Sonizin is a drug used in the treatment of urinary disorders associated with benign prostatic hyperplasia.

Release form and composition

Dosage form of release of Sonizin - capsules with modified release: size No. 2, hard gelatinous, opaque, brown cap, body - brown-yellow; the capsules contain white or almost white pellets (in a carton box 1 or 3 blisters of 10 capsules each).

Composition of 1 capsule:

  • active substance: tamsulosin hydrochloride - 0.4 mg;
  • auxiliary components: calcium stearate - 0.8 mg; triethyl citrate - 1.1 mg; talc - 2.5 mg; copolymer of ethyl acrylate and methacrylic acid (1: 1) - 43.8 mg (also contains polysorbate 80 - 1 mg, sodium lauryl sulfate - 0.3 mg); microcrystalline cellulose - 281.4 mg;
  • capsule cap: yellow iron oxide dye - 0.2%; titanium dioxide - 0.333 3%; black iron oxide dye - 0.53%; dye iron oxide red - 0.93%; gelatin - up to 100%;

Pharmacological properties

Pharmacodynamics

Sonisin is one of the alpha1-blockers.

Tamsulosin competitively and selectively blocks postsynaptic α 1A -adrenergic receptors located in the smooth muscles of the prostatic urethra, bladder neck and prostate gland, as well as α 1D -adrenergic receptors located mainly in the urinary bladder body.

Thanks to the action of the drug, there is a decrease in the tone of the smooth muscles of the prostate gland, the prostatic part of the urethra and the neck of the bladder, and an improvement in detrusor function. This leads to a decrease in the symptoms of obstruction and irritation that are associated with benign prostatic hyperplasia.

The therapeutic effect, as a rule, develops 14 days after the start of taking Sonizin, although in some patients a decrease in the severity of symptoms is noted on the first day of using the drug.

The ability of an active substance to affect α 1A -adrenergic receptors is 20 times higher than its ability to interact with α 1B -adrenergic receptors located in vascular smooth muscles. Due to this high selectivity, Sonisin does not cause any clinically significant decrease in blood pressure.

Pharmacokinetics

Tamsulosin, after oral administration, is almost completely and rapidly absorbed from the gastrointestinal tract. Bioavailability is approximately 100%. After a single dose of 0.4 mg of the substance, its C max (maximum concentration) in plasma is reached after 6 hours.

C max of tamsulosin in blood plasma after 5 days course of receiving (at equilibrium) higher than the C max after a single dose of the drug, 60-70%.

Plasma protein binding - 99%. V d (volume of distribution) of tamsulosin is insignificant (about 0.2 l / kg).

Tamsulosin is not affected by the "first pass" effect. The substance is slowly biotransformed in the liver, followed by the formation of metabolites with pharmacological activity (retain high selectivity for α 1A -adrenergic receptors). Most of tamsulosin is present in the blood unchanged.

Excretion is carried out by the kidneys, 9% of the dose received is excreted unchanged.

With a single dose, T 1/2 (half-life) of tamsulosin is 10 hours, after repeated use - 13 hours. The final T 1/2 is 22 hours.

Indications for use

According to the instructions, Sonizin is prescribed for the treatment of dysuria, which is associated with benign prostatic hyperplasia.

Contraindications

Sonizin is contraindicated for patients with individual intolerance to its components.

Relative contraindications (the drug is prescribed under medical supervision):

  • arterial hypotension (including orthostatic);
  • severe liver failure;
  • chronic renal failure (with creatinine clearance less than 10 ml / min).

Instructions for the use of Sonizin: method and dosage

Sonizin is taken orally, preferably at the same time of day, after meals, with a sufficient amount of water.

In order not to disrupt the prolonged release of the active substance, the capsule should not be chewed / broken into pieces.

The recommended dosage regimen is 1 capsule per day.

Side effects

  • digestive system: in some cases - nausea, constipation / diarrhea, vomiting;
  • central nervous system: rarely - asthenia, dizziness, headache, sleep disturbances (drowsiness / insomnia);
  • cardiovascular system: in isolated cases - palpitations, tachycardia, orthostatic hypotension;
  • genitourinary system: rarely - decreased libido, retrograde ejaculation;
  • allergic reactions: in some cases - angioedema, skin rash, itching;
  • others: rarely - rhinitis, back pain; in isolated cases - chest pain.

Overdose

Cases of acute overdose of Sonisin have not been described. Theoretically, acute arterial hypotension and compensatory tachycardia can develop.

Therapy: To restore blood pressure and normalize the heart rate, the patient must be laid down. Cardiotropic therapy is prescribed. It is necessary to monitor renal function and provide general supportive care. If symptoms persist, the introduction of volume-replacing solutions, vasoconstrictor drugs is required.

In order to prevent further absorption of tamsulosin, gastric lavage, osmotic laxative or activated charcoal may be prescribed. Dialysis is ineffective.

Sonisin can lead to low blood pressure and fainting (in rare cases). If the first signs of orthostatic hypotension appear (in the form of dizziness, weakness), the patient must be seated / laid down until they disappear.

In order to exclude any other disease proceeding with similar symptoms, a preliminary examination of the patient is shown before the appointment of Sonizin. A digital rectal examination of the prostate should be done and prostate specific antigen (PSA) levels measured. Examinations in the future should be carried out regularly.

Influence on the ability to drive vehicles and complex mechanisms

During the period of therapy, you should refrain from driving.

With impaired renal function

Sonisin for chronic renal failure (creatinine clearance less than 10 ml / min) should be used under medical supervision.

For violations of liver function

Sonizin in severe liver failure should be used under medical supervision.

Drug interactions

Possible interactions:

  • cimetidine: an increase in the plasma concentration of tamsulosin (dose adjustment is not required);
  • furosemide: a decrease in the plasma concentration of tamsulosin (dose adjustment is not required);
  • diclofenac and indirect anticoagulants: increased elimination of tamsulosin;
  • other alpha1-blockers, blockers of slow calcium channels, alprostadil, acetylcholinesterase inhibitors, beta-blockers, anesthetics, levodopa, diuretics, antidepressants, ethanol, nitrates: increased intensity of the hypotensive effect of tamsulosin.
