Neuromidin is a drug with an anticholinesterase effect, used to treat diseases of the central and peripheral nervous system.
Release form and composition
Neuromidine is available in the following dosage forms:
- Tablets: flat-cylindrical, white, round, with a chamfer (10 pcs. In blisters made of polyvinyl chloride film and aluminum foil, 5 packages in a cardboard box);
- Solution for intramuscular and subcutaneous administration: transparent, colorless (1 ml in neutral glass ampoules, 5 ampoules in blisters, 2 packages in a cardboard box).
The composition of 1 tablet includes:
- Active ingredient: ipidacrine hydrochloride - 20 mg (in the form of ipidacrine hydrochloride monohydrate);
- Auxiliary components: lactose monohydrate - 65 mg, potato starch - 14 mg, calcium stearate - 1 mg.
The composition of 1 ml of solution includes:
- Active ingredient: ipidacrine hydrochloride - 5 or 15 mg (in the form of ipidacrine hydrochloride monohydrate);
- Auxiliary components: concentrated hydrochloric acid - up to pH 3, water for injection - up to 1 ml.
Indications for use
- Diseases of the peripheral nervous system, including polyradiculopathy, mono- and polyneuropathy, myasthenic syndrome of various etiologies and myasthenia gravis;
- Diseases of the central nervous system, including bulbar paresis and paralysis. Also, Neuromidin is prescribed during the recovery period after organic lesions of the central nervous system, occurring with cognitive and / or motor impairment;
- Intestinal atony - therapy and prevention (tablets).
Contraindications
Absolute:
- Extrapyramidal diseases with hyperkinesis;
- Epilepsy;
- Vestibular disorders;
- Angina pectoris;
- Bronchial asthma;
- Severe bradycardia;
- Mechanical obstruction of the urinary tract or intestines;
- Exacerbation of gastric ulcer or duodenal ulcer;
- Age up to 18 years (due to the lack of systematic data on the efficacy and safety of the drug in this age group of patients);
- Pregnancy (due to increased uterine tone);
- Lactation period;
- Hypersensitivity to drug components.
Relative (Neuromidin should be used with caution in the presence of the following conditions / diseases):
- Peptic ulcer of the stomach and duodenum;
- Diseases of the cardiovascular system;
- Thyrotoxicosis;
- Diseases of the respiratory tract in the acute course or obstructive diseases of the respiratory system in history;
- Lactase deficiency, lactose intolerance, isomaltose / lactose malabsorption syndrome (tablets).
Method of administration and dosage
Neuromidine is administered orally, intramuscularly or subcutaneously. The duration of therapy and doses are determined by the doctor individually based on the severity of the disease.
Pills
- Polyradiculopathy, mono- and polyneuropathy, myasthenic syndrome of various etiology and myasthenia gravis: 1-3 times a day, 1 / 2-1 tablets (10-20 mg). The duration of the course is 1-2 months. According to the indications, it is possible to carry out several treatment courses with an interval of 1-2 months;
- Severe disorders of neuromuscular conduction, as well as prevention of myasthenic crises: 1-2 ml (15-30 mg) of 1.5% Neuromidin injection solution is administered parenterally for a short time, after which the drug is taken orally. The daily dose, if necessary, is increased to 5-10 tablets (1-2 tablets 5 times a day);
- Diseases of the central nervous system, including bulbar paresis and paralysis, recovery period after organic lesions of the central nervous system (vascular, traumatic and other genesis), occurring with cognitive and / or motor impairment: 2-3 times a day, 1/2 to 1 tablet, course duration - 2-6 months. Repeat therapy if necessary;
- Intestinal atony (therapy and prevention): 2-3 times a day, 1 tablet for 1-2 weeks.
Injection solution
- Mono- and polyneuropathies of various etiologies: 1-2 times a day, 5-15 mg subcutaneously or intramuscularly, the duration of the course is 10-15 days (in severe cases - up to 30 days). In the future, they switch to taking Neuromidin inside;
- Myasthenia gravis and myasthenic syndrome: 1-3 times a day, 15-30 mg subcutaneously or intramuscularly. In the future, they switch to taking Neuromidin inside. The total duration of treatment is 1-2 months. According to indications, it is possible to conduct several courses with a break of 1-2 months;
- Bulbar paresis and paralysis: 1-2 times a day, 5-15 mg subcutaneously or intramuscularly for 10-15 days. If possible, you should switch to the tablet form of Neuromidin;
- Rehabilitation for organic lesions of the central nervous system: 1-2 times a day, 10-15 mg intramuscularly for 15 days, then it is recommended to switch to the tablet form of the drug.
If you miss a single dose, you should not double the next one.
The maximum daily dose is 200 mg.
Side effects
During therapy, disorders associated with the excitation of m-cholinergic receptors may develop: increased sweating, salivation, palpitations, epigastric pain, diarrhea, nausea, jaundice, bradycardia, bronchospasm, increased secretion of bronchial secretions, convulsions, increased uterine tone, allergic skin reactions (bradycardia and salivation can be reduced with m-anticholinergics - atropine, etc.).
In rare cases (less than 10% of patients), after the use of high doses of Neuromidine, side effects such as: headache, dizziness, chest pain, general weakness, vomiting, drowsiness, skin reactions (in the form of itching and rash) may occur. With the development of these symptoms, the dose should be reduced or the treatment should be interrupted for a short time (by 1-2 days).
special instructions
During the period of therapy, it is necessary to exclude alcohol (due to the possible increase in the side effects of Neuromidin).
During treatment, it is recommended to refrain from driving vehicles, as well as performing potentially hazardous types of work that require quick psychomotor reactions and increased concentration of attention.
Drug interactions
When Neuromidin is used together with certain drugs, the following effects may occur:
- Other cholinesterase inhibitors and m-cholinomimetic agents: enhancement of the action and side effects of the drug;
- Drugs that depress the central nervous system: increased sedation;
- Local anesthetics, aminoglycosides, potassium chloride: weakening of the inhibitory effect on neuromuscular transmission and conduction of excitation along peripheral nerves;
- Other cholinergic drugs: increased risk of developing a cholinergic crisis in patients with myasthenia gravis;
- Alcohol: Increased side effects of ipidacrine hydrochloride.
The risk of developing bradycardia increases if β-blockers were used before starting treatment with Neuromidin.
It is possible to carry out a combined treatment with nootropic drugs.
