Nazik is a combined vasoconstrictor drug used in ENT practice.
Release form and composition
The drug is produced in the form of a dosed nasal spray: transparent, colorless (in amber glass vials, 10 ml each (at least 100 doses), 1 bottle in a cardboard box, complete with a dosing pump device).
The composition of 1 dose of the spray contains active substances:
- Xylometazoline hydrochloride - 0.1 mg;
- Dexpanthenol 5 mg
Auxiliary components: benzalkonium chloride - 0.04 mg, potassium dihydrogen phosphate - 0.853 mg, sodium hydrogen phosphate dodecahydrate - 0.027 mg, purified water - 93.98 mg.
Pharmacological properties
Pharmacodynamics
Xylometazoline hydrochloride is an imidazole derivative. It is an alpha adrenostimulant. The action is aimed at narrowing the blood vessels of the mucous membrane of the nasal cavity, as a result of which the edema and hyperemia of the mucous membrane is eliminated.
Xylometazoline facilitates nasal breathing in rhinitis. Its effect occurs approximately 5-10 minutes after the instillation of the drug and lasts for several hours.
Dexpanthenol is a derivative of pantothenic acid. In the body, it is converted into pantothenic acid, which is an integral part of coenzyme A. It is involved in fat and carbohydrate metabolism, acetylation processes, the synthesis of porphyrins, corticosteroids and acetylcholine; normalizes cell metabolism, stimulates the regeneration of mucous membranes and skin, increases the strength of collagen fibers and accelerates the process of cell division.
Dexpanthenol has a metabolic, mild anti-inflammatory and regenerative effect. Effectively affects skin trophism. When applied topically, it can satisfy the increased need for pantothenic acid in cases of damage to the mucous membranes or skin.
Pharmacokinetics
Xylometazoline is practically not absorbed into the systemic circulation when applied topically. Its concentration in plasma is so low that it is impossible to determine it using modern analytical methods.
Dexpanthenol is rapidly absorbed by the skin when applied topically and converted into pantothenic acid, which binds to plasma proteins. Its concentration in blood is 0.5–1 mg / l, in serum - 100 μg / l. It is not metabolized in the body (except for inclusion in coenzyme A). Pantothenic acid is excreted unchanged.
Indications for use
- Allergic rhinitis (in acute course);
- Respiratory diseases with symptoms of rhinitis (in acute course);
- Sinusitis;
- Vasomotor rhinitis;
- Otitis media (along with other drugs to reduce swelling of the nasopharyngeal mucosa).
Also, Nazik spray is prescribed after surgical interventions in the nasal cavity (to restore impaired nasal breathing) and to facilitate rhinoscopy.
Contraindications
- Glaucoma;
- Arterial hypertension;
- Hyperplasia of the prostate;
- Tachycardia;
- Atrophic rhinitis;
- Severe atherosclerosis;
- Porphyria;
- Surgical interventions performed on the meninges (in history);
- Hyperthyroidism;
- Concomitant use with monoamine oxidase inhibitors and tricyclic antidepressants;
- Age up to 6 years;
- Hypersensitivity to drug components.
The spray is not recommended for lactating and pregnant women, as there is no data on its reproductive toxicity.
According to the instructions, Nazik should be used with caution in patients with diabetes mellitus and pheochromocytoma.
Instructions for use Nazik: method and dosage
The drug is administered intranasally.
A single dose of Nasik spray for children from 6 years old and adults - 1 injection into each nasal passage, frequency of application - 3-4 times a day, course duration - 3-5 days.
Before the first use of the drug, you need to press the pump dosing device several times (until a uniform spray occurs).
During the use of the drug, the bottle should be held vertically, when injected, it should be easily inhaled through the nose.
Side effects
With prolonged and / or frequent use, side effects may develop such as: swelling of the nasal mucosa, hypersecretion, burning, dryness and / or irritation of the nasopharyngeal mucosa, tingling, dry rhinitis, sneezing, allergic reactions, vomiting, palpitations, headache, increased blood pressure, blurred vision, insomnia, depression (with prolonged use in high doses).
Overdose
In case of an overdose or accidental ingestion of the drug, the following symptoms may occur: vomiting, nausea, tachycardia, mydriasis, hypertension, cardiac arrhythmias, pulmonary edema, vascular insufficiency, cyanosis, convulsions, mental disturbances, fever, respiratory failure, cardiac arrest.
It is also possible that symptoms of depression from the central nervous system appear, accompanied by bradycardia, drowsiness, shock-type hypotension, a decrease in body temperature, coma and respiratory arrest.
Treatment consists of gastric lavage, intake of activated charcoal and oxygen ventilation of the lungs. In order to lower blood pressure, 100 mg of phentolamine by mouth or 5 mg of phentolamine in isotonic solution is prescribed slowly intravenously. The use of vasoconstrictor drugs is contraindicated. If necessary, prescribe anticonvulsant and antipyretic drugs.

Before using Nazik, the nasal passages must be cleaned.
Do not use the spray for more than 5 days.
During the period of treatment, it is necessary to observe precautions or avoid driving vehicles and performing other potentially hazardous types of work that require an increased concentration of attention and quick psychomotor reactions.

The use of the drug during pregnancy and breastfeeding is not recommended, since there is no data on its reproductive toxicity.

The use of Nazik for children is possible from the age of 6.

With the simultaneous use of monoamine oxidase inhibitors and tricyclic antidepressants, it is possible to increase the systemic action of Nazik.
