Dosage forms of Natekal D3:

1 tablet of chewable Natekal D3 contains:
- active substance: calcium carbonate - 1500 mg, which is equivalent to the content of 600 mg of calcium; colecalciferol (alpha-tocopherol acetate, sucrose, hydrated soybean oil, corn starch, gelatin) - 400 IU (international unit), which is equivalent to 4 mg of dry vitamin D 3;
- auxiliary components: sodium saccharin, sorbitol, lactose monohydrate, maltodextrin, aspartame, croscarmellose sodium, magnesium stearate, mint flavor, anise flavor, molasses flavor.

Pharmacological properties
Pharmacodynamics
Natekal D3 - regulates the metabolic processes of calcium and phosphorus in the body, replenishing the deficiency of calcium and vitamin D 3, provides an increase in bone density and a decrease in bone resorption.
Vitamin D 3 (cholecalciferol) improves the absorption of calcium from the gastrointestinal tract (gastrointestinal tract) and its binding in the bone tissue.
Therapeutic and prophylactic use of the combined drug prevents the growth of the synthesis of parathyroid hormone, which stimulates an increase in bone resorption.
Pharmacokinetics
Absorption of Natekal D3 occurs in the small intestine.
Calcium is absorbed from the proximal small intestine in ionized form by an active vitamin D-dependent transport mechanism.
Indications for use
- prevention and treatment of osteoporosis as part of complex therapy, including senile, menopausal, steroid, idiopathic forms, and its complications (bone fractures);
- replenishment of calcium and vitamin D deficiency in elderly patients 3.
Contraindications
- hypercalciuria;
- hypercalcemia, including in patients with overactive parathyroid glands;
- urolithiasis with the formation of calcium stones;

Instructions for use of Natekal D3: method and dosage
I take Natekal D3 orally, swallowing after chewing or dissolving (cannot be swallowed whole), preferably during meals.
The recommended dosage when taking the drug by adults as prevention of osteoporosis: 1–2 pcs. in a day.
For the treatment of osteoporosis and replenishment of calcium and vitamin D 3 deficiency in elderly patients, the doctor sets the dosage regimen and the duration of the course of therapy individually.
Side effects
- from the digestive system: abdominal pain, constipation, flatulence, diarrhea, nausea;
- others: the development of allergic reactions, hypercalcemia.
In addition, while taking tablets for resorption, hypercalciuria may develop.
Overdose
Symptoms: nausea, vomiting, thirst, weakness, polyuria, decreased appetite, dizziness, headache, constipation, fainting, coma; hypercalcemia with a plasma calcium content of approximately 2.6 mmol, hypercalciuria. Against the background of a long period of use of the drug, the development of vascular and tissue calcification is possible.
Treatment: rehydration. Loop diuretics, glucocorticosteroids, calcitonin, bisphosphonates. If necessary, hemodialysis sessions. Patient adherence to a calcium-restricted diet.

The use of Natekal D3 should be accompanied by regular monitoring of the concentration of calcium and creatinine in the blood plasma and the excretion of calcium in the urine. If the level of calcium in the urine exceeds 7.8 mmol / 24 h (300 mg / 24 h) and / or the level of calcium or creatinine in the blood serum has increased, the dose of the drug should be reduced or temporarily stopped.
When prescribing a single dose of the drug, it is necessary to take into account the intake of vitamin D 3 additionally from other sources.
The simultaneous use of multivitamin complexes containing calcium and D vitamins is contraindicated.
According to the instructions, Natekal D3 reduces the absorption of bisphosphonate and sodium fluoride, so it is recommended to take it only 3 hours after taking them.
Application during pregnancy and lactation
It is possible to use Natekal D3 during gestation and breastfeeding.
The daily dose of the drug should be no more than 1 tablet (1500 mg of calcium and 600 IU of colecalciferol).
The use of high doses of the drug during pregnancy can cause pathological disorders in the mental and physical development of the fetus.
Calcium and vitamin D 3 are excreted in breast milk.
With impaired renal function
The use of the drug is contraindicated in patients with severe impaired renal function, urolithiasis (with the formation of calcium stones), hypercalciuria.
Care should be taken when using Natekal D3 in case of renal failure.
Use in the elderly
The use of the drug is indicated for elderly patients as a prophylactic agent to compensate for calcium and vitamin D 3 deficiencies.

Drug interactions
With the simultaneous use of Natekal D3:
- phenytoin, barbiturates: reduce the activity of colecalciferol;
- cardiac glycosides: increase their effect under the influence of calcium, which is part of the drug, therefore, this combination must be accompanied by monitoring the patient's clinical condition, including electrocardiography;
- antibiotics of the tetracyclines group: their absorption from the gastrointestinal tract increases, it is recommended to observe a break of at least 3 hours between taking tetracyclines and the drug;
- bisphosphonates, sodium fluoride: their absorption decreases;
- glucocorticosteroids: help to reduce calcium absorption, it is possible to consider increasing the dose of the drug;
- thiazide diuretics: cause an increase in the reabsorption of calcium from the lumen of the renal tubules, increasing the risk of hypercalcemia;
- loop diuretics (diuretics), including furosemide: increase the excretion of calcium by the kidneys;
- cholestyramine, laxatives with vegetable or mineral oils in the composition: reduce the absorption of cholecalciferol.
It should be borne in mind that the acidic reaction of food products causes an increase in calcium absorption, and a weakly alkaline reaction reduces it.
