to the combination of which the drug has an anti-inflammatory and antibacterial effect when applied topically in ophthalmology.
Pharmacodynamic properties of the components of Tobrazon:
- tobramycin: a broad-spectrum antimicrobial agent belonging to the aminoglycoside group; inhibits the development and growth of both gram-positive and gram-negative microorganisms. Blocks a small (30S) subunit in ribosomes, formed by one molecule of ribosomal ribonucleic acid (rRNA) and about 30 proteins, and stops protein synthesis. In higher doses, it destabilizes the function of cytoplasmic membranes, thereby provoking cell death. The substance is active against gram-positive bacteria (staphylococci, including golden and epidermal) and gram-negative bacteria (citrobacter, enterobacter, Escherichia coli, Klebsiella, Morgan bacteria, Pseudomonas aeruginosa, Proteus mirabilis, Proteus vulgaris, Providence, Serrations);
- dexamethasone: GCS with a pronounced anti-inflammatory, antiallergic and immunosuppressive effect. When applied topically, it has an antipruritic and vasoconstrictor effect. Interacting with specific receptors of the cytoplasm, dexamethasone forms a complex that penetrates into the cell nucleus and stimulates the synthesis of matrix ribonucleic acid (m-RNA). The substance prevents the release of inflammatory mediators from eosinophils and mast cells, inhibits the activity of collagenase, hyaluronidase, proteases, reduces capillary permeability and stabilizes cell membranes.
Pharmacokinetics
Systemic absorption of Tobrazon when applied topically is minimal and clinically insignificant.
Indications for use
According to the instructions, Tobrazon is recommended to be used for the treatment of such diseases of the organ of vision as conjunctivitis, blepharitis, blepharoconjunctivitis, keratitis (without damage to the epithelium), and the prevention of infectious complications in the postoperative period.
Contraindications
- diseases of the cornea and conjunctiva of a viral nature (including keratitis due to infection with the herpes simplex virus and chickenpox);
- mycoses of the eyes;
- tuberculosis of the eyes;
- conditions after removing a foreign body from the cornea;
- hypersensitivity to any components of the solution.
Instructions for the use of Tobrazon: method and dosage
Eye drops Tobrazon are intended for instillation into the conjunctival sac.
Recommended dosage: 1-2 drops into the affected eye (or eyes) with an interval of 4-6 hours. In case of a severe course of the infection, 2 drops are instilled with an interval of 1 hour until a therapeutic response is obtained, after which the dose of the drug is gradually reduced until it stops completely treatment.
Side effects
As a result of the use of Torazon on the part of the organ of vision, the following side reactions are possible: long-term use - increased intraocular pressure; constant use for more than 3 months - corneal mycoses, development of cataracts, slowing down of regeneration processes. Local hypersensitivity reactions are also possible, manifested by edema / itching of the eyelids and conjunctival erythema.
Overdose
Symptoms of Tobrazon overdose are: erythema, punctate keratitis, increased production of tear fluid, itching / swelling of the eyelid.
In case of overdose, it is recommended to carry out symptomatic treatment with further discontinuation of the drug.
special instructions
When Torazon needs to be used for more than 10 days in a row, it is required to regularly measure intraocular pressure.
During treatment with Torazon, wearing soft contact lenses is prohibited. If the patient wears hard contact lenses, they should be removed before instillation of the drug and put on again 15-20 minutes after the procedure.
After each use of drops, the bottle must be securely closed. During instillation, do not touch the eye or any other objects with the pipette tip.
Influence on the ability to drive vehicles and complex mechanisms
Patients who, after instillation of Tobrazon, experience a temporary visual impairment, should refuse to drive a car and other types of work with complex equipment that require clarity of visual perception for the period of therapy.
Application during pregnancy and lactation
There is not enough experience with the use of Tobrazon during pregnancy / breastfeeding, and therefore, when treating pregnant / lactating women, it is recommended to use the drug only if the expected clinical effect for the patient exceeds the potential risk of side effects for the fetus / child.
Pediatric use
There is not enough experience of using Tobrazon in pediatric practice. In this regard, it is possible to use the drug for the therapy of children, but only if the expected clinical effect outweighs the potential risk of developing negative side reactions.
Drug interactions
When using eye drops Torazon simultaneously with GCS and / or antibiotics of the aminoglycoside group for systemic use, the risk of developing systemic side effects increases, which requires monitoring the total concentration of active substances in the blood serum.
