Release form and composition

Metrogyl is available in the following dosage forms:

Indications for use

Intravenous solution:

  • Protozoal infections: trichomonas vaginitis, giardiasis, extraintestinal and intestinal amebiasis (amoebic liver abscess, amoebic dysentery), trichomoniasis, balantidiasis, trichomonas urethritis, cutaneous leishmaniasis;
  • Infections caused by Bacteroides spp.: infections of the joints, bones, central nervous system (including meningitis), pneumonia, bacterial endocarditis, brain abscess, lung abscess, empyema;
  • Infections caused by species of Bacteroides spp., Including group B. fragilis, Clostridium spp., Peptococcus spp., Peptostreptococcus spp.: infections of the abdominal cavity (including liver abscess, peritonitis), infections of the skin, soft tissues, pelvic organs (including endometritis, endomyometritis, abscess of the fallopian tubes and ovaries, infections of the vaginal vault after surgery);

  • Anaerobic infections caused by Bac.fragilis and other bacteroids, fusobacteria, clostridia, eubacteria, anaerobic cocci;

  • Osteomyelitis;
  • Brain abscess, meningitis;

Vaginal gel:

Gel for external use:

  • Acne vulgaris;
  • Pressure sores, hard-to-heal wounds;
  • Trophic ulcers of the lower extremities with varicose veins, diabetes mellitus;
  • Hemorrhoids, fissures in the anus.

Suspension for oral administration:

  • Amoebiasis;
  • Giardiasis;
  • Anaerobic bacterial infections;
  • Amebic liver abscess.

Contraindications

Intravenous solution:

  • Organic lesions of the central nervous system, including epilepsy;
  • Diseases of the blood, including a history of them;
  • Hepatic failure (with the appointment of large doses);
  • I trimester of pregnancy and lactation;
  • Hypersensitivity to the components of the drug, as well as to other derivatives of nitroimidazole.

In the II-III trimesters of pregnancy, the appointment of Metrogyl is possible only for health reasons.

The drug should be used with caution in case of renal failure.

Tablets:

  • Organic lesions of the central nervous system, including epilepsy;
  • Diseases of the blood, including a history of them;
  • Hepatic failure (with the appointment of large doses);
  • I trimester of pregnancy;
  • Age under 12;
  • Hypersensitivity to the components of the drug, as well as to other derivatives of nitroimidazole.

Method of administration and dosage

Solution for intravenous administration:

Intravenous administration of Metrogyl is indicated in cases of severe infections and the inability to take the drug inside.

For adults and children over 12 years old, Metrogyl is prescribed in an initial dose of 500-1000 mg intravenously (infusion duration - 30-40 minutes), then 500 mg is administered every 8 hours at a rate of 5 ml per minute. After the first 2-3 infusions, with good tolerance, they switch to jet administration. The duration of therapy is 7 days. If necessary, intravenous administration can be carried out for a longer period. The maximum daily dose is 4000 mg. In the future, the patient is transferred to supportive therapy, prescribing oral administration of the drug in a daily dose of 1200 mg (3 times a day, 400 mg).

For children under 12 years old, Metrogyl is prescribed according to the same scheme, using a single dose of 7.5 mg / kg.

In the treatment of purulent-septic diseases, 1 course of treatment is usually carried out.

For prophylactic purposes, adults and children from 12 years of age, Metrogyl is prescribed intravenously, 500-1000 mg, on the eve of the operation. On the day of surgery and the next day, the drug is administered every 8 hours, 500 mg. After 1-2 days, you should switch to supportive therapy inside.

The maximum daily dose for chronic renal failure and creatinine clearance of less than 30 ml per minute and / or liver failure should not exceed 1000 mg (with a frequency of application 2 times a day).

Tablets, suspension for oral administration:

Metrogyl tablets are taken orally, without chewing, preferably during or after a meal with milk.

As a rule, adults and children from 15 years of age are prescribed 200-400 mg 2-3 times a day.

The dosage regimen is determined by the indications:

  • Trichomoniasis: single dose - 200 mg, frequency of administration - 3 times a day, course duration - 7 days; additionally, women should use metronidazole in the form of vaginal ointments or suppositories. If necessary, it is possible to carry out a repeated course of therapy (with a break of 3-4 weeks, during which repeated control laboratory tests should be carried out) or to increase the dose to 750-1000 mg per day. You can also use another regimen for taking the drug - once 2000 mg to the patient and his sexual partner;
  • Amebic liver abscess: adult daily dose - 1200-2400 mg (3 times a day, 400 or 800 mg), therapy should be carried out simultaneously with antibiotic drugs (most often tetracycline); children's daily dose - 30-35 mg / kg (in 3 divided doses); course duration - 5-10 days;
  • Amoebiasis: adult daily dose - 1200 mg (3 times a day, 400 mg); children's daily dose - 30-40 mg / kg (in 3 divided doses); course duration - 7-10 days;
  • Anaerobic bacterial infections: adult daily dose - 400-1200 mg (2-3 times a day, 200-400 mg); children's daily dose - 21 mg / kg (every 8 hours at 7 mg / kg); the duration of the course is 7-10 days.

Before carrying out surgical interventions on the large intestine and pelvic organs for the prevention of anaerobic infection, a single dose of Metrogyl is shown at a dose of 1000 mg, then - 3 times a day, 200 mg.

The daily dose of metronidazole in combination with amoxicillin (2250 mg per day) is 1500 mg (in 3 divided doses). With pronounced functional disorders of the kidneys and liver, daily doses should be reduced: metronidazole - up to 1000 mg, amoxicillin - up to 1500 mg (in 2 divided doses).

Vaginal gel:

Metrogyl is used intravaginally.

The recommended single dose is 1 full applicator (5000 mg), the frequency of application is 2 times a day (in the morning and in the evening).

The duration of therapy is 5 days.

Gel for external use:

Metrogyl should be applied in a thin layer 2 times a day (in the morning and in the evening) on previously cleansed affected skin areas.

The duration of therapy is 3-4 months, as a rule, improvement occurs 3 weeks after the start of therapy.

Side effects

Solution for intravenous administration, tablets:

  • Digestive system: pseudomembranous colitis, dry mouth, vomiting, nausea, constipation, diarrhea, intestinal colic, lack of appetite, unpleasant metallic taste in the mouth, increased liver enzymes, cholestatic jaundice, hepatitis, stomatitis, glossitis, pancreatitis;
  • Hematopoietic system: thrombocytopenia, leukopenia;
  • Urogenital system: burning sensation in the urethra, dysuria, candidiasis (fungal flora of the vagina), red-brown staining of urine, polyuria, urinary incontinence;
  • Nervous system: impaired coordination of movements, headache, hallucinations, ataxia, dizziness, epileptic seizures, impaired consciousness, increased excitability, sleep disturbances, depression, weakness, peripheral neuropathy;
  • Allergic reactions: anaphylactic shock, nasal congestion, arthralgia, skin flushing, skin rash, itching, fever, urticaria;
  • Local reactions (with intravenous administration): thrombophlebitis (pain, swelling or hyperemia at the injection site);
  • Others: flattening of the T wave on the electrocardiogram, neutropenia.

Vaginal gel:

  • Systemic reactions: dizziness, dry mouth, leukocytosis or leukopenia, headache, change in taste (including a metallic taste), loss of appetite, nausea, cramping abdominal pain, vomiting, diarrhea or constipation, dark urine staining;
  • Local reactions: burning sensation or frequent urination, vulvitis; for a sexual partner - irritation or burning sensation of the penis; in women after the end of treatment - vaginal candidiasis;
  • Allergic reactions: urticaria, skin rash.

Gel for external use:

The risk of systemic side effects when using Metrogyl topical gel is small. In rare cases, during therapy, there may be observed: allergic reactions (in the form of urticaria, skin rash), lacrimation (when applying the gel close to the eyes), peeling, hyperemia, slight burning sensation and dry skin.

special instructions

During the use of Metrogyl, alcohol should not be consumed, since this can lead to the development of a disulfiram-like reaction, manifested in the form of abdominal pain of a spastic nature, vomiting, nausea, headache, sudden flushes of blood to the face.

The simultaneous use of Metrogyl with amoxicillin is not recommended for patients under the age of 18.

The possibility of continuing treatment for leukopenia is determined by the risk of developing an infectious process.

During long-term therapy, it is desirable to monitor peripheral blood parameters.

If ataxia, dizziness and any other deterioration in neurological status appear, therapy should be discontinued.

The use of Metrogyl can immobilize treponemas and lead to a false positive Nelson's test.

During the therapy of Trichomonas vaginitis in women and Trichomonas urethritis in men, one should refrain from sexual activity and carry out simultaneous treatment of sexual partners. Treatment should not be interrupted during menstruation. At the end of the course, you need to conduct control tests for three successive cycles before and after menstruation.

After the end of the treatment of giardiasis, if symptoms persist after 3-4 weeks, it is necessary to conduct 3 fecal analyzes with interruptions of several days (in some successfully treated patients, lactose intolerance caused by invasion can persist for a long time, resembling the symptoms of giardiasis).

Avoid contact with the eyes of the vaginal and external gel. In case of accidental contact, they must be washed immediately with plenty of water.

Drug interactions

When Metrogyl is used together (inside, intravenously) with some drugs, the following effects may occur:

  • Indirect anticoagulants: increasing their action (leads to an increase in the time of prothrombin formation);
  • Ethanol: the development of its intolerance;
  • Medicines that stimulate enzymes of microsomal oxidation in the liver (phenobarbital, phenytoin): acceleration of the elimination of metronidazole (may lead to a decrease in its concentration in plasma);
  • Sulfonamides: strengthening the antimicrobial action of Metrogyl;
  • Cimetidine: inhibition of metronidazole metabolism (may lead to an increase in its concentration in the blood serum and the risk of side effects);
  • Lithium preparations (high doses): an increase in the concentration of lithium in the blood plasma with the development of signs of intoxication.

The simultaneous use of Metrogyl with depolarizing muscle relaxants (vecuronium bromide) is not recommended.

When using Metrogil topically (external and vaginal gel), the interaction with other drugs is negligible, however, during therapy, care must be taken when prescribing it with warfarin and other indirect anticoagulants.

Metrogil injection solution should not be mixed with other drugs.
