Itomed is a drug that stimulates the motility of the gastrointestinal tract (GIT) by increasing the release of acetylcholine.
Release form and composition
Dosage form Itomed - film-coated tablets: biconvex, round, notched on one side; the color of the core and shell is from white to almost white (20 pcs. in blisters, in a cardboard box 2 or 5 blisters).
1 tablet weighing 138 mg contains:
- active ingredient: itoprid hydrochloride - 50 mg;
- auxiliary components, core (weight 135 mg): pregelatinized corn starch, lactose monohydrate, croscarmellose sodium, magnesium stearate, colloidal silicon dioxide;
- auxiliary components, shell (weight 3 mg): white opadry 85F18422 (partially hydrolyzed polyvinyl alcohol, macrogol-4000, titanium dioxide, talc).
Pharmacological properties
Pharmacodynamics
Enhancing gastrointestinal motility by inhibiting acetylcholinesterase and antagonism with D2-dopamine receptors, itopride activates the release of acetylcholine and stops its destruction.
The antiemetic effect of the drug is due to the interaction with D2 receptors located in the trigger zone. Itopride causes dose-dependent suppression of apomorphine-induced vomiting, and also activates propulsive motility of the gastrointestinal tract due to antagonism with dopamine D2 receptors and dose-dependent inhibition of acetylcholinesterase activity.
Itomed has a specific effect on the upper gastrointestinal tract, accelerating the transit through the stomach and improving its emptying. Serum gastrin concentrations are not affected.
Pharmacokinetics
Itopride is well and quickly absorbed in the digestive tract. The indicator of the relative bioavailability of the drug is 60%. The Cmax value (maximum plasma concentration) is 0.28 μg / ml. TCmax (time to reach maximum plasma concentration) after taking itopride 50 mg ~ 30–45 minutes. When taken again at a dose of 50-200 mg 3 times a day for 1 week, the pharmacokinetics of the active substance is linear, and its cumulation is minimal.
Itopride binds 96% to plasma proteins (mainly albumin), and less than 15% to α1-acid glycoprotein. It is actively distributed in tissues; in high concentrations it is recorded in the small intestine, stomach, kidneys, liver, and adrenal glands. The volume of distribution is 6.1 l / kg. In the brain and spinal cord, as well as in breast milk, it penetrates slightly when taking therapeutic doses.
Under the action of flavin-dependent monooxygenase, itopride is metabolized in the liver. Of the 3 identified metabolites, only one exhibits insignificant activity at the level of 2-3% of the activity of the substance.
It is excreted by the kidneys. its half-life (T½) is 6 hours, in patients with trimethylaminuria T½ increases.
Indications for use
According to the instructions, Itomed is recommended to be taken for the treatment of the following symptoms of chronic gastritis (functional non-ulcer dyspepsia): gastralgia, flatulence, discomfort in the epigastric region, heartburn, nausea / vomiting, anorexia.
Contraindications
Absolute:
- bleeding from the gastrointestinal tract;
- mechanical obstruction and perforation of the gastrointestinal tract;
- lactose intolerance, lactase deficiency, glucose-galactose malabsorption;
- period of pregnancy and lactation;
- children and adolescents up to 16 years old;
- individual hypersensitivity to any component of the drug.
Relative: the drug should be taken with caution together with cholinesterase inhibitors and M-cholinomimetics, as well as in old age with reduced liver / kidney function.
Instructions for use of Itomed: method and dosage
Itomed tablets are taken orally, before meals.
Recommended dosage: 1 pc. 3 times a day, the maximum daily dose is 150 mg (3 tablets).
For elderly patients, the dose should be reduced.
If the next dose is missed, the tablets should be taken at regular intervals, avoiding the use of a double dose to compensate for the missed one.
Side effects
- endocrine system: hyperprolactinemia, gynecomastia;
- hematopoietic organs: thrombocytopenia, leukopenia;
- digestive system: nausea, increased salivation, constipation, diarrhea, epigastric pain, jaundice;
- nervous system: sleep disturbance, dizziness, headache, irritability, tremor;
- hypersensitivity reactions: urticaria, anaphylactic shock;
- laboratory data: hyperbilirubinemia, increased activity of liver enzymes - ACT (aspartate aminotransferase), alkaline phosphatase (alkaline phosphatase), ALT (alanine aminotransferase), GGT (gamma glutamyl transferase).
Overdose
In case of overdose, gastric lavage should be performed, and symptomatic treatment should be provided to the patient.

The manifestation of symptoms of gynecomastia and galactorrhea requires immediate interruption or complete cessation of treatment.
In the course of treatment of primary biliary cirrhosis, transient decompensation of liver cirrhosis is possible, which disappears on its own after discontinuation of the drug.
Influence on the ability to drive vehicles and complex mechanisms
Taking itopride, it is necessary to refrain from potentially hazardous activities that require a high speed of psychomotor reactions and increased concentration.
Application during pregnancy and lactation
Itoprid is contraindicated for the entire period of pregnancy.
If there is a need to use the drug during lactation, breastfeeding should be interrupted or discontinued, since a slight ingestion of itopride into breast milk was recorded.
Use in the elderly
The drug should be administered with extreme caution to elderly patients with hepatic / renal impairment.
Drug interactions
When using Itomed with any other drugs, their absorption is accelerated.
Under the action of agents that reduce the acidity of gastric juice, such as cetraxate, cimetidine, teprenone, ranitidine, the prokinetic effect of the drug remains unchanged. The effectiveness of itopride is reduced by M-anticholinergics. The cholinergic effect of itopride can increase in the case of its simultaneous use with M-cholinomimetics, as well as cholinesterase inhibitors.

According to a few reviews, Itomed, when taken in recommended doses, practically does not cause side reactions (headache, abdominal pain and diarrhea were mentioned) and relieves symptoms of chronic gastritis such as heartburn, bloating and a feeling of heaviness in the stomach. Appetite improves, chronic constipation disappears. But therapy should be long-term, at least 6-8 weeks.
