Release form and composition
Dosage form Laktinet - film-coated tablets: almost white or white, round, biconvex, marked "D" on one side, "75" on the other (28 pcs. In blisters, in a cardboard box 1 or 3 blisters).
The active ingredient is desogestrel, in 1 tablet - 0.075 mg.
Auxiliary components: colloidal silicon dioxide, D, L-α-tocopherol, stearic acid, potato starch, lactose monohydrate, povidone K30, magnesium stearate.
The composition of the film shell: opadry II white (macrogol 3000, titanium dioxide (E171) CI 77891, polyvinyl alcohol, talc).

Lactinet is a monocomponent progestogen-containing oral contraceptive that includes desogestrel, a progestogen, as an active substance, which can be taken by breastfeeding mothers, women if they have contraindications to estrogens, and also if they do not want to take contraceptives that contain estrogen.
Unlike other progestational contraceptive drugs, Lactinet only inhibits the ovulation process. This fact is confirmed by the absence of an ovulatory follicle during ultrasound examination, the absence of an increase in the values of luteotropic hormone and progesterone in the blood serum in the middle of the menstrual cycle.
Like other progestogens, desogestrel increases the viscosity of the cervical mucus, which interferes with the movement of sperm.
Pearl's index (an indicator that reflects the number of pregnancies occurring among 100 women who used contraceptives for 1 year) is 0.4. This number is comparable to that when using combined hormonal oral contraceptives.
Lactinet reduces the content of estradiol in the blood plasma to values corresponding to the early follicular phase. The drug affects lipid and carbohydrate metabolism.
Pharmacokinetics
Desogestrel is rapidly absorbed after oral administration. The maximum concentration reaches after 1.8 hours. Its bioavailability is about 70%.
It is characterized by a high bond with plasma proteins - 95–99%, mainly with albumin, to a lesser extent - with SHBG (globulin that binds sex hormones). The equilibrium concentration in plasma is established on the 4-5th day after the start of taking Lactinet.
It is metabolized by hydroxylation and dehydrogenation to form the active metabolite of etonogestrel, which, in turn, is metabolized by the formation of sulfate and glucuronide conjugates.
Etonogestrel and its metabolites are excreted as conjugates and free steroids by the kidneys and through the intestines (in a ratio of 1.5: 1).
The half-life of the drug is on average 30 hours, both after a single dose and with multiple doses.
In women who are breastfeeding, etonogestrel passes into breast milk in a ratio of 0.37–0.55. Thus, a newborn infant consuming approximately 150 mg / kg milk per day can receive 0.01–0.05 μg / kg of etonogestrel.
Indications for use
The use of Laktinet is indicated for women for oral contraception.
Contraindications
- Hepatic impairment, including history;
- Severe liver dysfunctions, including history (before recovery of liver function indicators);
- Venous thromboembolism, including deep vein thrombosis of the lower extremities and thromboembolism of the pulmonary artery (including those indicated in history);
- Suspected or diagnosed malignant hormone-dependent neoplasms, including liver and breast cancer;
- Vaginal bleeding of unknown etiology;
- Lactase deficiency, lactose intolerance, glucose-galactose malabsorption;
- Prolonged immobilization caused by severe illness or surgery;
- Confirmed pregnancy or suspected pregnancy;
- Hypersensitivity to drug components.
According to the instructions, Laktinet should be prescribed with caution to patients with resistant arterial hypertension, developing against the background of the lack of effect of antihypertensive therapy or taking the drug, systemic lupus erythematosus, chloasma (especially when indicating chloasma during pregnancy in history), porphyria, herpes (during pregnancy history), diabetes mellitus.
Instructions for the use of Laktinet: method and dosage
Lactinet is taken orally, 1 tablet 1 time per day, with a small amount of liquid.
Having determined the time of admission for herself, a woman must strictly adhere to it throughout the entire period of therapy, the interval between taking two tablets should not exceed 24 hours.
The use of Laktinet does not imply a break in taking, the next day after the end of the tablets in the blister, you must take a tablet from a new blister.
If a woman did not use oral contraceptives in the previous menstrual cycle, you can start taking Lactinet on the first day of menstrual bleeding and then follow the direction indicated on the package. In this case, additional contraceptive measures are not required.
When you start taking the drug from 2 to 5 days of menstruation, during the first 7 days of taking pills from the first blister, you should use additional contraceptives.
When switching from another oral combination contraceptive, the first Lactinet tablet must be taken the next day after the last tablet of the previous drug, without resorting to additional contraceptive measures.
When switching from a progestogenic contraceptive (mini-pili), you can start taking Laktinet any day, with an injection - on the day of the proposed next injection, with an implant - on the day of its removal. In any of these cases, you must use barrier contraception for the first 7 days.
When carrying out an abortion in the first trimester of pregnancy, it is recommended to start taking pills directly on the day of the abortion without using additional methods of contraception.
Reception of Laktinet during an abortion in the II trimester of pregnancy or after childbirth should be started in 21-28 days. In the case of a later initiation of therapy, barrier contraceptives should be used within the first 7 days. Moreover, women who have had sexual intercourse (unprotected) during the specified period should exclude pregnancy or start taking the drug on the first day of the next menstrual bleeding (when the menstrual cycle resumes).
The effectiveness of the tablets is reduced if the interval between taking two tablets is more than 36 hours.
If a woman forgot to take a pill at the set time and remembered about it within the next 12 hours, the effect of Lactinet does not decrease. The use of the following pills should be continued as usual without additional contraception.
If the delay in admission exceeds 12 hours (more than 36 hours from the moment of taking the last pill), the contraceptive effect of Lactinet may decrease. Therefore, in the next 7 days of subsequent oral contraception, it is necessary to use additional contraceptive methods.
If the delay in taking the next pill by more than 12 hours occurred in the first week of using the drug, and during this period the woman had unprotected sexual intercourse, then there is a high risk of pregnancy.
If after taking Laktinet during the first 3-4 hours the woman vomited, then the process of drug absorption was not completed. To maintain the achieved contraceptive effect, you must take the appropriate pill from a new package.
Menstrual irregularities can also occur with regular pills. If menstrual bleeding occurs irregularly and very often, you should consider using another drug or methods of contraception. If menstrual irregularities persist, an organic cause should be excluded.
The occurrence of amenorrhea during the period of use of the drug may be due to a violation of the regimen and the presence of pregnancy. If pregnancy is confirmed, you should immediately stop taking the tablets.
Lactinette does not protect against infection with sexually transmitted diseases, including HIV infection (AIDS).
Side effects
The use of Laktinet can cause side effects:
- Often: menstrual irregularities, nausea, acne, mood changes, headache, decreased libido, weight gain, breast tenderness;
- Uncommon: dysmenorrhea, vomiting, alopecia, discomfort when wearing contact lenses, fatigue, vaginitis, ovarian cysts;
- Rarely: rash, skin redness, erythema nodosum, urticaria;
- Perhaps: the exact connection with the drug intake has not been established - itching, cholestatic jaundice, chorea, cholelithiasis, herpes of pregnant women, deafness, otosclerosis, the appearance of hemolytic-uremic syndrome.

Overdose
In case of an overdose of Lactinet, spotting or bleeding from the vagina, nausea and vomiting may occur.
There is no specific antidote for desogestrel, the treatment is symptomatic.

Lactinet helps to reduce the level of estradiol in the blood to levels corresponding to the early follicular phase.
The use of the drug is shown during breastfeeding.
While taking the tablets, chloasma may develop, more often in patients with a history of chloasma of pregnant women. Women prone to chloasma are advised to avoid direct sunlight and ultraviolet radiation while taking the drug.
Due to the content of lactose monohydrate in tablets, Lactinet cannot be taken by women with such rare hereditary pathologies as lactase deficiency, lactose intolerance, glucose-galactose malabsorption.

The use of the drug does not affect the ability to drive vehicles and mechanisms.
Application during pregnancy and lactation
Lactinet is contraindicated for use during pregnancy.
In preclinical studies, due to the use of progestogen in high doses, masculinization of the female fetus was observed. In the course of epidemiological studies, no increased risk of teratogenic effect and birth defects was found in children whose mothers took contraceptives before pregnancy or unintentionally during early pregnancy.
Since Lactinet contains only progestogen, it does not affect the quantity and quality of breast milk, but a small amount of etonogestrel (the main metabolite of desogestrel) is excreted in breast milk. With a daily intake of 150 ml / kg of milk, a newborn receives 0.01–0.05 μg / kg of the drug per day. During a 7-month follow-up of newborns whose mothers took progestogen, there were no increased risks in terms of growth, physical and psychomotor development. However, it is recommended to carefully monitor the growth and development of infants if their mothers are taking Lactinet for contraception.
For violations of liver function
The appointment of Laktinet is contraindicated in patients with hepatic insufficiency, which is present or in history.
You can not take the drug if you currently have or have a history of severe liver disease until the liver function is normalized.
Drug interactions
With the simultaneous use of Lactinet with inducers of microsomal liver enzymes, such as phenytoin and other hydantoin derivatives, topiramate, rifabutin, primidone, barbiturates, griseofulvin, rifampicin, carbamazepine, oxcarbazepine, felbamate, the contraceptive action of the drug decreases and the risk of bleeding increases. If it is necessary to take these funds, you should temporarily additionally use barrier methods of contraception. Achievement of the maximum induction of microsomal liver enzymes occurs not earlier than 2-3 weeks of therapy with an appropriate inducer and continues to persist for no more than 4 weeks after stopping the intake.
With other drugs, specialized studies on the interaction of desogestrel have not been conducted.
Ampicillin, tetracycline, and other antibiotics reduce the effectiveness of oral contraceptives. Therefore, during the entire period of treatment with these drugs and 7 days after their cancellation, a woman must additionally use barrier contraceptives.
In the case of the simultaneous administration of activated charcoal, additional contraception will be needed within 7 days after its cancellation.
