Ostalon is a bone resorption inhibitor.

Release form and composition

Dosage form Ostalon - film-coated tablets: round, biconvex, white, engraved with "M14" on one side (4 pcs. In a blister, 1 blister in a cardboard box).

Composition of 1 tablet:

  • active substance: alendronate sodium trihydrate - 91.35 mg (equivalent to alendronic acid - 70 mg);
  • auxiliary components: microcrystalline cellulose, colloidal silicon dioxide, croscarmellose sodium, magnesium stearate;
  • film shell: Lustre Clear LS (microcrystalline cellulose, carrageenan, macrogol 8000).

Pharmacological properties

Pharmacodynamics

Ostalon is a blocker of osteoresorption in osteoporosis. Its active ingredient, alendronate sodium - bisphosphonate, a synthetic analogue of pyrophosphate, is required for the binding of hydroxyapatite to bone tissue.

The drug is a non-hormonal inhibitor of osteoclasts, does not affect bone formation, but has an inhibitory effect on bone resorption. It normalizes a positive balance of bone repair and resorption, stimulates osteogenesis. Thanks to alendronate, bone mineral density increases, its structure and composition are normalized.

Pharmacokinetics

When the drug is taken in the morning on an empty stomach, its absorption is 0.64%, in the case of a break between taking Ostalon and food for less than 2 hours - 0.39-0.46%. This decrease in absorption, however, does not significantly affect the effectiveness of the drug. With the joint use of coffee or orange juice, the bioavailability of alendronate is reduced by 60%.

The substance is quickly incorporated into bone tissue after temporary distribution in soft tissues. Plasma protein binding is about 78%. The substance that is not embedded in the bone tissue is excreted by the kidneys in a short time.

Indications for use

According to the instructions, Ostalon is indicated for the treatment of osteoporosis in postmenopausal women, as well as in men, to reduce the risk of fractures of the spinal vertebrae and pelvic bones.

Contraindications

Absolute contraindications:

  • impaired patency of the esophagus (developmental anomalies, achalasia, stricture, etc.);
  • inability of the patient to remain upright for more than half an hour;
  • hypercalcemia;
  • vitamin D deficiency;
  • severe disorders of mineral metabolism;
  • chronic renal failure with CC (creatinine clearance) ˂ 35 ml / min;
  • pregnancy and lactation;
  • age up to 18 years;
  • increased sensitivity to the components of the drug.

The drug should be used with caution in exacerbations of diseases of the gastrointestinal tract (GIT) (gastritis, dysphagia, diseases of the esophagus, ulcer, duodenitis), as well as within 1 year after a serious illness of the digestive tract or surgery, with the exception of surgery on the spastic pylorus stomach.

Instructions for use of Ostalon: method and dosage

Ostalon is taken orally, whole, the tablets are not chewed and do not dissolve, washed down with plain water. For 30 minutes after ingestion, you should remain upright. After this period you can have breakfast.

Recommended dosage: 1 pc. (70 mg) 1 time per week, in the morning on an empty stomach, 2 hours (but not less than 30 minutes) before a meal or liquid.

Before taking the drug, you should drink at least 200 ml of water.

Along with alendronate, calcium and vitamin D are recommended.

Side effects

  • digestive system: dyspepsia, abdominal pain, constipation, diarrhea, esophageal ulcer, flatulence, dysphagia, bloating, heartburn, nausea, vomiting, gastritis, esophageal erosion, esophagitis, melena, narrowing of the esophagus, oropharyngeal ulcer, perforation, upper gastrointestinal ulcers, bleeding (the relationship with treatment is not always clear);
  • musculoskeletal system: arthralgia, ostalgia, myalgia;
  • nervous system: headache, irritability;
  • sense organs: uveitis, scleritis;
  • other: rash, itching, skin flushing, photosensitivity, hypocalcemia;
  • allergic reactions: urticaria, angiodema, Stevens-Johnson syndrome, toxic epidermal necrolysis.

At the beginning of treatment, there may be transient symptoms that resemble the phase of an acute reaction (weakness, myalgia, feeling unwell, fever).

Overdose

Ostalon overdose symptoms: heartburn, gastritis, ulcer, esophagitis, hypophosphatemia, hypocalcemia.

There is no specific antidote. It is recommended to give the patient an upright position, the intake of milk and antacids is indicated. Vomiting should not be induced due to the risk of esophageal irritation.

Due to the fact that alendronate can irritate the mucous membrane of the digestive tract, the course of diseases of the upper gastrointestinal tract may worsen.

There is a risk of serious damage to the esophagus due to taking Ostalon, when the first signs of irritation appear, such as dysphagia, chest pain when swallowing, heartburn, therapy should be interrupted and a doctor should be consulted.

Violation of the rules for taking the drug is not allowed, as well as the use of two tablets in one day. If the next pill is missed, it should be taken the next morning.

During the period of Ostalon therapy, you should adhere to a diet rich in calcium salts.

Influence on the ability to drive vehicles and complex mechanisms

Treatment with Ostalon does not affect the ability to drive vehicles and other complex mechanisms that require the speed of psychomotor reactions.

With impaired renal function

Ostalon is not prescribed for patients with chronic renal failure with CC less than 35 ml / min.

In case of impaired renal function with CC more than 35 ml / min, dose adjustment is not required.

Use in the elderly

Elderly patients do not require dose adjustment.

Drug interactions

Drugs, as well as food, drinks, including mineral water, should be taken orally no earlier than 1 hour after taking Ostalon.

Ranitidine increases the bioavailability of alendronate.

Nonsteroidal anti-inflammatory drugs increase the side effects of Ostalon.
