Release form and composition

Pharmacological properties

Pharmacodynamics

L-Thyroxin 50 Berlin-Chemie, a thyroid drug whose active ingredient is levothyroxine sodium, is a synthetic levorotatory isomer of thyroxine, the thyroid hormone of the thyroid gland.

After oral administration in the liver and kidneys, levothyroxine sodium is partially metabolized into triiodothyronine and passes into the cells of the body, where it affects the development and growth of tissues, as well as metabolic processes. Low doses of the drug have an anabolic effect on protein and fat metabolism, medium doses - stimulate the growth and development of tissues, increase their oxygen demand, potentiate the metabolism of proteins, fats and carbohydrates, increase the functional activity of the cardiovascular and central nervous system. High doses of levothyroxine sodium inhibit the synthesis of thyrotropin-releasing hormone (TRH) of the hypothalamus and thyroid-stimulating hormone (TSH) of the pituitary gland.

The therapeutic effect occurs after 7-12 days of treatment, during the same period the effect of the drug remains after its withdrawal. In hypothyroidism, the clinical effect is observed after 3-5 days of taking the drug. In hyperthyroidism, regular pills are required for 90-180 days to reduce or disappear diffuse goiter.

Pharmacokinetics

After oral administration, absorption of levothyroxine sodium occurs exclusively in the upper part of the small intestine and accounts for up to 80% of the dose taken. Simultaneous food intake reduces its absorption. The maximum concentration in the serum is reached after 5-6 hours.

Binding to blood serum proteins (thyroxine-binding globulin, thyroxine-binding prealbumin and albumin) is more than 99% of the absorbed active substance.

Approximately 80% of sodium levothyroxine undergoes monodeiodination (loss of one iodine atom) in various tissues, resulting in the formation of triiodothyronine (T3) and inactive products. Thyroid hormone metabolism occurs primarily in the liver, kidneys, brain and muscles. In the liver, a small part of the drug is biotransformed by deamination, decarboxylation, conjugation with sulfuric and glucuronic acids.

The half-life (T 1/2) is 6-7 days. T 1/2 with thyrotoxicosis is reduced to 3-4 days, hypothyroidism - extended to 9-10 days.

Levothyroxine sodium is excreted in the form of metabolites through the kidneys and intestines.

Indications for use

  • hypothyroidism;
  • thyroid cancer (after surgery);
  • euthyroid goiter;

Contraindications

Absolute:

  • untreated thyrotoxicosis;
  • untreated adrenal cortex insufficiency;
  • acute myocarditis, acute myocardial infarction;
  • hypersensitivity to drug components.

L-Tyroxin 50 Berlin-Chemie should be taken with caution in severe long-term hypothyroidism, arterial hypertension, coronary heart disease (angina pectoris, atherosclerosis, myocardial infarction in history), arrhythmias, diabetes mellitus, malabsorption syndrome, during breastfeeding.

L-Thyroxin 50 Berlin-Chemie tablets are taken orally whole, without chewing.

The drug is recommended to be used once a day in the morning on an empty stomach (0.5 hours before breakfast), washed down with a small (100 ml) amount of water.

For infants, the daily dose is given 0.5 hours before the first feeding.

For children under three years of age, the tablet must be thoroughly dissolved in water to a fine suspension, immediately before taking.

The doctor prescribes the daily dose and duration of treatment individually, taking into account clinical indications, age, sex and weight of the patient.

When carrying out replacement therapy for hypothyroidism in patients without cardiovascular diseases, the daily dose of the drug is determined at the rate of 1.6-1.8 μg per 1 kg of body weight. In severe obesity, the dose is calculated as “ideal weight”.

Recommended daily dosage for the initial stage of replacement therapy for hypothyroidism:

  • patients under 55 years of age without cardiovascular diseases: women - 50-100 mcg, men - 50-150 mcg;
  • patients over 55 years old or with cardiovascular diseases: initial dose - 25 mcg. It is increased step by step by 25 μg every 21–42 days until the TSH level in the blood normalizes. If symptoms from the cardiovascular system appear or if they worsen, the therapy of cardiovascular disease should be corrected.

In severe long-term hypothyroidism, the initial daily dose of L-Tyroxin 50 Berlin-Chemie should not exceed 25 μg. It is gradually increased (by 25 μg every 14 days) until a maintenance dose is reached, by regularly determining the concentration of TTT in the blood. For patients with hypothyroidism, the use of L-Thyroxin 50 Berlin-Chemie is indicated throughout life.

The recommended daily dosage for the treatment of congenital hypothyroidism, taking into account the age and weight of the patient:

  • infants up to 0.5 years: 25-50 mcg or 10-15 mcg / kg body weight;
  • children from 0.5 to 1 year: 50-75 mcg or 8-10 mcg / kg body weight;
  • children from 2 to 10 years old: 75-125 mcg or 4-6 mcg / kg of body weight;
  • children and adolescents from 10 to 16 years old: 100-200 mcg or 3-4 mcg / kg of body weight;
  • adolescents over 16 years old and adults: 100-200 mcg or 2-3 mcg / kg of body weight.

Recommended daily dosage of L-Tyroxin 50 Berlin-Chemie according to indications:

  • treatment of euthyroid goiter: 50-200 mcg;
  • prevention of relapse after surgical treatment of euthyroid goiter: 50-200 mcg;
  • thyrotoxicosis (after reaching the euthyroid state as part of complex therapy with antithyroid drugs): 50–100 mcg;
  • suppressive therapy for thyroid cancer: 150–300 mcg;
  • differential diagnostic test of thyroid suppression: 28 days before the test - 75 μg, 21 days before the test - 75 μg, 14 days before the test - 150-200 μg, 7 days before the test - 150-200 μg.

L-Tyroxin Berlin-Chemie tablets are available in a dosage of 50, 75, 100, 125 or 150 μg, which allows you to use the most appropriate form for treatment.

Side effects

In patients with hypersensitivity to the components of the drug, allergic reactions may develop.

Usually, when taking therapeutic doses of L-Tyroxin 50 Berlin-Chemie, side effects are not observed.

Overdose

Symptoms: increased fatigue, insomnia, anxiety, tremors, heart rhythm disturbances, tachycardia, heart pain, hyperhidrosis, headache, decreased appetite, weight loss, vomiting, diarrhea, muscle spasm.

Treatment: taking into account the severity of symptoms, the doctor may recommend a decrease in the daily dose or the withdrawal of the drug for several days. After the undesirable effects disappear, it is recommended to resume treatment with a lower dose. If necessary, the use of beta-blockers is indicated. Antithyroid drugs should not be prescribed.

Drug interactions

With simultaneous use with levothyroxine sodium:

  • amiodarone, aminoglutethimide, ethionamide, paraaminosalicylic acid, antithyroid drugs, chloral hydrate, diazepam, beta-blockers, levodopa, dopamine, lovastatin, metoclopramide, somatostatin: concomitant therapy with each of the listed drugs disrupts the distribution and metabolism of levothyroxine;
  • tricyclic antidepressants: their therapeutic effect is enhanced;
  • insulin and oral hypoglycemic agents: patients with diabetes should take into account the possible increase in the need for insulin or oral hypoglycemic drugs. At the beginning of treatment and when the dose of levothyroxine sodium is changed, it is recommended to monitor blood glucose concentration more often;
  • cardiac glycosides: their effectiveness decreases;
  • cholestyramine, colestipol, aluminum hydroxide: the combination with each of these agents reduces the rate of absorption of levothyroxine sodium in the intestine, and helps to reduce its plasma concentration;
  • indirect anticoagulants: the effect of indirect anticoagulants increases, requiring in some cases a decrease in their dose;
  • anabolic steroids, asparaginase, tamoxifen: there is a risk of pharmacokinetic interaction of these drugs with levothyroxine sodium at the level of protein binding;
  • phenytoin, clofibrate, salicylates, furosemide in high doses: increase the content of sodium levothyroxine and T4 (thyroxine), not associated with blood plasma proteins;
  • somatotropin: the combination with somatotropic hormone accelerates the closure of the epiphyseal growth zones;
  • phenobarbital, carbamazepine, rifampicin: contribute to an increase in the clearance of levothyroxine sodium, an increase in the dose of the drug may be required;
