Release form and composition

Dosage form of Terazosin - tablets: round, flat, engraved on one side "NU", or "APO", or "PRO"; in a dosage of 2 mg - pink with an orange tint, engraving on the other side - "T2", in a dosage of 5 mg - light brown with a pink tint, engraving on the other side - "T5" (10 tablets in a blister, in a cardboard box by 2 blisters; 100 tablets in a bottle, 1 bottle in a cardboard box).

Composition of 1 tablet:

  • active substance: terazosin hydrochloride dihydrate in terms of terazosin - 2 mg or 5 mg;

Pharmacological properties

Pharmacodynamics

Terazosin is an alpha1-adrenergic blocker capable of selectively blocking alpha1-adrenergic receptors in the smooth muscles of the celiac vessels, the vessels of the prostate gland, and the bladder neck. Possessing an antihypertensive effect, it promotes the expansion of large resistance vessels and a decrease in OPSS (total peripheral vascular resistance), activation of the renin-angiotensin-aldosterone system.

The drug helps to reduce the dysuric phenomenon by relaxing the smooth muscles of the bladder neck and contractile elements of the prostate, for example, in adenoma. Reduces the level of total cholesterol, triglycerides, increases the level of high density lipoproteins, normalizing the lipid profile in plasma. With prolonged therapy, there is a regression of left ventricular hypertrophy.

Pharmacokinetics

Terazosin when taken orally is rapidly and almost completely absorbed. Its bioavailability is about 90%. The maximum concentration of the substance in the plasma is reached within 1 hour. The connection with plasma proteins is 90–94%. It is metabolized in the liver to form 4 metabolites. The half-life is 12 hours. Excretion occurs through the intestines (60%) and kidneys (40%). With liver diseases, a decrease in clearance and, as a result, prolongation of pharmacological effects is possible.

Indications for use

  • arterial hypertension;
  • benign prostatic hyperplasia (as part of complex therapy).

Contraindications

Absolute:

  • arterial hypotension;
  • age up to 18 years;
  • lactation period;
  • increased sensitivity to the components of the drug or other adrenergic blockers.

  • angina pectoris;

Instructions for the use of Terazosin: method and dosage

Terazosin tablets are taken orally, 1 time per day. The dose of the drug is selected in each case individually and depends on the patient's response to therapy.

Recommended dosage:

  • arterial hypertension: the initial dose is 1 mg / day at bedtime. Further, it is possible to gradually increase the dose until the desired blood pressure level is reached. The usual dose of the drug is 1 to 5 mg / day. The maximum recommended dose is 20 mg / day. If, 24 hours after taking Terazosin, its effect is greatly reduced, you can increase the dose of the drug or the frequency of its administration (up to 2 times a day);
  • benign prostatic hyperplasia: the initial dose is 1 mg / day at bedtime, while the patient needs monitoring due to the risk of acute arterial hypotension. Further, a gradual increase in the dose is possible at intervals of 1 week to 2, 5 or 10 mg / day.

In the event of a break in taking the drug for several days or longer, its administration should be resumed from the initial dose.

Side effects

  • drowsiness, fatigue, weakness;
  • headache, dizziness, anxiety;
  • decreased libido;
  • hearing and vision impairment;
  • hypotension, tachycardia, arrhythmia;
  • peripheral edema;

After taking the first minimum dose (not exceeding 1 mg / day), the patient should stay in bed for 6-8 hours to avoid orthostatic hypotension, the risk of which is greatest during the first 30-90 minutes after taking the first dose of the drug. The risk group includes patients receiving beta-blockers, diuretics, patients with a reduced volume of fluid in the body, on a diet low in salt. In addition, care should be taken when resuming the drug after a break of several days.

Before starting therapy for benign prostatic hyperplasia, prostate cancer should be excluded. The effectiveness of therapy should be assessed after 4-6 weeks of treatment with maintenance doses.

Drinking alcohol, standing for a long time, exercising, and in hot weather increases the risk of orthostatic hypotension.

Influence on the ability to drive vehicles and complex mechanisms

Care should be taken when driving vehicles or other potentially dangerous mechanisms that require the speed of psychomotor reactions within 12 hours after taking the first dose of the drug, as well as after each dose increase.

Application during pregnancy and lactation

According to the instructions, Terazosin is not recommended to be taken during pregnancy, except in cases where the expected benefit to the mother outweighs the possible risk to the fetus.

There is no data on the ability of the drug to penetrate into breast milk, and therefore, if it is necessary to use Terazosin during lactation, you should consider stopping breastfeeding.

With impaired renal function

Patients with impaired renal function require careful monitoring when using Terazosin.

Use in the elderly

In elderly patients, the risk of developing orthostatic hypotension is increased, and therefore the use of Terazosin in them requires special care. In addition, the risk of visual impairment, dizziness and fainting increases. Over the age of 75, the drug may have only a slight positive effect.

Drug interactions

  • beta-blockers, nitrates, diuretics, slow calcium channel blockers, ACE inhibitors, sympatholytics, ethanol, levodopa, alprostadil, antidepressants, local anesthetics, antipsychotics (neuroleptics), muscle relaxants: enhance the effect of terazosin;
  • verapamil: the risk of symptomatic arterial hypotension and tachycardia increases;
  • hypnotics and anxiolytics: increased hypotensive and sedative effects;
  • glucocorticosteroids, estrogens, combined oral contraceptives, non-steroidal anti-inflammatory drugs: weaken the hypotensive effect;
  • antacids and adsorbents: reduce the absorption of the drug.
