Collalizine is a keloidolytic agent.
Release form and composition
The dosage form of Collalizin is a lyophilisate for preparation of a solution for local and parenteral use: powder or porous mass of white color (in ampoules, 10 ampoules in a cardboard box, complete with an ampoule knife or scarifier).
The active ingredient is collagenase, in 1 ampoule - 100, 200, 300, 400, 500, 600, 700, 800, 900 or 1000 KU (collagenase units).
Pharmacological properties
Pharmacodynamics
Collalizin's active substance is collagenase, which selectively affects collagen (the main component of connective tissue), which causes its destruction.
The introduction of small doses of collagenase into the tissues of the burnt fornix of the eye 15 days after the burn can either slow down the formation of scar tissue between the burnt surfaces (eyeball and eyelid), or prevent the development of a narrow simblepharon.
Under the influence of aqueous solutions of collagenase on the keloid tissue, a lytic effect is observed, its degree is determined by the exposure time.
Indications for use
Treatment and prevention:
- Simblefaron;
- Corneal opacity;
- Postburn scars of the conjunctiva;
- Cicatricial changes (keloid scars) of the skin of the eyelids;
- Retinal scars;
- Clouding of the vitreous humor;
- Secondary cataract;
- Strictures of the lacrimal ducts (lacrimal tubules and lacrimal-nasal canal);
- Traumatic hemorrhage into the vitreous body, occupying more than 1/2 of its volume (within 3 days after the formation of hemophthalmos);
- Plastic iridocyclitis (at the stage of subsiding of inflammatory phenomena);
- Cicatricial changes in the skin (cutaneous).
Contraindications
Absolute:
- The presence of defects in the sclera;
- Combination of simblepharon with a corneal ulcer;
- The presence of signs of blood coagulation in the vitreous body;
- Incomplete process of corneal epithelization after a burn;
- Vitreous hemorrhage less than 1/2 of its volume;
- Age under 18;
- Pregnancy and lactation;
- Hypersensitivity to collalizin (manifested by a pronounced reaction after the introduction of 1 KE).
Instructions for the use of Collalizin: method and dosage
Collalizin in ampoules is used externally, injected subconjunctivally, as well as directly into the lesion (scar, adhesion, vitreous body, etc.) using electro- or phonophoresis.
Before use, the contents of the ampoule are dissolved in water for injection, 0.5% novocaine solution or 0.9% sodium chloride solution.
Before using Collalizin, the patient's sensitivity to collagenase is checked. To do this, 1 KE of the drug is injected under the conjunctiva of the sick eye and the patient is monitored for 48 hours. Treatment is carried out only in the absence of an allergic reaction.
If necessary, the cutaneous application of Collalizin is checked for its tolerance by the skin; for this, the solution in a therapeutic dose is applied for 24 and 48 hours.
Schemes for using Collalizin:
- Prevention of post-burn simblepharon: 10 KE in a solution of novocaine 0.5% is injected into the tissues of the burnt vault after complete epithelialization of the cornea. Subsequent injections are made at intervals of 2-3 days (so that the reactive phenomena on the previous injection subside). The course of treatment consists of 5-7 injections, the total dose is 60-70 KU, the duration of therapy is 2-3 weeks;
- Treatment of simblefaron, keloid and other eyelid scars: 100 KE in a solution of novocaine 0.5% is injected immediately under the conjunctiva into the adhesion between the eyeball and the eyelid or into the thickness of the scar. The procedure is carried out with careful visual control using a binocular loupe. After 2-5 days, after the reactive phenomena subsided, a second injection is made. The course of treatment consists of 3 injections, the total dose is 300 KE, the duration of therapy is 3 weeks;
- Treatment of narrowing of the lacrimal passages: 50 KE in 2 ml of novocaine 0.5% solution is used for washing. The course of treatment consists of 7-10 procedures. If necessary, repeat the therapy after 2-3 weeks;
- Treatment of traumatic vitreous hemorrhage: 1 CU in 0.2 ml of 0.9% sodium chloride solution is injected once into the central zone of the vitreous through a scleral incision in the area of the flat part of the ciliary body;
- Treatment of opacity of the vitreous body, formed hemophthalmus, cicatricial process in the retina: 10 KU in 0.2 ml of novocaine 0.5% solution is administered daily or every other day subconjunctivally (under the conjunctiva of the eyeball) or retrobulbar every other day (under Tenon's capsule). The course of treatment consists of 7-10 injections, the total dose is 70-100 KU. If necessary, repeat the course of therapy after 2-3 weeks;
- Treatment of fresh growing keloid scars, hypertrophic scars, formed scars no more than 1 year old, as well as prevention of scar formation after excision of keloid scars (after epithelialization of the wound surface or complete wound healing) in patients with an increased tendency to excessive proliferation of connective tissue: Collalizin is used by electrophoresis. Aqueous solutions of Collalizin are injected every other day from the positive electrode for 20 minutes using a current strength of 0.03-0.1 mA per cm 2… The course of treatment includes no more than 15 procedures. A total of 2 or 3 courses are carried out at 2-month intervals. Doses of Collalizin are selected individually depending on the size, stage of development and clinical manifestations of the scar: hypertrophic scars - 500-1000 KU in 10 ml of injection water, total dose - 22,500-45,000 KU; fresh growing keloid scars, formed scars more than 1 year old, prevention of the formation of cicatricial changes after excision of keloid scars - 1000-2000 CU in 10 ml of injection water, total dose for 3 courses of treatment - 45 000-90 000 CU.
Treatment of corneal opacities, secondary cataracts and plastic iridocyclitis (posterior synechiae) is carried out in one of the following ways:
- Phonophoresis: 50 KE in 10 ml of water for injection is injected into the eye using the IUT 0.88-4-2 sensor of the Ultrasound T-5 apparatus. The duration of the procedure is 5 minutes at an intensity of 0.2 W / cm. The course of treatment includes 10 procedures. If necessary, a second course is carried out after 2-3 weeks;
- Electrophoresis: 50 KU in 10 ml of water for injection is injected into the eye from the positive pole using an electric current up to 1 mA. One procedure lasts up to 10 minutes. The general course of treatment consists of 10 procedures. If necessary, a second course is carried out after 2-3 weeks;
- Electrophoresis: electrophoresis is connected during phonophoresis. The duration of the procedures is 5 and up to 10 minutes, respectively. If necessary, a second course is carried out after 2-3 weeks;
- Subconjunctival administration: 10 KE in 0.2 ml of novocaine solution 0.5% is injected under the conjunctiva of the eyeball. The course of treatment includes 7-10 injections, the total course dose is 70-100 CU. If necessary, repeat the therapy after 2-3 weeks.
Outwardly, Collalizin is recommended for the prevention of scar hypertrophy in patients after dermabrasion performed for the removal of scars or facial wrinkles of the face, as well as for the prevention of scar formation after electrocoagulation for various skin neoplasms. Treatment begins in the postoperative period after epithelialization of the wound surface or complete wound healing. The drug is applied to the scar at 500 KE in 10 ml of injection water daily for 4-5 weeks. The total dose is 15,000-30,000 IU.
Side effects
Collalizine can cause allergic reactions. The formation of corneal ulcers is also possible.

Side effects can be avoided if treatment is started after the end of corneal epithelialization and a negative test for drug sensitivity is obtained.
With the development of allergic reactions, Collalizin is canceled and antiallergic therapy is carried out: local exposure to cold, instillation of corticosteroids, prescription of calcium preparations and antihistamines (diphenhydramine or suprastin).
In the event of a corneal ulcer, the use of Collalizin should be discontinued immediately, antibiotic therapy should be carried out and drugs that stimulate epithelization should be prescribed.
Application during pregnancy and lactation
According to the instructions, Collalizin is not prescribed during pregnancy / lactation.
Pediatric use
Collalizin therapy is contraindicated in patients under 18 years of age.
Drug interactions
Heavy metals, antiseptics, detergents and soaps weaken the action of collagenase.
Collalizin should not be used concomitantly with tetracyclines, gramicidin and tyrothricin.
