Tafen Nasal is a glucocorticosteroid (GCS) drug for intranasal use.

Release form and composition

The dosage form of the drug is a dosed nasal spray: a homogeneous suspension of white or almost white color (200 doses in dark glass vials, complete with a dispenser, a tip and a nosepiece; in a cardboard box 1 set).

Composition of 1 dose of the drug:

  • active substance: budesonide - 50 mcg;
  • auxiliary components: propyl parahydroxybenzoate, methyl parahydroxybenzoate, sodium carboxymethyl cellulose / microcrystalline cellulose, polysorbate 80, simethicone emulsion, sucrose, EDTA disodium salt, hydrochloric acid, propylene glycol, purified water.

Pharmacological properties

Pharmacodynamics

The active component of Tafen Nasal is budesonide, a glucocorticosteroid for intranasal use, which has a pronounced anti-inflammatory and antiallergic effect. Does not possess mineralocorticoid activity. When used in therapeutic doses, it has almost no resorptive effect. Well tolerated with long-term treatment.

Budesonide inhibits the release of inflammatory mediators, increases the synthesis of anti-inflammatory proteins, and reduces the number of eosinophilic granulocytes and mast cells. Reduces the release of lymphokines from lymphocytes, free radicals from macrophages and toxic proteins from eosinophils. Also, budesonide reduces the binding of adhesive molecules to endothelial cells, thereby reducing the flow of leukocytes to the site of allergic inflammation. Increases the number of beta-adrenergic receptors of smooth muscles. It inhibits the activity of phospholipase 2A, as a result of which the synthesis of surfactants (surfactants), leukotrienes and prostaglandins, which are mediators that induce an inflammatory reaction, is inhibited.

Budesonide also inhibits the synthesis of histamine, as a result of which its level in mast cells decreases.

Tafen Nasal suppresses the late and early phases of an allergic reaction, reduces the severity of symptoms in allergic rhinitis, and reduces inflammation in the upper respiratory tract. The therapeutic effect appears after 2-3 days of using the drug.

Pharmacokinetics

After inhalation of budesonide at the maximum recommended daily dose (400 μg), the maximum plasma concentration is 1 nmol / L and is reached within 0.7 hours. Approximately 20% of the dose enters the systemic circulation.

Budesonide is characterized by good tissue distribution and binding to plasma proteins. The volume of distribution is 301 liters.

The systemic bioavailability of the drug is low, since more than 90% of absorbed budesonide is inactivated during the one-step metabolism in the liver. The glucocorticoid activity of metabolites does not exceed 1%.

The half-life is 2-3 hours. The metabolites are excreted mainly in the urine (90%) and feces.

Indications for use

Spray Tafen Nasal is recommended for the treatment and prevention of year-round and seasonal allergic rhinitis, non-allergic rhinitis and nasal polyposis.

Contraindications

  • mycoses, viral and bacterial infections of the respiratory tract;
  • pulmonary tuberculosis in active form;
  • children under 6 years old;
  • individual hypersensitivity to budesonide or other components of the drug.

Instructions for the use of Tafena Nazal: method and dosage

Spray Tafen Nasal is intended for injection into the nasal passages.

Recommended dosage regimen for children over 6 years of age and adult patients:

  • the initial period of treatment: in each nostril, 2 doses (100 mcg of budesonide) 2 times a day;
  • standard maintenance therapy: in each nostril 1 dose 2 times a day or 2 doses 1 time a day, in the morning (maintenance dose is the lowest effective dose that relieves rhinitis symptoms).

The maximum single dose is 200 mcg (100 mcg in each nostril), the maximum daily dose is 400 mcg, the duration of use is no more than three months.

For a full therapeutic effect, it is necessary to apply the spray regularly and correctly.

If the next dose is missed, it should be taken as soon as possible, but no later than an hour before the next regular dose.

Side effects

Side effects that develop when using Tafen Nasal spray (usually extremely rare and transient):

  • respiratory system: cough, irritation of the mucous membrane of the nasal passage and throat, nosebleeds; less often there is dryness of the nasal mucosa, sneezing;
  • dermatological responses: urticaria, rash, dermatitis;
  • others: dizziness, increased fatigue.

In isolated cases, due to the use of nasal corticosteroids, the following were noted: angioedema, perforation of the nasal septum, loss of smell, growth retardation, tachycardia.

Overdose

In case of an accidental overdose of Tafen Nasal, no obvious symptoms were recorded. Acute overdose is unlikely.

Long-term use of high doses, as well as the simultaneous administration of other corticosteroids, can cause signs of hypercortisolism. In this case, you should stop using the drug, reducing the dose gradually.

When switching from therapy with systemic corticosteroids to a nasal spray (due to the risk of developing adrenal insufficiency), increased attention to patients is required during the period of restoration of the function of the hypothalamic-pituitary-adrenal system (HPA).

GCS slow down wound healing, therefore, when prescribing Tafen Nasal spray, patients who have recently undergone a nose injury / operation should be careful.

In the treatment of allergic rhinitis, the drug must be taken regularly for the full therapeutic effect.

Avoid getting the spray in the eyes.

According to the instructions, Tafen Nazal does not affect the concentration of attention and the speed of psychomotor reactions necessary for driving vehicles and controlling complex mechanisms.

Application during pregnancy and lactation

Tafen Nasal can be used during pregnancy only as directed by a doctor, who will assess the degree of the ratio of the expected benefits and potential risks.

If treatment is necessary during lactation, it is recommended to stop breastfeeding.

Pediatric use

Tafen Nazal is allowed to be used to treat children over 6 years old.

Drug interactions

  • phenytoin, phenobarbital, rifampicin (inducers of microsomal oxidation): the effectiveness of the drug may decrease;
  • methandrostenolone, ketoconazole, estrogens: enhance the effect of budesonide.
