Doloprokt is a drug for local use with antipruritic, local anesthetic, anti-inflammatory action.

Release form and composition

  • rectal cream 1 mg + 20 mg: a white mass with an opaque structure (10, 15, 30 or 50 g in an aluminum tube, in a cardboard box 1 tube complete with an applicator);
  • suppositories for rectal administration 1 mg + 40 mg: torpedo-shaped, yellowish-white in color, with a smooth surface (5 or 6 pcs. in a strip, in a cardboard box 1 or 2 strips).

1 g of cream contains:

  • active ingredients: fluocortolone pivalate - 1 mg, lidocaine hydrochloride (in terms of anhydrous substance) - 20 mg;
  • auxiliary components: sodium dihydrogen phosphate dihydrate, disodium edetate, cetostearyl alcohol, sodium hydrogen phosphate dodecahydrate, polysorbate 60, benzyl alcohol, sorbitan stearate, white petrolatum, liquid paraffin, purified water.

1 suppository contains:

  • active ingredients: fluocortolone pivalate - 1 mg, lidocaine hydrochloride (in terms of anhydrous substance) - 40 mg;
  • auxiliary component: solid fat (Witepsol W 35).

Pharmacological properties

Pharmacodynamics

When exposed to the skin, fluocortolone prevents marginal accumulation of neutrophils. Reducing the processes of infiltration, granulation and exudation is achieved by reducing the level of lymphokines and slowing the migration of macrophages. Suppresses inflammation, allergic skin reactions, relieves burning, itching and pain. The action of fluocortolone helps to reduce capillary dilation, interstitial edema, tissue infiltration.

The local anesthetic lidocaine, by depolarizing sodium channels, suppresses the formation and conduction of nerve impulses along afferent nerve fibers, providing the drug with an analgesic effect.

Pharmacokinetics

The local therapeutic effect is achieved at a low concentration of active substances in the blood plasma.

The absorption of fluocortolone pivalate after the administration of one suppository or a single rectal application of 1 g of cream is a maximum of 5% of the drug dose.

With the daily introduction of 2 suppositories 3 times a day for 28 days, the level of fluocortolone in the blood plasma does not have a systemic effect.

When hydrolysis of fluocortolone pivalate with esterases and enzymes of the inflammatory focus, fluocortolone, trimethylacetic acid, 11-ketofluocortolone are formed.

After rectal administration Doloprokta T 1 / 2 (half-life) from blood plasma fluocortolone is approximately 1.3 hours, and its metabolites - 4 hours. Fluocortolone is excreted from the body mainly in the urine, in the form of metabolites.

The bioavailability and absorption of lidocaine with rectal cream administration is 30%, suppositories - 24%.

T 1 / 2 of lidocaine from the plasma is 1-2 hours. The main metabolite of lidocaine, 4-hydroxy-2,6-xylidine, is formed as a result of oxidative N-dealkylation, hydroxylation of the aromatic ring, and hydrolysis of the amide bond. About 70% of the drug is excreted in the urine as this metabolite.

Indications for use

  • hemorrhoids;
  • proctitis.

In addition, the use of rectal cream is indicated for the treatment of eczema in the anus.

Contraindications

  • chickenpox, herpes zoster, vaccination reactions and other viral pathologies in the area of application;
  • syphilitic and tuberculous processes at the site of application of the drug;

  • hypersensitivity to the components of the drug.

Instructions for the use of Doloprokt: method and dosage

Doloprokt should be used after defecation and hygienic treatment of the anal canal.

The duration of treatment should not exceed 14 days.

Rectal cream

The cream is intended for external and intrarectal use.

After squeezing out a pea-sized strip of cream, use your fingertip to lubricate the area around and inside the anus.

The introduction of the drug into the rectum is carried out using an applicator screwed onto a tube. It is inserted into the anus and a small amount of cream is squeezed out by pressing.

The recommended frequency of procedures per day: at the beginning of therapy 3 times, then 2 times, morning and evening. When relief of symptoms of the disease occurs, one application per day can be limited.

Rectal suppositories

Suppositories are used intrarectally, by deep insertion into the anus.

Recommended dosage: 1 pc. 2 times a day, morning and evening. During a period of severe exacerbation of the disease during the first 3 days, the frequency of application of the drug can be increased up to 3 times a day. After the onset of a stable improvement, you can switch to the introduction of 1 suppository 1 time per day or every other day.

In case of severe inflammation of the hemorrhoids, treatment is recommended to begin with the use of a cream, abundantly lubricating and gently setting the protruding hemorrhoids back with a finger.

Side effects

Against the background of prolonged treatment (more than 28 days) with Doloproct, there is a risk of developing atrophy, stretch marks, telangiectasia and other local skin changes.

In addition, undesirable effects may appear in the form of a burning sensation at the site of application, in rare cases - allergic reactions, irritation.

Overdose

An overdose of Doloprokt may occur if you accidentally swallow a certain amount of cream or several suppositories. Symptoms depend on the dose: on the part of the cardiovascular system - suppression of heart function or cardiac arrest; from the side of the central nervous system - respiratory depression, convulsions, respiratory arrest.
