Nativa is a drug for the treatment of diabetes insipidus.

Release form and composition

Dosage form of Nativa - tablets 0.1 / 0.2 mg: round, flat, white, with a chamfer and a notch on one side (30 pieces in polymer bottles, in a cardboard box 1 bottle).

Composition of 1 tablet 0.1 / 0.2 mg:

  • active substance: desmopressin acetate - 0.1 / 0.2 mg, in terms of desmopressin - 0.089 / 0.178 mg;
  • auxiliary components: lactose monohydrate, crospovidone XL, magnesium stearate, ludipress (lactose monohydrate, crospovidone, povidone).

Pharmacological properties

Pharmacodynamics

Desmopressin is a structural analogue of the hormone arginine vasopressin and has a pronounced antidiuretic effect. It was obtained in the course of changes in the structure of the vasopressin molecule.

The action of Nativa is due to its ability to increase the permeability of the epithelium of the distal sections of the convoluted tubules of the nephron for water, increasing its reabsorption. At the same time, there are no spastic side effects due to the less pronounced effect of desmopressin on the smooth muscles of blood vessels and internal organs in comparison with vasopressin. The drug works for a long time and does not increase blood pressure.

In the treatment of central diabetes insipidus with desmopressin, the volume of excreted urine decreases, its osmolarity increases, and the osmolarity of blood plasma also decreases, which leads to a decrease in the frequency of urination and a decrease in nocturnal polyuria. The effect of the drug reaches its maximum after 4–7 hours and lasts up to 8–12 hours, depending on the dose of Nativa taken.

Pharmacokinetics

The maximum concentration of desmopressin is reached after 0.9 hours. Food intake reduces the absorption of the substance by 40%. The volume of distribution is 0.2-0.3 l / kg. The substance is unable to penetrate the blood-brain barrier. The half-life is 1.5-2.5 hours. Desmopressin is excreted by the kidneys.

Indications for use

  • diabetes insipidus of central genesis;
  • nocturnal polyuria in adults (for symptomatic therapy);
  • primary nocturnal enuresis in children over 5 years of age.

Contraindications

Absolute contraindications:

  • polydipsia (habitual or psychogenic);
  • conditions requiring the appointment of diuretics (for example, heart failure);
  • hyponatremia with a plasma sodium ion concentration below 135 mmol / l (including a history);
  • renal failure with creatinine clearance (CC) 50 ml / min;
  • syndrome of inadequate production of antidiuretic hormone;
  • lactose intolerance, lactase deficiency, glucose-galactose malabsorption;
  • age up to 4 years;
  • increased sensitivity to the components of the Nativa.

Relative contraindications:

  • age over 65 (due to the risk of fluid retention and hyponatremia);
  • renal failure with CC 50 ml / min;
  • fibrosis of the bladder;
  • violations of water and electrolyte balance;
  • conditions associated with the risk of increased intracranial pressure;
  • pregnancy.

Instructions for use of Nativa: method and dosage

Nativ tablets are taken orally some time after meals. The dose of the drug is selected individually in each case.

Recommended dosage:

  • central diabetes insipidus: the recommended initial dose is 0.1 mg, 1-3 times a day, then the dose is increased depending on the patient's response;
  • primary nocturnal enuresis: the recommended starting dose is 0.2–0.4 mg at bedtime. The duration of treatment should be about 3 months. The decision on the advisability of further therapy is made on the basis of clinical data obtained within 7 days after the drug was discontinued. During the Nativa treatment period, it is recommended to adhere to a mode of limited fluid intake in the evening;
  • nocturnal polyuria in adults: the recommended starting dose is 0.1 mg at bedtime. If there is no effect after 7 days of taking the drug, it is possible to increase the dose to 0.2, and subsequently 0.4 mg / day, with an interval of a week. In the absence of a therapeutic effect within 4 weeks of taking Nativa, its further use is impractical.

Side effects

  • nervous system: headache, dizziness, convulsions;
  • digestive system: nausea, vomiting, dry mouth;
  • cardiovascular system: transient tachyarrhythmia;
  • organ of vision: conjunctivitis;

In addition, peripheral edema may occur, as well as an increase in body weight.

Taking Nativa without restriction on fluid intake can cause fluid retention and hyponatremia.

Overdose

An overdose of Nativa can lead to fluid retention and hyponatremia with the concentration of sodium ions in the blood plasma below 135 mmol / L.

Recommended treatment: immediately stop taking the drug, cancel the limited fluid intake, if necessary, infuse 0.9% or hypertonic sodium chloride solution. In case of the development of symptoms of severe fluid retention (convulsions, loss of consciousness), furosemide is prescribed.

In patients with acute urinary incontinence, nocturia, dysuria, urinary tract infection, polydipsia, decompensated diabetes mellitus, as well as if a tumor of the bladder or prostate is suspected, it is advisable to treat and diagnose these diseases before taking Nativa.

It is recommended to stop taking Nativa in case of development of systemic infections, gastroenteritis and fever.

Influence on the ability to drive vehicles and complex mechanisms

Considering the possibility of developing such undesirable effects as dizziness and headache, care should be taken when driving vehicles and other complex mechanisms that require the speed of psychomotor reactions.

Application during pregnancy and lactation

There is no data on the adverse effects of desmopressin on the course of pregnancy, the condition of the fetus, newborn, and mother, however, before using Nativa, the benefit / risk ratio should be carefully weighed.

The amount of the drug excreted in the mother's milk when Nativa is taken in high doses is insignificant in order to influence the infant's diuresis.

Drug interactions

  • hypertensive drugs: the risk of increasing their effect;
  • buformin, tetracycline, norepinephrine, lithium preparations: reduce the antidiuretic effect of desmopressin;
  • non-steroidal anti-inflammatory drugs (NSAIDs): the risk of side effects increases;
  • indomethacin: enhances the effect of desmopressin without increasing its duration of action;
  • tricyclic antidepressants, selective serotonin reuptake inhibitors, narcotic analgesics, carbamazepine, chlorpromazine, lamotrigine, NSAIDs: may enhance the antidiuretic effect of Nativa, increasing the risk of fluid retention and hyponatremia;
  • loperamide and possibly other drugs that slow down peristalsis: can lead to a threefold increase in plasma desmopressin concentration, significantly increasing the risk of fluid retention and hyponatremia;
  • dimethicone: the absorption of desmopressin may decrease.
